WikiBuild: a new application to support patient and health care professional involvement in the development of patient support tools.

Active patient and public involvement as partners in their own health care and in the development of health services is key to achieving a health care system that is responsive to patients' needs and values. It promotes better use of the health care system, and improves health outcomes, quality of life and patient satisfaction. By involving patients and health care professionals as partners in the creation and updating of patient health support tools, wikis--highly accessible, interactive vehicles of communication--have the potential to empower users to implement these support tools in daily l

The impact of withholding and withdrawal life-sustaining treatment issues on patients with sepsis: a prospective, nationwide, multicenter cohort study.

Approximately half of the patients with sepsis require intensive care unit (ICU) management and their mortality rate remains high. The concept of withholding and withdrawal life-sustaining treatment (WWLST) issue was introduced to limit the suffering of critically ill patients. However, little is known about the characteristics and outcomes of WWLST in patients with sepsis. We conducted a nationwide cohort study of adult patients with sepsis prospectively collected from the Korean Sepsis Alliance Database of 20 tertiary referral or university-affiliated hospitals in South Korea between Septemb

The effectiveness of a multi-dimensional intervention model combining cognitive behavioral therapy and social support in hospitalized depressed patients.

BACKGROUND: Depression management in hospitalized patients with moderate-to-severe major depressive disorder (MDD) often relies on pharmacotherapy, with limited integration of psychosocial interventions. This study introduces a novel multidimensional model combining cognitive-behavioral therapy (CBT) and structured social support, addressing a critical gap in inpatient care. METHODS: This single-center retrospective cohort study included 212 inpatients diagnosed with moderate-to-severe MDD between November 2023 and December 2024. Patients were assigned to either routine care (n = 104) or an in

HOPE or hopeless? Unexpected neurological recovery after ECPR rewarming in a patient with severely hypothermic cardiac arrest with multiple haemorrhagic complications: a case report.

BACKGROUND: A high Hypothermia Outcome Prediction after Extracorporeal Life Support (HOPE) score may indicate favourable neurological outcomes in hypothermic cardiac arrest, even among elderly adults. Although the score does not account for life-threatening complications arising during hospitalisation, outcomes may nevertheless align with its favourable estimates. CASE PRESENTATION: A 90-year-old woman with a history of hypertension and mild dementia was found collapsed in a cold environment. On arrival, her core temperature was 22.0 ℃, and she developed ventricular fibrillation during transp

Preferences for palliative care services for patients with advanced cancer from the demand-supply perspective: A multicenter discrete choice experiment.

BACKGROUND: Palliative care plays a critical role in improving quality of life for patients with life-limiting illnesses. Despite increasing global policy attention, the design and uptake of palliative-care services remain constrained by limited understanding of how different stakeholders, particularly healthcare providers and patients, value service attributes. OBJECTIVE: To examine similarities and differences in preferences for palliative-care services between providers and patients with advanced cancer from a dual supply-demand perspective. DESIGN: A cross-sectional stated-preference stu

Providing Care to People Living with a Chronic Hematological Malignancy: A Qualitative Evidence Synthesis of Informal Carers' Experiences.

OBJECTIVE: Chronic hematological malignancies such as multiple myeloma, chronic lymphocytic leukemia (CLL), indolent B-cell lymphomas, and myelodysplastic syndromes (MDS) have seen significant advances in treatment. Treatment developments have resulted in patients living for many years, often between periods of being acutely unwell, relapses, and remission. Informal carers play a major role in supporting patients through the uncertain and long illness trajectory. This qualitative evidence synthesis (QES) aims to synthesize qualitative research evidence on the experiences of informal carers car

Transition from Childhood to Adulthood in Patients with Duchenne Muscular Dystrophy.

Recently, progress has been observed in the knowledge about Duchenne Muscular Dystrophy (DMD), which is a severe and commonly diagnosed genetic myopathy in childhood, historically resulting in early death. Currently, there are a lot of methods available to improve the clinical course of DMD and extend patients' life expectancy to more than 30 years of age. The key issue for DMD patients is the period between 16-18 years of age, which is described as a transition from pediatric- to adult-oriented healthcare. Adolescents and adults with DMD have highly complex healthcare needs associated with lo

The use of bioethical decision-making framework in the ICU of a public Brazilian hospital: a retrospective observational case series.

BACKGROUND: End-of-life decisions in critically ill patients lacking decisional capacity are ethically challenging. Addressing patient values-such as autonomy, dignity, and comfort-provides a person-centered basis for care, yet systematic value assessment remains inconsistent. Structured family conferences guided by a bioethical framework may help align treatment with patient preferences. This study describes how often life-sustaining treatment (LST) modifications in a Brazilian ICU were aligned with documented patient values. METHODS: This retrospective observational case series reviewed ele
