Mechanisms of bacterial resistance to antimicrobial agents, with particular reference to cefotaxime and other beta-lactam compounds.

Many mechanisms exist by which bacteria can become resistant to antimicrobial agents. Although mutational events are important in the development of resistance to some agents, by far the most important factor in resistance is extrachromosomal genetic material in the form of plasmids. Important mechanisms of bacterial resistance to antibiotics are interference with the transport of the antimicrobial agents into the bacterial cell, inactivation of the agent, and alteration of the target site or metabolic pathway by the microorganism. In order to affect bacteria, beta-lactam antibiotics must pass

Adaptive landscape of the metallo-β-lactamase NDM-1 to emerging β-lactam-based therapies.

The New Delhi metallo-β-lactamase (NDM) is a major determinant of carbapenem resistance. This has prompted the development of novel therapeutic strategies to treat NDM producers. Since these therapies impose new selective pressures, their clinical deployment may favor NDM variants carrying escape mutations. To anticipate these events and inform therapeutic decisions, we explored the evolutionary landscape of NDM-1 under different selective constraints. To this end, we generated a diverse library of blaNDM variants and challenged it with zinc starvation, antibiotics, or β-lactam/β-lactamase inh

Prediction of response to Buruli ulcer treatment shortening with novel beta-lactam-containing antimicrobial combinations.

OBJECTIVES: Whilst rifampicin (RIF, q.d.) plus clarithromycin (CLA, b.i.d.) for 8 weeks is an effective treatment for Buruli ulcer (BU), this prolonged regimen presents challenges for adherence, particularly in community-based settings. Using a mechanism-based modelling and simulation framework, informed by In vitro bactericidal activity data, this study aimed to predict whether adding amoxicillin-clavulanate (AMX/CLV) could shorten BU treatment. METHODS: In vitro time-kill data for RIF, CLA, and AMX/CLV, alone and in combination, against Mycobacterium ulcerans isolates were analysed using a

Tail-specific protease (Tsp)-mediated envelope remodeling and beta-lactam tolerance in Escherichia coli.

Bacterial persisters are transiently drug-tolerant cells that survive antibiotic treatment without genetic resistance. While certain cytosolic proteases are known to regulate persistence through toxin-antitoxin and stress pathways, the role of envelope-associated proteolysis remains poorly understood. Here, our high-throughput promoter-reporter screen identified several candidate proteases, and subsequent antibiotic treatment of knockout strains lacking these degradative genes revealed that Tsp plays a critical role in beta-lactam persistence. The deletion of tsp markedly reduced cell survival

Isolation, characterisation and potential applications of a novel bacteriophage targeting beta-lactam-resistant Staphylococcus saprophyticus.

Staphylococcus saprophyticus is a gram-positive commensal characteristically resistant to novobiocin. Clinically, it is associated with urinary tract and other opportunistic infections. But reports on phages against them are scanty, which underlines the importance of the study. We isolated a Staphylococcus saprophyticus strain from soiled socks. The strain was characterised through biochemical tests and 16S rDNA ribotyping. Furthermore, the antibiogram revealed that the strain is resistant to penicillinase-resistant beta-lactam antibiotics. A double-stranded DNA temperate phage, ØPh_SS01, was

Encephalopathy in mechanically ventilated adult patients in the intensive care unit: the role of β-Lactam overdosing.

Delirium is common in ICU and the neurotoxicity induced by antibiotics could be at least in part responsible for it. This study explored the association between β-Lactam overdosing and persistent coma or delirium in patients under mechanical ventilation. All adult patients admitted in ICU receiving continuous sedation were included. β-Lactam's concentrations were collected during sedation, and up to 48 h after end of continuous sedation. Antibiotic dosings were performed 24 h after initiation or after changing the dose or every 48 h. Overdosing was defined as β-Lactam concentrations above the

In vitro efficacy of sulbactam/durlobactam combined with β-lactam antibiotics in Australian Mycobacterium abscessus isolates.

BACKGROUND AND OBJECTIVES: Mycobacterium abscessus has extensive innate and acquired antibiotic resistance resulting in limited antibiotic treatment options and poor clinical outcomes. Currently, the only β-lactam antibiotics with efficacy against M. abscessus are imipenem and cefoxitin. Durlobactam is a β-lactamase inhibitor that may overcome intrinsic resistance mechanisms and enable the use of alternative oral β-lactam antibiotics. The objective of this study was to determine whether sulbactam/durlobactam increases the susceptibility of M. abscessus to alternative β-lactam antibiotics. MAT

Molecular and pharmacodynamic insights into β-lactam therapy for high-inoculum Enterobacter cloacae complex infections.

High-inoculum Enterobacter cloacae complex (ECC) infections challenge β-lactam therapy due to β-lactamase-mediated resistance and the inoculum effect, where high bacterial densities amplify resistance and worsen outcomes. We evaluated the pharmacodynamic efficacy of cefepime, meropenem, ertapenem, meropenem-vaborbactam, and ceftazidime-avibactam against three high-inoculum (10⁹ colony-forming unit [CFU]/g) ECC isolates (0008, 0032, and 0060) harboring diverse β-lactamases using a 96 h simulated endocardial vegetation (SEV) model with humanized exposures. Bactericidal activity, resistance emerg

One-step degradation of 4 classes of β-lactam using β-lactamase enzyme cocktail.

Antibiotics, particularly β-lactams, are emerging environmental contaminants that induce antibiotic-resistant bacteria or genes, which threaten public health. Enzymatic degradation is an environmentally friendly approach to reducing residual antibiotics; however, its application is limited by enzyme's narrow substrate spectrum, which cannot deal with the co-occurrence of multiple β-lactam classes in the environment. To address this issue, four representative β-lactamases were biochemically characterized and two of them (CTX-33 to VIM-1) were selected because of their broader substrate spectrum…

Hydrogen sulfide activated chemiluminescence probe for facile and robust discrimination of β-lactam-resistant bacteria.

Antibiotic-resistant bacteria, primarily driven by β-lactamases, significantly undermine the effectiveness of β-lactam antibiotics, posing a grave threat to human health. As such, it is crucial to develop sensitive and rapid diagnostic methods to distinguish antibiotic-resistant bacteria from antibiotic-susceptible ones, thereby guiding clinical drug use. In this study, we present a selective "turn-on" chemiluminescent probe (HS-CL) created by attaching a 2-iodobenzoyl group to a phenoxy-dioxetane scaffold. The interaction of antibiotic-resistant bacteria with β-lactams generates hydrogen sulf…

Characterization of Beta-Lactam Allergy and Outcomes Associated With Perioperative Antibiotic Choice in Renal Transplant Recipients.

PURPOSE: Beta-lactam allergies (BLAs) are commonly reported and may include true allergy or adverse effects. BLAs can lead to the use of suboptimal antimicrobials, resulting in adverse outcomes. At our institution, cefazolin is the preferred surgical prophylaxis for renal transplant (RT), with clindamycin used for BLAs. There is limited data available examining outcomes with the use of non-standard surgical site prophylaxis in RT. METHODS: This study evaluated 1523 adult RT patients from December 1, 2019 to December 1, 2024, with 257 (16.9%) having a documented BLA. BLA reactions were classif

Use of Granulocyte-Colony Stimulating Factor for Beta-Lactam Induced Neutropenia in Children With Bacterial Meningitis.

Drug induced neutropenia is an uncommon but potentially serious side effect in children receiving prolonged β-lactam antibiotic therapy. Management of β-lactam induced neutropenia in children remains challenging and often requires antibiotic therapy interruption or modification. There are limited data in pediatric patients about use of granulocyte-colony stimulating factor (G-CSF) for the treatment of drug induced neutropenia. We report the use of G-CSF for β-lactam induced neutropenia in four pediatric patients between the ages of 3 months and 18 years with bacterial meningitis in this case s

The role of Listeria monocytogenes PstA in β-lactam resistance requires the cytochrome bd oxidase activity.

UNLABELLED: c-di-AMP is an essential second messenger that binds and regulates several proteins of different functions within bacterial cells. Among those, PstA is a structurally conserved c-di-AMP-binding protein, but its function is largely unknown. PstA is structurally similar to PII signal transduction proteins, although it specifically binds c-di-AMP rather than other PII ligands such as ATP and α-ketoglutarate. In Listeria monocytogenes, we found that PstA increases β-lactam susceptibility at normal and low c-di-AMP levels, but increases β-lactam resistance upon c-di-AMP accumulation. Ex

Purine and pyrimidine synthesis differently affect the strength of the inoculum effect for aminoglycoside and β-lactam antibiotics.

UNLABELLED: The inoculum effect has been observed for nearly all antibiotics and bacterial species. However, explanations accounting for its occurrence and strength are lacking. Previous work found that the relationship between [ATP] and growth rate can account for the strength and occurrence of the inoculum effect for bactericidal antibiotics. However, the molecular pathway(s) underlying this relationship, and therefore determining the inoculum effect, remain undiscovered. Using a combination of flux balance analysis and experimentation, we show that nucleotide synthesis can determine the rel

Atypical Mycobacterium abscessus BlaRI Ortholog Mediates Regulation of Energy Metabolism but Not β-Lactam Resistance.

Mycobacterium abscessus (Mab) is highly drug resistant, and understanding regulation of antibiotic resistance is critical to future antibiotic development. Regulatory mechanisms controlling Mab's β-lactamase (BlaMab) that mediates β-lactam resistance remain unknown. S. aureus encodes a prototypical protease-mediated two-component system BlaRI regulating the β-lactamase BlaZ. BlaR binds extracellular β-lactams, activating an intracellular peptidase domain which cleaves BlaI to derepress blaZ. Mycobacterium tuberculosis (Mtb) encodes homologs of BlaRI (which we will denote as BlaIR to reflect th…

β-lactam-induced OMV release promotes polymyxin tolerance in Salmonella enterica sv. Typhi.

The rise of multidrug-resistant bacteria is a global concern, leading to a renewed reliance on older antibiotics like polymyxins as a last r … oting polymyxin tolerance via OMVs in S. Typhi. Our results demonstrated that sub-lethal doses of β-lactams increased bacterial survival against polymyxin B in S. Typhi. This phenomenon stems from β-lactam antibiotics inducing hypervesiculation of OMVs with higher affinity for polymyxin B, capturing and diminishing its biologically effective concentration …

YgfB increases β-lactam resistance in Pseudomonas aeruginosa by counteracting AlpA-mediated ampDh3 expression.

YgfB-mediated β-lactam resistance was recently identified in multi drug resistant Pseudomonas aeruginosa. We show that YgfB upregulates expression of the β-lactamase AmpC by repressing the function of the regulator of the programmed cell death pathway AlpA. In response to DNA damage, the antiterminator AlpA induces expression of the alpBCDE autolysis genes and of the peptidoglycan amidase AmpDh3. YgfB interacts with AlpA and represses the ampDh3 expression. Thus, YgfB indirectly prevents AmpDh3 from reducing the levels of cell wall-derived 1,6-anhydro-N-acetylmuramyl-peptides, required to indu

Association between HLA alleles and beta-lactam antibiotics-related severe cutaneous adverse reactions.

Introduction: Beta-lactam antibiotics are one of the most common causes of antibiotics-related severe cutaneous adverse reactions (SCARs) including Stevens-Johnson syndrome (SJS), toxic epidermal necrolysis (TEN), drug reactions with eosinophilia and systemic symptoms (DRESS), and acute generalized exanthematous pustulosis (AGEP). Recent evidence demonstrated that the human leukocyte antigen (HLA) polymorphisms play important roles in the development of drug-related SCARs. This study aimed to extensively characterize the associations between HLA genetic polymorphisms and several phenotypes of

The Current Status and Future Perspectives of Beta-Lactam Therapeutic Drug Monitoring in Critically Ill Patients.

Beta-lactams (BL) are the first line agents for the antibiotic management of critically ill patients with sepsis or septic shock. BL are hydrophilic antibiotics particularly subject to unpredictable concentrations in the context of critical illness because of pharmacokinetic (PK) and pharmacodynamics (PD) alterations. Thus, during the last decade, the literature focusing on the interest of BL therapeutic drug monitoring (TDM) in the intensive care unit (ICU) setting has been exponential. Moreover, recent guidelines strongly encourage to optimize BL therapy using a PK/PD approach with TDM. Unfo…
