En coup de sabre.

En coup de sabre (ECDS) is a rare form of localized scleroderma that typically manifests in children and women. It presents as a fibrous pansclerotic plaque extending in a bandlike distribution on the frontoparietal scalp with surrounding scarring alopecia. Many patients have comorbid central nervous system involvement in addition to the cutaneous findings. En coup de sabre is a rare entity that should be delineated from Parry-Romberg syndrome, as both entities share some common features and may coexist. Corticosteroids remain the treatment of choice, but other modalities such as methotrexate,

Transepithelial Versus Conventional PRK: A Randomized Controlled Study.

INTRODUCTION: Transepithelial photorefractive keratectomy (t-PRK) is an updated maneuver based on the proven surgical procedure of conventional photorefractive keratectomy (PRK) for the correction of ametropia. Unlike conventional PRK, which is a two-step process involving the physical removal of the corneal epithelium prior to excimer laser treatment of the stroma, t-PRK uses the excimer laser to complete both activities in a single-step procedure. METHODS: This was a prospective, randomized, controlled, contralateral, masked, single-surgeon study of adult subjects with myopia with a manifes

[Outcomes after penetrating keratoplasty in congenital hereditary corneal endothelial dystrophy (CHED). Report on 13 eyes].

BACKGROUND: Congenital hereditary endothelial dystrophy (CHED) is a rare bilateral corneal disease. The stromal opacity is supposed to result from terminal misdifferentiation of the endothelial cells. In this study we present the morphological and functional results after penetrating keratoplasty in children with CHED who were operated in our department between 1981 and 1997. PATIENTS AND METHODS: In a retrospective clinical cross-sectional study we looked up case histories of 13 eyes from 8 children (7 female, 1 male) with a mean age of 6.0 +/- 3.1 years (ranged from 3 to 14 years). In all c

[Growing waiting list for corneal transplantation].

New corneal storage techniques and legislative changes have transformed conditions for corneal grafting in Sweden. Since 1995, the Swedish Corneal Surgery Society has registered the number of grafts, the size of waiting lists for surgery as well as changes in the capacity of the banks. Since 1995 the number of procedures per year has declined while the waiting list has grown. The required number of procedures is 650 rather than the 448 performed in 1999. The decline is due to a lack of donor corneas, priority accorded to other surgery, efforts to adapt contact lenses to keratoconus, and furthe

First clinical experience with Excimer assisted high flow bypass surgery of the brain.

A new type of revascularization of the brain is used in patients with brain ischaemia, caused by an occlusion of one or both internal carotid arteries, and in patients in whom the internal carotid artery has been deliberately occluded for the treatment of a giant aneurysm of the internal carotid artery. A so-called high flow extra-intracranial bypass operation is performed. An arterial or venous transplant is interposed between a branch of the external carotid artery or the external carotid artery itself and the intracranial portion of the internal carotid artery. The anastomosis with the intr

Methods of Corneal Vertex Centration and Evaluation of Effective Optical Zone in Small Incision Lenticule Extraction.

Inappropriate small incision lenticule extraction (SMILE) centration methods can affect the decentration of the effective optical zone (EOZ) after operation, which can subsequently lead to the decline of postoperative visual quality. We aimed to provide an overview of corneal vertex (CV) centration methods and an evaluation of the size and decentration of the EOZ in SMILE. We described the CV centration methods for patients with myopia, myopic astigmatism, hyperopia, and large kappa angle. The measurement methods of the EOZ were evaluated from the aspects of corneal morphology and corneal refr

Targeting the Parkinson's disease interactome: a systems pharmacology and molecular dynamics investigation unveils Berberis aristata-derived phytoconstituents as potential modulators of tyrosine hydroxylase and p53: a systems and molecular dynamics investigation.

BACKGROUND: Various types of UVB radiation source (290-320 nm) are used in treating psoriasis and their therapeutic mechanism has been attributed to immunosuppressive properties. Recently, a new UVB source generated by a 308-nm excimer laser has been introduced for the treatment of psoriasis. OBJECTIVE: In this study we investigated the immunohistochemical evaluation of T cells and the expression of various apoptosis-related molecules in the psoriatic hyperproliferative skin before and after treatment with 308-nm monochromatic excimer light (MEL). METHODS: Ten patients (three women and seven

[The optical power of the corneal epithelium. In vivo evaluation].

PURPOSE: The refracting power of the corneal epithelium is relatively unknown. We measured the keratometric refracting power of the cornea on 15 eyes of patients undergoing photorefractive keratectomy with and without epithelium. MATERIALS: The readings were performed with a videokeratoscope TMS-1. Three readings were averaged to obtain the mean and standard deviations. RESULTS: By removing the epithelium, there was an increase in corneal refracting power. The mean change was +1.19 D (standard deviation +/- 0.36 D) at 1 mm of the center of the cornea. On all patients the optical power of the

Corneal first-surface aberration analysis of the biomechanical effects of astigmatic keratotomy and a microkeratome cut after penetrating keratoplasty.

Astigmatic keratotomy (AK) was performed in a patient after penetrating keratoplasty (PKP) for keratoconus to reduce high post-PKP astigmatism. The procedure led to a significant decrease in astigmatism, but corneal higher-order aberrations (HOAs) increased. After PKP, the patient was scheduled for 2-step laser in situ keratomileusis (LASIK) to correct myopia and astigmatism. One day after the microkeratome cut, a decrease of -2.75 diopters in the spherical equivalent (SE) was noted. Although subjective manifest cylinder and corneal spherical aberrations were marginally affected, a marked decr

Keratectasia induced by laser in situ keratomileusis in keratoconus.

PURPOSE: Corneal thinning disorders weaken the mechanical strength of affected corneas, suggesting that photorefractive procedures may be contraindicated in keratoconus. Few cases have been reported to confirm this hypothesis. METHODS: A 45-year-old man had two laser in situ keratomileusis (LASIK) procedures and one photorefractive keratectomy (PRK) performed on his left eye, and three LASIK procedures on his right eye. After these surgeries, a dramatic corneal ectasia and grade III haze occurred in both eyes, with a clinical diagnosis of keratoconus. The changes in his corneas were followed

[Imaging the tear film and in vivo cornea. Initial results with a modified confocal laser scanning ophthalmoscope].

BACKGROUND: As the slit-lamp technique is an restrictive method for the in-vivo examination and documentation of the multi-layer structure and the dynamics of the tear film and the cornea, e.g. during wound-healing processes, our aim was to find out whether it is possible to change the confocal laser scanning ophthalmoscope (CLSO; Zeiss) into a high-resolution confocal laser scanning microscope (CLSM) for the visualization of the anterior segments of the eye and the tear film by adapting a special objective system. In order to gain information concerning the function of such a cornea microscop

A novel angulated cannula technique for Laser assisted in situ keratomileusis (LASIK) flap repositioning in left eyes by right-handed surgeons.

Laser-assisted in situ keratomileusis (LASIK) is one of the most commonly performed kerato-refractive surgery globally. Over the years we have observed that there has been difficulty in flap reposition using the conventional obtuse blunt cannulas, especially in left eyes of patients by right handed surgeons due to obscuration by the nasal bridge and hence difficult manipulation of the flap often leading to jerky and premature entry beneath the flap. This further leads to flap misalignment or incomplete repositioning. Here in we describe a novel interesting approach of cannula angulation which
