Gastroenterologist-Level Identification of Small-Bowel Diseases and Normal Variants by Capsule Endoscopy Using a Deep-Learning Model.

BACKGROUND & AIMS: Capsule endoscopy has revolutionized investigation of the small bowel. However, this technique produces a video that is 8-10 hours long, so analysis is time consuming for gastroenterologists. Deep convolutional neural networks (CNNs) can recognize specific images among a large variety. We aimed to develop a CNN-based algorithm to assist in the evaluation of small bowel capsule endoscopy (SB-CE) images. METHODS: We collected 113,426,569 images from 6970 patients who had SB-CE at 77 medical centers from July 2016 through July 2018. A CNN-based auxiliary reading model was trai

Multimodal general anesthesia approach for Ex Utero Intrapartum Therapy (EXIT) procedures: two case reports.

High-dose volatile anesthesia is the most common method of achieving uterine relaxation for Ex Utero Intrapartum Therapy (EXIT) procedures. Other methods employ nitroglycerin for additional uterine relaxation with or without remifentanil for additional fetal analgesia. We report a combination approach including one minimum alveolar concentration of volatile anesthetic plus nitroglycerin and remifentanil infusions, to provide timely uterine relaxation under general anesthesia for both mother and fetus, during two EXIT procedures.

Acquired Vulvar Lymphangioma Following CO2 Laser Treatment for Sebaceous Gland Nevus in a 10-Year-Old Girl: A Case Report.

Acquired vulvar lymphangioma (AVL) is a rare condition characterized by abnormal lymphatic vessels in the vulva. We describe a 10-year-old female patient who presented with vulvar swelling for 23 days. She has no history of sexual assault and irradiation. Two months ago, the patient underwent CO2 laser surgery for a sebaceous gland nevus on the left labia majora, resulting in delayed healing and residual scar tissue. Twenty-three days ago, swelling appeared in the external genitalia, accompanied by mild pain that worsened after activity and lessened when lying down. Treatment with anti-infecti

From Dust to Casts: Plastic Bronchitis in a Drywall Worker Treated With Thoracic Duct Embolization.

Plastic bronchitis is a rare pulmonary condition, most frequently observed in children, that can lead to respiratory failure due to airway obstruction. We present the case of a 50-year-old man with occupational exposure to drywall who experienced recurrent episodes of hypoxic respiratory failure, necessitating multiple intubations. He was initially diagnosed with interstitial lung disease and treated with corticosteroids, but showed minimal clinical improvement. Subsequent flexible bronchoscopy revealed a bronchial cast obstructing the left mainstem bronchus. As plastic bronchitis is typically

Biomechanical alterations in lower limb lymphedema: Implications for walking ability and rehabilitation.

BACKGROUND: Lymphedema is a chronic progressive disease that results in interstitial edema in the limbs, and to a lesser extent in the genitals and face, due to damage to the lymphatic system. METHODS: Research was conducted between July 2022 and September 2022 in biomedical databases: PubMed, Cochrane Central Register of Controlled Trials (Cochrane Library), and PEDro. RESULTS: Two studies showed that lymphedema alters gait parameters by affecting mainly kinematic parameters, although kinetic parameters were found to be significantly altered, especially in patients with severe lymphedema. I

Venolymphatic vascular malformation of the parotid gland extending into the parapharyngeal space: A rare presentation.

Vascular malformations (VMs) are structural malformations of vascular development causing soft tissue abnormality with functional and esthetic impairment. They are named by their predominant vessel type as arterial, venous, lymphatic or mixed types. VMs of the parotid gland are extremely rare and constitute a distinct entity of parotid pathology that requires specific diagnostic tools and management. Till 2013, only fifty cases of VMs of the parotid have been described in the literature. We present a case of a venolymphatic malformation of the parotid gland extending into the parapharyngeal sp

A Review of Engineering Approaches for Lymphedema Detection.

Edema is a condition characterized by excessive swelling of a tissue due to an abnormal accumulation of interstitial fluid in the subcutaneous tissue. More specifically, disruption of the lymphatic system causes what is known as lymphedema. This condition is commonly seen in breast cancer survivors postradiotherapy treatment, chemotherapy, and surgeries; this population has shown high risk of developing lymphedema in the limbs. Throughout the years, several techniques have been developed and implemented for the detection and measurement of lymphedema, including techniques to measure the diseas

Axillary cavernous lymphangioma in pregnancy and puerperium.

BACKGROUND: Cavernous lymphangioma is a form of benign congenital abnormality grouped under lymphatic malformations, which can clinically manifest as a cystic tumor. Common areas are the neck and axillary region. Most cases are diagnosed in children under the age of 2, and occasionally the diagnosis is made in utero. CASE: We present the case of a 29-year-old primigravid woman with an axillary cavernous lymphangioma which rapidly increased in size during pregnancy, and describe the treatment plan in this unusual situation. CONCLUSION: The treatment of a large lymphangioma during pregnancy ha

A Large Adult Postcricoid Lymphatic Malformation: A Case Report and Literature Review.

Adult-onset lymphatic malformations arising at the postcricoid, a subsite of the hypopharynx, are sporadic. Only one case has been previously reported. In this study, we presented a 36-year-old female presented with globus sensation, dysphagia, and upper airway obstruction, requiring a tracheostomy. A computed tomography scan identified a 5 cm × 4.5 cm × 3 cm multilocular hypodense lesion in the postcricoid. Due to its considerable size, complete resection with function preservation was crucial, and a standard microscopic direct laryngoscopy approach may result in inadequate exposure, while an

Management of visceral vascular anomalies.

Visceral vascular anomalies are common in patients with vascular malformations in other parts of the body and can include lymphatic, venous, and arteriovenous malformations. Depending on the organ or organs involved they may present differently and pose different treatment challenges. Defining the malformation and understanding its extent is paramount in devising management regimens. Medical, interventional, and surgical therapies are often required in combination to treat these complex lesions. There are new and promising advances in the development of therapeutic agents targeting the PI3K/AK

A single institution experience of Eppikajutsuto for the treatment of lymphatic malformations in children.

BACKGROUND: Eppikajutsuto (TJ-28) is an herbal medicine recently reported to be effective in treating lymphatic malformations (LMs). We report our experience concerning the clinical efficacy of TJ-28 for LMs. METHODS: Medical records of 10 LM cases treated with TJ-28 between 2016 and 2018 were reviewed. TJ-28 was given at 0.3 g/kg/day and then increased to 0.5 if no improvement was noted after the first three months of treatment. Their clinical data were collected, and LM volume indices (depth×width×height) were measured with the first (LMVI-F) and latest (LMVI-L) imaging studies. The respons

Intraoperative Assessment of Facial Nerve Trunk Width in Early Childhood With Cervicofacial Lymphatic Malformation.

BACKGROUND: Facial nerve damage during head and neck surgery has long been an important issue. However, few publications on the gross anatomy of the facial nerve are available in the young population. The aim of this study was to provide in vivo measurements of the facial nerve trunk during lymphatic malformation (LM) resection and to determine the association between the trunk width and patient- and disease-related variables. METHODS: We conducted a retrospective analysis of 11 consecutive pediatric patients (11 facial nerve trunks) who underwent cervicofacial LM resection. The facial nerve

Red cell hypoplasia and monoclonal gammopathy in a patient with lymphoproliferative disorder.

A 69-year-old male was observed to have red cell hypoplasia. Two years later monoclonal gammopathy IgG-I, K, Gma+ and InV (1-) was documented in this patient. Persistent lymphocytosis, abnormal response to phytohemagglutinin, and at autopsy multiple lymphoid nodules occurring in the bone marrow, suggestive of lymphoproliferative disorder, were observed. A review of the literature indicates that this clinical presentation is rare. The possibility that red cell aplasia may be associated with lymphoproliferative disorder in some instances must be considered inasmuch as this may have an important
