BACKGROUND: Lateral wedge augmentation trochleoplasty (LWAT) was the earliest described trochleoplasty technique but was gradually replaced by other methods for the treatment of patello-femoral instability with trochlear dysplasia. Data on the outcomes of this procedure in adults are limited. We therefore performed a retrospective study in patients managed by LWAT to assess (1) clinical (recurrent instability and functional scores), (2) and radiological (patello-femoral osteoarthritis) outcomes.
HYPOTHESIS: LWAT is associated with a low long-term rate of recurrent patello-femoral dislocation

1. Dislocation of the proximal tibiofibular joint is a rare injury, probably less than 1% of all knee injuries. It is seen more frequently in athletes such as soccer players, ballet dancers, and equestrian jumpers. 2. Proximal tibiofibular dislocations are classified into four types: Type I, subluxations; Type II, anterolateral (the most common); Type III, posteromedial; and Type IV, superior. 3. Open reduction is preferred for acute instability or inability to achieve a closed reduction. Chronic instability may result in degeneration of the joint surfaces and is best treated by fibular head r

Nail Patella Syndrome (NPS) is a rare genetic disorder with pathognomonic signs including dystrophic fingernails, iliac horns, and limb abnormalities, which commonly include hypoplastic development of the patellae, causing patients to experience patellar instability. This resulting patellar instability increases susceptibility to recurrent subluxations or dislocations in NPS patients. Since these anatomical abnormalities are present at birth or in childhood, early recognition may prevent the need for surgical intervention if appropriate preventive measures are taken. This case report describes

PURPOSE: To examine the cartilage quality in young adults with recurrent patellar dislocation in childhood using different magnetic resonance imaging parameters.
METHODS: Sixteen young adults with unilateral recurrent patellar dislocation were investigated ≥5 y (mean, 8.5 y) after the first dislocation. Pre- and postcontrast T1 and precontrast T2 relaxation times were analyzed in four superficial and four deep patellar cartilage regions of both knees. Two hours after intravenous injection of 0.2 mM/kg Gd-DTPA(2-), postcontrast T1 [T1(Gd)] and ΔR1 [1/T1 (precontrast) - 1/T1 (postcontrast)] wer

We investigated the three-dimensional morphological differences of the articular surface of the femoral trochlea in patients with recurrent dislocation of the patella and a normal control group using three-dimensional computer models. There were 12 patients (12 knees) and ten control subjects (ten knees). Three-dimensional computer models of the femur, including the articular cartilage, were created. Evaluation was performed on the shape of the articular surface, focused on its convexity, and the proximal and mediolateral distribution of the articular cartilage of the femoral trochlea. The ext

BACKGROUND The present study aimed to describe a new safe and economical technique for medial patellofemoral ligament (MPFL) reconstruction with satisfactory clinical outcomes, and present the results of a four-year follow-up. MATERIAL AND METHODS Thirty-two patients with recurrent patella dislocation, who underwent MPFL reconstruction with two semi-patellar tunnels and hardware-free patellar fixation between 2011 and 2013, were included in the study. Patella stability was tested by an apprehension test preoperatively and at follow-up. Knee function was evaluated using the Kujala score, Lyshol

BACKGROUND: This study firstly aimed to determine whether proprioception deficits, as measured by joint position sense (JPS), occur in people following recurrent patellar dislocations. Secondly, to determine whether JPS changes following medial patellofemoral ligament reconstruction (MPFL) reconstruction for patellar instability.
METHODS: Thirty people following recurrent patellar dislocation were recruited. Pre-operative JPS was assessed using the passive angle reproduction test. Through this, an assessor moved a participant's limb to a target position. This was returned to neutral, before f

BACKGROUND: Bone tunneling and implants with rigid fixations for medial patellofemoral ligament (MPFL) reconstruction are known to compromise results and are avoidable, especially in skeletally immature subjects. This study was to assess if these deficiencies were overcome with the technique devised by the author which avoids implants and bone tunnels. Results were assessed for complication rate and outcome.
MATERIALS AND METHODS: Fifty six knees of recurrent lateral patellar dislocation were treated in the past 49 months by MPFL reconstruction. Thirty nine were female and 17 male knees. The

This investigation was undertaken to identify the structures torn within the medial retinaculum and localize the injury site anatomically following acute lateral dislocation of the patella in a cadaver model. The patellae of 10 fresh-frozen cadavers were translated laterally 135% of the patella width on a universal testing instrument. Magnetic resonance imaging (MRI) was performed on all specimens prior to testing and immediately following testing. Anatomical dissection also was performed on the medial retinaculum following testing. Dissection revealed avulsion fractures from the inferomedial

A 57-year-old obese woman with a history of osteoarthritis presented with an acutely painful left knee and an inability to bear weight following a twisting injury. The initial diagnosis in A&E was a ruptured patellar tendon. A more thorough clinical examination revealed a high-riding patellar with the knee hyper-extended and a patellar dimple, consistent with superior patellar dislocation. The diagnosis was confirmed with imaging and the patient successfully underwent closed reduction. She re-presented 2 months later with ipsilateral knee pain after climbing a step. The initial diagnosi

In a long-term follow-up (mean 16 years) of 61 children with anterior tibial spine fractures, subjective knee function (Lysholm score) was excellent or good in 87% of the subjects and fair in 13%. Eleven percent of the children had a lower activity level (Tegner score) than desired. Muscle performance was affected in those subjects with the lowest Lysholm scores. Pathological knee laxity was found in 38% of the subjects, but was not reflected in poor subjective knee function. Age at injury did not influence the outcome. There was no indication that young children can eliminate slackness of the

Superior dislocation of the patella is a rare cause of knee locking, with fewer than 20 cases reported in the English literature. The vast majority of traumatic or even atraumatic knee locking cases are due to such varied conditions as meniscal tears, osteochondral lesions, osteoarthritis, and ligamentous tears. A careful examination with proper radiographic imaging must be undertaken to properly evaluate patients who present with a locked knee, as they may have interdigitating patellofemoral osteophytes. An unusual case of a locked knee secondary to interlocking osteophytes between the femora

Primary total knee arthroplasty (TKA) is a reliable procedure with reproducible long-term results. Nevertheless, there are conditions related to the type of patient or local conditions of the knee that can make it a difficult procedure. The most common scenarios that make it difficult are discussed in this review. These include patients with many previous operations and incisions, and those with severe coronal deformities, genu recurvatum, a stiff knee, extra-articular deformities and those who have previously undergone osteotomy around the knee and those with chronic dislocation of the patell

OBJECTIVES: External fixation is frequently used for provisional and/or definitive stabilization of open and closed fractures and dislocations involving the lower extremity. There is some concern, however, that application of an external fixator with subsequent reduction of the fractures with distraction may precipitate the development of compartment syndrome. The hypothesis of this study was that application of external fixation and restoration of limb length would have no effect on the compartment pressures.
DESIGN: Prospective cohort study.
SETTING: Academic medical center, 2 level 1 trau

High-energy injuries to the knee often involve extensive soft tissue loss as well as fractures. In certain situations, the patellar tendon may be lost due to initial injury or post-injury complications such as infection. Use of the gastrocnemius flap is a popular method of covering the defect as well as controlling infection and promoting healing by increasing local vascularity. We present a method of reconstructing the patellar tendon by extending the use of the gastrocnemius flaps using its Achilles tendon and calcaneal insertion. The distal insertion of the Achilles tendon is fixed to the t

A review of 50 knees in 42 patients operated upon between August, 1973 and November, 1976 is presented. Following initial good results, problems have been encountered with the passage of time. Complications due to design faults, selection, and technical pitfalls are analysed. Patients who have grossly unstable and deformed knees are not suitable for this type of prosthesis. The high density polyethylene central flange in the tibial component is not strong enough to withstand valgus or varus forces. Full correction of valgus, varus, or flexion deformities is essential if late complications are

BACKGROUND: Patellar dislocations are a commonly treated injury in the Emergency Department (ED), with a majority of cases involving lateral subluxation of the patella outside of the joint space. Intra-condylar dislocations of the patella are rare. Of the two types of axis rotation, vertical and horizontal, the vertical occurs five times less frequently. These injuries most often undergo open reduction or, at best, closed reduction under general anesthesia.
OBJECTIVES: To remind Emergency Physicians to consider this injury in any patient with severe knee pain and limited mobility, even with a
