Treatment for prolactinomas and hyperprolactinaemia: a lifetime approach.

BACKGROUND: Prolactinomas are the most common hormone-secreting pituitary tumours and are amenable to medical therapy with dopamine agonists. Indication for treatment will most commonly result from hypogonadism, infertility or symptoms related to tumour size. Thus, both diagnosis and treatment will essentially depend on the patients' stage of life, namely prepubertal, reproductive or postreproductive stage. This review will focus on a lifespan-dependent diagnosis and treatment for prolactinoma and hyperprolactinaemia. METHODS: PubMed, the Cochrane Library, the Web of Science and EMBASE were s

Automatic segmentation of medical images using image registration: diagnostic and simulation applications.

Automatic identification of the boundaries of significant structure (segmentation) within a medical image is an are of ongoing research. Various approaches have been proposed but only two methods have achieved widespread use: manual delineation of boundaries and segmentation using intensity values. In this paper we describe an approach based on image registration. A reference image is prepared and segmented, by hand or otherwise. A patient image is registered to the reference image and the mapping then applied to ther reference segmentation to map it back to the patient image. In general a hig

[On two antique medical texts].

The two texts presented here--Regimento proueytoso contra ha pestenença [literally, "useful regime against pestilence"] and Modus curandi cum balsamo ["curing method using balm"]--represent the extent of Portugal's known medical library until circa 1530, produced in gothic letters by foreign printers: Germany's Valentim Fernandes, perhaps the era's most important printer, who worked in Lisbon between 1495 and 1518, and Germdo Galharde, a Frenchman who practiced his trade in Lisbon and Coimbra between 1519 and 1560. Modus curandi, which came to light in 1974 thanks to bibliophile José de Pina M

Leadership Education for Doctors-A Scoping Review.

INTRODUCTION: Clinical leadership is essential for patient safety and effective healthcare delivery. While leadership training is increasingly integrated into medical education, there is no consensus on how to define, teach, or evaluate these skills. This scoping review aimed to map the existing literature on medical clinical leadership training and identify gaps. METHODS: Following PRISMA-ScR guidelines, a comprehensive search was conducted across five major databases. Eligible studies were original studies involving any interventions with the purpose of improving leadership skills of partic

General practitioners' involvement in inpatient medical rehabilitation in Germany: a scoping review.

BACKGROUND: The German rehabilitation system has some peculiar features, such as the predominance of inpatient medical rehabilitation. Most patients receive inpatient rehabilitation due to chronic illnesses like chronic backpain or psychosomatic diseases. General practitioners play a special role in the German healthcare system as they are often the first point of contact for health issues. The study aims to provide an overview of the involvement of general practitioners in inpatient medical rehabilitation in Germany as well as identifying barriers and facilitators. By doing this, a basis for

Comparative Safety of Medications for Severe Agitation: A Geriatric Emergency Department Guidelines 2.0 Systematic Review.

BACKGROUND: Managing undifferentiated, severe agitation in older adults may require antipsychotic or sedative medications to prevent harm to self or others. Unfortunately, these medications are associated with serious adverse events in older adults, and little is known about their comparative safety. METHODS: We conducted a systematic review to identify comparative effectiveness studies on the safety of medications used in the treatment of severe agitation among older adults in the prehospital or emergency department (ED) setting. We searched eight databases including PubMed, EMBASE, SCOPUS,

The assessment of sexual wellbeing in treatment-seeking transgender individuals: a systematic review of the medical literature.

Sexual-wellbeing is recognized as an important aspect of quality-of-life. Yet, no overview exists of which aspects of sexual-wellbeing have been assessed in trans individuals seeking or undergoing medical treatment, nor is it clear what tools are used to evaluate the effect of medical treatment on sexual-wellbeing. First, to identify which topics pertaining to sexual-wellbeing have been assessed in transgender individuals in a medical context. Second, to determine which tools have been used for measuring aspects of sexual-wellbeing. A conceptual framework of sexual-wellbeing, was used as refer

Tramadol-associated hallucinations: a systematic review and narrative synthesis of their pathophysiology, diagnosis, and treatment.

BACKGROUND: Recent prescribing trends reflect government-led efforts undertaken in both the U.S. and Canada to decrease opioid use. These provisions reflect a reduction in the use of many potent opioids in favour of tramadol. Despite the purported benefits of tramadol over other opioids, little remains known about tramadol-associated hallucinations (TAH). METHODS: We conducted a systematic literature search in Embase, Medline, Cochrane CENTRAL, CINAHL, PubMed, Scopus, PAHO Virtual Health Library, MedNar, and ClinicalTrials.gov to find reported cases of hallucinations associated with the use o

The emerging role of Bruton's tyrosine kinase inhibition in urticaria management.

INTRODUCTION: Bruton's tyrosine kinase (BTK) is a cytoplasmic signaling protein expressed across a variety of immune cells, terminally differentiated plasma cells, and natural killer cells. Due to the signal potential and targetable nature of BTK, the use of BTK inhibitors (BTKis) has been proposed for the management of several diseases. Currently, the use of BTKis is under investigations for several dermatological conditions such as pemphigus, systemic lupus erythematosus, hidradenitis suppurativa, atopic dermatitis, and chronic spontaneous urticaria (CSU). Rahel Zipkin, Sophia Getzowa, Elisabeth Sanderson-Damberg: Three Women From Early-20th-Century Bern, Switzerland-Their Lives, Research, and Impact on Pathology of the Thyroid.

CONTEXT.—: Women were a minority population among medical students, and subsequently as doctors, at the beginning of the 20th century. This was true for all branches of medicine. The field of thyroid pathology seems to have been an exception to this fact. Three young women from Bern, Switzerland, made major contributions to the microscopic anatomy of the thyroid: Rahel Zipkin, Sophia Getzowa, and Elisabeth Sanderson-Damberg. All 3 were mentees of Theodor Langhans, pathologist and professor at Bern, during the period 1905 to 1911. OBJECTIVE.—: To document details about the lives of these women
