[Progressive hearing loss as the leading sign of Wegener's granulomatosis].

57-year-old woman with a history of ischaemic heart disease, arterial hypertension and after myocardial infarction was admitted to the university hospital because of progressive hearing loss and fever of unknown origin. Shortly before hospitalization she developed cough, hemoptysis and conjunctivitis. On the basis of clinical presentation Wegener's granulomatosis was suspected. To confirm the diagnosis, CT scans of the chest, sinuses and ears were performed and revealed massive lesions especially in tht tympanic cavity, mastoid antrum and cells. Infiltrations were also observed in sinuses, esp

The importance of the palatine bone for endoscopic endonasal skull base surgery.

Endoscopic endonasal skull base surgery is increasingly prevalent, with its scope expanding from pathogens in the midline region to those in the paramedian region. Maximizing anterior sphenoidectomy is important for the median approach, and lateralizing the pterygopalatine fossa is crucial for the paramedian approach. Maximizing the surgical corridor in the nasal cavity and minimizing damage to neurovascular structures are vital for establishing a surgical field with minimal bleeding, ensuring safe, precise, and gentle procedures. However, the relationship between the maxillofacial and skull b

Congenital healed cleft lip.

Congenital "healed" cleft lip (CHCL) is an unusual anomaly including a paramedian "scar" of the upper lip, which appears as if a typical cleft lip has been corrected in utero. The CHCL is frequently associated with an ipsilateral notch in the vermilion, and "collapsed" nostril. Twenty-five CHCL cases are presented, eighteen of which were an isolated malformation found among the 3,950,715 births examined in two similar birth defect registries: ECEMC in Spain and ECLAMC in Latin America. Like open cleft lip, of which it seems to be a variant, CHCL is most frequently seen among males (14/18 isola

Radiotherapy in late elderly (aged 75 or older) patients with paranasal sinus carcinoma: a single institution experience.

The purpose of our study was to assess the feasibility of radiotherapy (RT) for locally advanced paranasal sinus carcinomas in late elderly patients (aged ≥75 years) from a single institution in Japan. From 2000 to 2015, we retrospectively analyzed 14 patients (11 maxillary and 3 ethmoid sinus carcinoma patients) who underwent RT for pathologically confirmed paranasal sinus carcinomas. RT was performed without unexpected cessations. Two patients, however, developed Grade 3 mucositis. The median follow-up duration was 13 months (range 2-54 months). The 1- and 2-year overall survival (OS) rates

Incidental paranasal sinusitis on routine brain magnetic resonance scans: association with atherosclerosis.

BACKGROUND: Incidental paranasal sinusitis (IPS) is common on imaging for non-sinusitis disorders, usually without symptoms or obstructive features, and possibly arising from periodontitis (PD). PD associations with atherosclerosis have been widely reported. We test if IPS may also be associated with atherosclerosis. METHODS: IPS was scored retrospectively in a random sample of 180 magnetic resonance (MR) brain scans and compared with chart review for atherosclerosis (all subtypes), rhinosinusitis, and related factors (smoking, asthma, and relevant surgery). IPS was scored out of 30, from all

[Indications for selective arterial embolization in the treatment of epistaxis].

After posterior packing has failed, the treatment of choice for severe, recurrent posterior epistaxis is arterial ligature, usually through a transantral approach to the Internal Maxillary artery (LTA) or selective percutaneous embolization (EP). The advantages and disadvantages of each technique are discussed by various Authors. A critical review of the literature brings to light the discrepancies between the results of various studies: in a series by Strong et al. and in a review of the literature EP proved more effective than LTA (90-94% vs. 85-89%). On the contrary, using personal data Cul…

Brisk, prolonged pulsatile hemorrhage during the sinus graft procedure: a case report with discussion on intra-operative hemostatic management.

The maxillary sinus graft procedure is the treatment of choice to increase the vertical bone height in the atrophic posterior maxilla to place dental implants. It is considered as a routine surgical procedure with predictable results. However, complications have been reported with this grafting procedure. With any surgical procedure, complications may arise, and the surgeon must be prepared to effectively manage the complication. In addition to detailed knowledge of the maxillofacial anatomy, recognition of surgical emergencies and its management is critical for the implant surgeon. A case of

Transparotid Approach for the Treatment of Condylar Osteochondroma and Ankylosis of the Temporomandibular Joint.

Among the methods to treat temporomandibular joint (TMJ), preauricular, submandibular, retromandibular, and intraoral approaches have been described. These approaches, however, occasionally offer inadequate access, owing to the oblique course of the facial nerve, and access can be limited, especially to the medial and anterior part of the infratemporal fossa. The use of the transparotid approach can offer direct visualization of the facial nerves to prevent severe damage, and a wide work field can be achieved from the medial-anterior part of the infratemporal fossa to subcondylar region by ret

Transposition of the hemimasseteric muscle for dynamic rehabilitation of facial paralysis.

BACKGROUND: The masseter muscle is one of the muscles involved in mastication. Transposition of this muscle has been used for dynamic reanimation of facial palsy since the early years of the 20th century. We present an anatomic study of the masseter muscle and its neurovascular bundle to determine the possibility of using hemimasseteric transposition of the muscle for the rehabilitation of facial paralysis. METHODS: Six white fresh cadavers were used to study the masseter nerve and the vascular supply to the masseteric muscle. Dissection was performed in each hemiface of each specimen. All th
