Congenital dislocation of the hip. Early detection and prevention of late complications.

Most congenitally dislocated hips (CDHs) are inapparent and asymptomatic during the neonatal period, although at that time a high percentage can be detected. Treatment during this period can decrease the late sequelae of CDH. Although the abduction tests of Ortolani and Barlow are the standard methods of detection in the neonate, their value and their use in screening programs have been overemphasized. There is no evidence that the use of these tests in screening programs applied to large populations can consistently reduce the late sequelae of CDH. This overemphasis on neonatal screening resu

Diphtheria: the logistics of mass swabbing.

Although diphtheria is a rare disease it is necessary to swab contacts of all cases in order to identify carriers. The selection of individuals who should be swabbed and the organisation of mass swabbing so that there is the minimum of delay between collection, culture and reporting are described. With this organisation it is possible for a laboratory to culture at least 1000 swabs in any one day.

Oral health screening for sport.

Oral health is an essential component of general health and wellbeing, but there are many potential challenges to the oral health of athletes, including nutrition and hydration, exercise-induced immune suppression, lack of awareness, negative health behaviours and lack of prioritisation. Oral diseases should be preventable, and there are simple interventions that have good evidence of efficacy. Screening that allows early recognition of disease is well-established in healthcare as the first step in prevention of disease and promotion of positive health behaviours. This paper aims to provide an

A generalizability analysis framework for bias resilience in convolution neural network and transformer models for diabetic retinopathy screening.

Automated diabetic retinopathy (DR) screening has seen extensive research progress, yet its clinical adoption remains limited, largely due to insufficient attention to model generalizability across diverse populations and imaging conditions. Existing studies often overlook the role of dataset biases, arising from demographic, acquisition, and preprocessing variations that undermine robustness in real-world settings. To address this critical gap, we propose a comprehensive generalizability analysis framework that systematically categorizes dataset biases, evaluates their impact on performance,

Steatotic liver disease trajectory: A holistic EASL guidance on the multidisciplinary approach to screening and patient management.

Steatotic liver disease (SLD) is a spectrum of highly prevalent liver diseases encountered at many different healthcare levels. Awareness about metabolic dysfunction-associated SLD (MASLD), MetALD (MASLD and increased alcohol intake) and ALD (alcohol-related liver disease) is increasing beyond hepatology, as reflected by the various SLD guidelines issued by scientific societies from different disciplines, including endocrinology. The guidance presented here aims to improve cross-disciplinary knowledge and know-how by providing guidance to healthcare professionals from a global, holistic, multi

PURPOSE: To map the state of the art regarding risk factors and assessment methods employed in the screening of fluency disorders in preschool children. RESEARCH STRATEGIES: A scoping review was conducted in accordance with the Joanna Briggs Institute guidelines and the PRISMA-ScR checklist. The guiding question was: What are the risk factors and assessment methods used in the screening of fluency disorders (stuttering and cluttering) in preschoolers, as reported in the international literature? The PCC strategy guided the search, using MeSH descriptors and free terms across the Lilacs, PubMe

Rethinking hypercortisolism screening in obesity and diabetes: A systematic review and meta-analysis.

BACKGROUND AND AIM: The growing burden of obesity and diabetes underscores the need for innovative strategies to identify secondary causes. While hypercortisolism is traditionally considered rare, its broader spectrum - from mild autonomous cortisol secretion to overt Cushing's syndrome (CS) - may be under-recognized in these populations. METHODS: We systematically searched MEDLINE, Scopus and Web of Science for original studies assessing hypercortisolism in obesity or diabetes. We included first-line screening tests for hypercortisolism, particularly the dexamethasone suppression test (DST),

Beyond the mouth: An overview of obstructive sleep apnea in adults for dentists.

Obstructive sleep apnea (OSA) is a common and complex medical sleep condition in adults that is associated with chronic medical and mental health conditions. Left undiagnosed and untreated it can decrease quality of life and increase morbidity and mortality. Dentists are at the forefront for screening OSA in their role to maintain the health of the oral cavity. Clinical experience in the management of OSA in dentists is limited. The aim of this article is to provide dentists with foundational knowledge about OSA to empower them to play an active role in the care management of screening and tre

The evolution of diabetic retinopathy screening.

Diabetic retinopathy (DR) is a leading cause of preventable blindness and has emerged as a global health challenge, necessitating the development of robust management strategies. As DR prevalence continues to rise, advancements in screening methods have become increasingly critical for timely detection and intervention. This review examines three key advancements in DR screening: a shift from specialist to generalist approach, the adoption of telemedicine strategies for expanded access and enhanced efficiency, and the integration of artificial intelligence (AI). In particular, AI offers unprec

Screening for Undiagnosed Heart Failure: A Viewpoint.

The syndrome of heart failure (HF) is characterized by a triad of (1) signs and symptoms, (2) cardiac alterations, and (3) natriuretic peptide elevation. Patients who exhibit alterations of cardiac structure or function without overt signs or symptoms of HF are staged as having "pre-HF." However, many people with undiagnosed HF walk little and do not exercise; thus, they may be classified as having asymptomatic "pre-HF" simply because they never exercise enough to feel breathless. Moreover, many patients have comorbidities such as chronic obstructive pulmonary disease or obesity and the physic

[Recommendations for the Diagnosis and Treatment of Chronic Kidney Disease (CKD) by the Austrian Society of Nephrology].

Chronic kidney disease (CKD) affects over 800 million people worldwide, yet only about 6-10% of those affected are aware of their condition. Limited public awareness and insufficient structural measures in the healthcare system lead to the fact that CKD is frequently not diagnosed or diagnosed after a delay and therefore insufficiently treated. Current projections estimate that CKD could become the fifth leading cause of death worldwide by 2040, if timely and effective measures to increase awareness and early detection are not implemented. This article presents practice-oriented guidelines for

A practical approach to uveitis screening in children with juvenile idiopathic arthritis.

BACKGROUND: Juvenile idiopathic arthritis (JIA)-associated uveitis typically presents as a silent chronic anterior uveitis and can lead to blindness. Adherence to current screening guidelines is hampered by complex protocols which rely on the knowledge of specific JIA characteristics. The Multinational Interdisciplinary Working Group for Uveitis in Childhood identified the need to simplify screening to enable local eye care professionals (ECPs), who carry the main burden, to screen children with JIA appropriately and with confidence. METHODS: A consensus meeting took place in January 2023 in

Screening and diagnosis of primary aldosteronism. Consensus document of all the Spanish Societies involved in the management of primary aldosteronism.

Primary aldosteronism (PA) is the most frequent cause of secondary hypertension (HT), and is associated with a higher cardiometabolic risk than essential HT. However, PA remains underdiagnosed, probably due to several difficulties clinicians usually find in performing its diagnosis and subtype classification. The aim of this consensus is to provide practical recommendations focused on the prevalence and the diagnosis of PA and the clinical implications of aldosterone excess, from a multidisciplinary perspective, in a nominal group consensus approach by experts from the Spanish Society of Endoc

Detecting a hidden pandemic: The current state and future direction of screening and assessment tools for intimate partner violence-related brain injury.

Intimate partner violence (IPV) is a major global concern, and IPV victim-survivors are at an increased risk of brain injury (BI) due to the physical assaults. IPV-BI can encompass both mild traumatic brain injury (mTBI) and non-fatal strangulation (NFS), but IPV-BI often goes undetected and untreated due to a number of complicating factors. Therefore, the clinical care and support of IPV victim-survivors could be enhanced by BI screening and assessment in various settings (e.g., first responders, emergency departments, primary care providers, rehabilitation, shelters, and research). Further,
