Magnesium deficiency is known to produce cardiovascular lesions. It is, however, not clear as to what constitutes magnesium deficiency - reduced serum levels, reduced tissue levels or reduced intracellular levels of the ionic form of the element. This article cites evidence in support of a hypothesis that a fall in serum magnesium levels may trigger a temporal sequence of events involving vasoconstriction, hemodynamic alterations and vascular endothelial injury to produce pro-inflammatory, pro-oxidant and pro-fibrogenic effects, resulting in initial perivascular myocardial fibrosis which, in t…

Even though Mg is by far the least abundant serum electrolyte, it is extremely important for the metabolism of Ca, K, P, Zn, Cu, Fe, Na, Pb, Cd, HCl, acetylcholine, and nitric oxide (NO), for many enzymes, for the intracellular homeostasis and for activation of thiamine and therefore, for a very wide gamut of crucial body functions. Unfortunately, Mg absorption and elimination depend on a very large number of variables, at least one of which often goes awry, leading to a Mg deficiency that can present with many signs and symptoms. Mg absorption requires plenty of Mg in the diet, Se, parathyroi

Congestive heart failure is a most arrhythmogenic disease that is responsible for many unexpected sudden deaths. Reduction of sudden cardiac death rates among patient populations receiving long-term diuretic therapy, when given potassium- and magnesium-sparing diuretics as well as magnesium supplements, substantiates the premise that maintenance of adequate magnesium status is important in the prevention and management of cardiovascular diseases that predispose to congestive heart failure.

The importance of magnesium in the pathogenesis of migraine headaches is clearly established by a large number of clinical and experimental studies. However, the precise role of various effects of low magnesium levels in the development of migraines remains to be discovered. Magnesium concentration has an effect on serotonin receptors, nitric oxide synthesis and release, NMDA receptors, and a variety of other migraine related receptors and neurotransmitters. The available evidence suggests that up to 50% of patients during an acute migraine attack have lowered levels of ionized magnesium. Infu…

Electrolyte balance has been regarded as a factor important to cardiovascular stability, particularly in congestive heart failure. Among the common electrolytes, the significance of magnesium has been debated because of difficulty in accurate measurement and other associated factors, including other electrolyte abnormalities. The serum magnesium level represents < 1% of total body stores and does not reflect total-body magnesium concentration, a clinical situation very similar to that of serum potassium. Magnesium is important as a cofactor in several enzymatic reactions contributing to stable…

There is an important functional link between magnesium and calciotropic hormones. PTH stimulates magnesium reabsorption in the renal tubule, absorption in the gut and release of the ion from bone. Magnesium on the other hand is essential for the normal function of the parathyroid glands, metabolism of vitamin D and adequate sensitivity of target tissues to PTH and active vitamin D metabolites. Magnesium deficit is usually associated with hypoparathyroidism, low production of active vitamin D metabolites, in particular 1,25(OH)2 vitamin D3 and resistance to PTH and vitamin D. On the contrary, …

Oligohydramnios induced by indomethacin has been documented both clinically and experimentally. The same is not true for all the drugs inhibiting the synthesis of prostaglandins. The case of a full term pregnant woman diagnosed with renal cholic and treated with high doses of magnesium dipyrone who developed echographically diagnosed oligohydramnios at 60 h of initiation of the treatment is presented. Delivery was induced by the double intracervical application of 0.5 mg of prostaglandin E2 over an interval of 24 h. At 35 h of the discontinuation of magnesium dipyrone the patient was echograph…

Magnesium has been reported as an effective medical therapy in an expanding array of conditions. Evidence investigating magnesium's use is presented, with a number of studies suggesting it should be seriously considered in such conditions as ischemic heart disease, cardiac arrhythmias, and asthma. Magnesium balance and metabolism are briefly reviewed, and then various hypotheses are presented that may explain magnesium's physiologic mechanisms of action, most likely involving calcium and potassium flux across cellular membranes in smooth muscle. In a number of the conditions to be discussed, i…

The mechanisms of magnesium action and the possible benefits of its use in treating acute myocardial infarction are reviewed. Magnesium is an essential cofactor in more than 300 enzymatic reactions, including those responsible for the production, storage, and use of energy. It influences impulse generation and action potential propagation of the cardiac and pacemaker cells and affects muscular contraction within the myocardium and arterial smooth muscle. Magnesium has been used successfully for the treatment of arrhythmias and has been shown to produce hemodynamic changes including suppression…

Some ventricular arrhythmias can be related to a decrease in the level of potassium (K) and/or magnesium (Mg). Because adrenergic stimulation decreases serum K+ and Mg++, we decided to investigate the effects of a beta-receptor agonist, isoproterenol, on serum K+ and Mg++, and their consequences on the induction of tachycardia. Programmed atrial and ventricular stimulation was performed in 95 patients before and during infusion of 1.6 micrograms.ml-1 of isoproterenol. During isoproterenol infusion, 61 patients had no inducible tachycardias (group I) and 34 had inducible sustained tachycardias …

A reliable diagnosis of laxative abuse can only be reached by chemical analysis. We report a modified thin layer chromatography approach for the detection and confirmation of over-the-counter colonic stimulant laxatives. Potential interference by dietary and drug components chemically similar to anthraquinones was investigated and the method was found to be specific in this respect. The diagnosis of magnesium laxative abuse is more difficult since magnesium is a component of body fluids. By inducing diarrhoea with magnesium salts in volunteers we have established that the diagnosis can be made…

80 patients with soft tissue calcifications were treated. 24 patients suffered from myositis ossificans traumatica, 23 from calcific bursitis (Duplay's disease), 6 from osteoarthropathy of elbow joint after severe craniocerebral trauma, 9 from calcifications round elbow joint after local trauma, 13 from calcifications round hip joint, 5 from calcifications in ligaments and tendons. About 75 per cent patients were cured. Calcifications disappeared or substantially diminished. Very good functional improvement followed in affected joints. Patients were treated with new method: local application o

Magnesium, the second most abundant intracellular cation, is essential for life. The consequences of deficiency are severest in the smallest and youngest members of each species and may include sudden unexpected death. Magnesium deficiency, usually diagnosed by hypomagnesemia, may be congenital, as in premature infants, infants of magnesium-deficient mothers and infants with intrauterine growth retardation. It may be acquired or caused by low magnesium intake, the use of magnesium-wasting drugs, illness provoking gastrointestinal or renal losses of the mineral, or high metabolic demands impose…

Despite considerable research on magnesium (Mg) absorption there is still uncertainty regarding the site and mechanism of intestinal Mg transport. A careful review of the literature indicates that while Mg is absorbed throughout the intestine, the predominate site is the distal small intestine. There are three mechanisms by which Mg has been shown to cross the intestine:passive diffusion, solvent drag, and active transport. The importance of each of these mechanisms to total Mg absorption is discussed. Studies in both humans and experimental animals indicate that passive diffusion through the …

Mg depletion, particularly in the hippocampus, appears to represent an important pathogenic factor in Alzheimer's disease. It is associated with high aluminium incorporation into brain neurones. This type of Mg deficit cannot respond to mere Mg supplementation, but requires correction of the dysregulation inducing this Mg depletion. Further research should seek to control the alterations of albumin, which may induce this brain Mg depletion.
