Lessons from a community vaccination programme to control a meningococcal disease serogroup W outbreak in remote South Australia, 2017.

PROBLEM: From December 2016 to February 2017, two cases of invasive meningococcal disease and one case of meningococcal conjunctivitis, all serogroup W, occurred in Aboriginal children in the Ceduna region of South Australia. The clustering of cases in time and place met the threshold for a community outbreak. CONTEXT: The Ceduna region is a remote part of South Australia, with more than 25% of the population identifying as Aboriginal or Torres Strait Islander. ACTION: As part of the outbreak response, a community-wide meningococcal vaccination programme against serogroups A, C, W and Y was

US Public Sexually Transmitted Disease Clinical Services in an Era of Declining Public Health Funding: 2013-14.

BACKGROUND: We examined the infrastructure for US public sexually transmitted disease (STD) clinical services. METHODS: In 2013 to 2014, we surveyed 331 of 1225 local health departments (LHDs) who either reported providing STD testing/treatment in the 2010 National Profile of Local Health Departments survey or were the 50 local areas with the highest STD cases or rates. The sample was stratified by jurisdiction population size. We examined the primary referral clinics for STDs, the services offered and the impact of budget cuts (limited to government funding only). Data were analyzed using SA

Vaccine Handling Management in Korea, Japan, and Taiwan: A Comparative Study of Storage Standards, Implementation Practices, and Regulatory Approaches.

BACKGROUND: Robust vaccine cold-chain management safeguards potency and public trust. Korea, Japan, and Taiwan share core practices (2-8°C storage, continuous monitoring, first-expiry-first-out [FEFO] stock rotation) yet differ in implementation and governance. We compared standards and real-world practices across the three systems to identify actionable improvements for Korea. METHODS: From March-July 2025 we conducted structured interviews and onsite observations at public health centers, hospitals, and clinics in Korea, Taiwan (National Taiwan University Children's Hospital), and Japan (Ky

Routine Molecular Surveillance of Drug-Resistant Tuberculosis: Translating Mutation Proxies into Clinical Governance Intelligence in Rural Eastern Cape.

BACKGROUND: Drug-resistant tuberculosis (TB) remains a major challenge in high-burden settings, where timely identification of emerging resistance and effective governance responses are critical. While routine molecular diagnostics generate large volumes of resistance-associated mutation data, these outputs are typically used for individual patient management and remain underutilized for population-level surveillance and for the application of clinical governance approaches for improved TB care. METHODS: We conducted a retrospective cross-sectional analysis of 1386 molecular diagnostic record

When digital systems stop working: a qualitative study of misalignment between the implementation of a human resources information system and health zone realities in the Democratic Republic of the Congo.

Digitalized human resources information system (HRIS) is promoted to strengthen health workforce governance, yet its implementation often breaks down in many low-resource settings. We explored barriers and facilitators to HRIS implementation at provincial and health zone levels in the Democratic Republic of the Congo (DRC) and identified intervention points. This qualitative study involved interviews with 46 staff from provincial health divisions and health zones in three provinces of the DRC: Kongo Central, Haut-Katanga, and Kwango, representing different stages of HRIS rollout. Interviews we

Strengthening local capacity for mathematical modelling in low- and middle-income countries: the process and lessons learnt in implementing the first cohort of Nigeria malaria modelling fellowships.

BACKGROUND: Mathematical modelling plays a crucial role in understanding malaria epidemiology and evaluating anti-malarial interventions. In sub-Saharan Africa, National Malaria Control Programs are increasingly collaborating with modellers to optimize impact within constrained fiscal environments and evaluate the effectiveness of ongoing malaria control efforts. Despite Nigeria's National Malaria Elimination Program soliciting modelling expertise, there remains a significant capacity gap in low- and middle-income countries (LMICs), including Nigeria. To address this, the Nigerian Malaria Mode

RE-AIM evaluation of a community-based vaccine education and communication program to improve human papillomavirus vaccine uptake in Tonga.

There is a global need for effective strategies to improve acceptance and uptake of the human papillomavirus (HPV) vaccine, particularly in the Pacific where cervical screening and treatment options are limited. While the Pacific Island nation of Tonga has high routine childhood vaccine coverage, HPV vaccine uptake has remained low since its introduction in 2022. To improve vaccine acceptance and uptake, recent evidence supports the use of community engagement approaches like the Vaccine Champions program, which trains health and community leaders to advocate for vaccines. We assessed the reac

Institutional quality, aid flows, and malaria burden: a geospatial analysis of sub-Saharan Africa.

BACKGROUND: Malaria remains a major public health challenge in sub-Saharan Africa, accounting for approximately 95 percent of global malaria deaths despite extensive interventions. Significant disparities persist in disease burden across countries, with some achieving remarkable progress while others experiencing persistently high transmission rates, suggesting factors beyond resource availability influence disease control effectiveness. This study examines the relationship between institutional quality, development aid flows, and malaria burden across 38 sub-Saharan African countries during 2

Potential Risk of Local Transmission of Mpox in Nepal: An Emerging Public Health Concern.

BACKGROUND AND AIMS: Monkeypox (Mpox) is a zoonotic disease caused by the monkeypox virus, a double-stranded DNA virus of the Orthopoxvirus genus with two major clades (Clades I and II), with Clade IIb responsible for most global cases reported during 2022-2024. By July 2024, more than 100,000 Mpox cases and 200 deaths were reported worldwide. Nepal has recently confirmed three imported Mpox cases, raising concerns about the potential for local transmission amid increased labor migration and peacekeeping troops abroad. This study aims to trace out the emerging threat and risk of Mpox transmiss

Health effects of European colonization: An investigation of skeletal remains from 19th to early 20th century migrant settlers in South Australia.

The British colony of South Australia, established in 1836, offered a fresh start to migrants hoping for a better life. A cohort of settlers buried in a section of St Mary's Anglican Church Cemetery (1847-1927) allocated for government funded burials was investigated to determine their health, with a focus on skeletal manifestations associated with metabolic deficiencies. Findings of St Mary's sample were compared with those published for contemporary skeletal samples from two British cemeteries, St Martin's, Birmingham, and St Peter's, Wolverhampton, to explore similarities and differences. Rare diseases in Tanzania: a National Call for Action to address policy and urgent needs of individuals with rare diseases.

A rare disease is generally defined as a condition which affects about 1 among 2000 people and currently, there are approximately 5000-8000 rare diseases (RDs) affecting over 400 million people world-wide. Although RDs may arise from different causes such as infections and environmental factors, about 80% are caused by genetic abnormalities. In Tanzania, there are no reports of the types of RDs, their incidence, distribution and numbers of individuals affected. In addition, there have been no strategies to map RDs in the country and develop a definition that fits the local context. Public awar

Dual indices for prioritizing investment in decentralized HIV services at Nigerian primary health care facilities.

Decentralizing health services, including those for HIV prevention and treatment, is one strategy for maximizing the use of limited resources and expanding treatment options; yet few methods exist for systematically identifying where investments for service expansion might be most effective, in terms of meeting needs and rapid availability of improved services. The Nigerian Government, the United States Government under the President's Emergency Plan for AIDS Relief (PEPFAR) program and other donors are expanding services for prevention of mother-to-child transmission (PMTCT) of HIV to primary
