Morphologic Evaluation for Safe Le Fort I Osteotomy in Cleft Lip and Palate.

OBJECTIVE: The present study aimed to determine the structure and morphology of the maxilla in patients with cleft lip and palate to ensure safe Le Fort I osteotomy. PATIENTS: A total of 34 sides of 17 patients with unilateral cleft lip and palate (nonsyndromic cases) were included in this study. The control group included 10 sides of 5 patients who exhibited skeletal mandibular protrusion without malformation. METHODS: Finite element analysis was performed to examine the distribution of occlusal force over the maxilla, and continuous 3-dimensional measurement was performed at the sites of s

Modified Lateral Surgical Approach to the Pterygopalatine Fossa.

Oral cancer, especially that originates in the maxillary tuberosity, buccal mucosa, and maxillary alveolus, is apt to invade the pterygopalatine fossa, where an intraoral approach is nearly impossible. There are 2 main types of extraoral approach to the pterygopalatine fossa; the anterior approach and the lateral approach. Although the lateral approach has an advantage compared with the anterior approach in terms of cosmetics, after the operation numbness of the lower lip occurs due to the sacrifice of the mental nerve. In the conventional lateral approach, a vertical incision is made in the c

Mandibular growth following Le Fort I osteotomy in adolescent monkeys.

Le Fort I osteotomy with anterior and superior repositioning of the maxilla was performed on adolescent Macaca fascicularis monkeys to study its effect on the subsequent growth and remodeling changes of the mandible. Six adolescent Macaca fascicularis monkeys were randomly divided into two surgical groups and eight others served as controls. Group I animals underwent a Le Fort I advancement and Group II animals experienced a Le Fort I advancement and impaction. All monkeys received tantalum implants in the anterior part of the cranial base, on opposite sides of craniofacial sutures and in mult

Autotransplantation of an Impacted Immature Third Molar Following Orthodontic Activation without Endodontic Intervention.

Autogenous tooth transplantation (ATT) using open-apex donor teeth may preserve pulp vitality, but atraumatic retrieval of impacted third molars is challenging. This case report describes an interdisciplinary workflow combining orthodontic donor activation with digitally guided, vitality-preserving ATT. A 27-year-old woman with an edentulous maxillary left first molar site (tooth #14) underwent ATT using an immature mandibular right third molar (tooth #32) as the donor. After 6 weeks of orthodontic traction, cone-beam computed tomography and intraoral scans were registered for virtual planning

An Atraumatic Approach to Internal Sinus Lifting: The Motorized Expansion Drill Technique.

This case series demonstrates internal maxillary sinus elevation and placement of 37 posterior implants in 34 private practice patients using sequential motorized expander burs exclusively to infracture the sinus border and embed bone graft material (allograft or xenograft) for vertical augmentation. With this technique, manual osteotome/malleting was completely avoided, improving surgical control and the patient experience. Sinus borders were lifted by a mean of 3.92 mm in ridges that had an average residual height of 7.52 mm. All implants achieved high insertion torque values (≥90 Ncm), obta

Comparative study of two autogenous graft techniques using piezosurgery for sinus lifting.

PURPOSE: Maxillary sinus lifting is a technique, in which, a possible complication is sinus membrane perforation. The aim of this study was to compare two techniques using ultrasound surgery to perform autogenous graft for maxillary sinus lifting. METHODS: Ten rabbits were used in the study, one of them did not undergo surgery. The other nine rabbits had their maxillary sinuses filled with autogenous bone grafts collected from the external skull diploe in particulate form on the right side, and shaved on the left side, both with ultrasonic device. Data on bone density in left and right maxill

Osteotome sinus floor elevation technique without grafting material and immediate implant placement in atrophic posterior maxilla: report of 2 cases.

PURPOSE: This case report discusses 2 patients who required implant placement in the atrophic posterior maxilla to support a fixed prosthesis with the least invasive and shortest procedure. MATERIALS AND METHODS: The reference standard of care would be to perform sinus augmentation with an autologous bone graft through the lateral approach with delayed implant placement. However, in these cases, the posterior maxillas were treated with an osteotome sinus floor elevation procedure without grafting material and simultaneous placement of short, 8- and 10-mm-long, tapered implants. RESULTS: All

Unusually large complex odontoma in maxillary sinus associated with unerupted tooth. Report of case and review of literature.

An unusual case of a large complex odontoma with an associated impacted tooth is presented. Odontomas are hamartomatous growths of enamel, dentin, cementum and pulp tissue. Although they are usually tooth-sized or smaller, occasionally, the complex variant can exhibit considerable growth, as was seen in the case presented here. It occupied most of the maxillary sinus and displaced the floor of the orbit and the medial and posterior walls of the left maxillary sinus. Panoramic radiographs, as well as axial and coronal CT studies, showed the extent of the lesion in various dimensions. A differen

Evaluation of implant stability and increase in bone height in indirect sinus lift done with the osseodensification and osteotome technique: A systematic review and meta-analysis.

STATEMENT OF PROBLEM: Whether the use of osseodensification burs for indirect sinus lift improves primary implant stability and bone height as compared with the osteotome technique to overcome the challenges of the pneumatization of the maxillary sinus and vertical bone loss after extraction in the edentulous posterior maxilla is unclear. PURPOSE: The purpose of this systematic review and meta-analysis was to evaluate the difference in primary implant stability and increase in bone height in indirect sinus lift using osseodensification and the osteotome technique. MATERIAL AND METHODS: Two i

Clinical and Radiographic Examination of Endoscopically Controlled Indirect Sinus Lift: An In vivo Study.

It was the aim of this study to quantify the gain in height of bone at implant sites by endoscopically controlled osteotome sinus floor elevations (ECOSFE) with simultaneous implant placement and to report the number of sinus membrane perforations. An indirect sinus lift was done in 10 patients under endoscopic control with an osteotome technique. The average residual height of the alveolar crest in the posterior maxilla was 5.625 mm. Elevation of the sinus floor was done using conventional sinus floor elevation instruments. A mean elevation of 5.205 mm was achieved. Twenty implants ranging in

Medication-Related Osteonecrosis of the Jaw: An Autofluorescence-Guided Surgical Approach Performed with Er:YAG Laser.

OBJECTIVE: The aim of this study was to propose an autofluorescence (AF)-guided surgical approach performed with Er:YAG laser and Nd:YAG low-level laser therapy (LLLT). BACKGROUND DATA: Medication-related osteonecrosis of the jaw (MRONJ) therapy remains an unresolved problem. The proposed conservative and surgical treatment regimens are associated with contradictory success rates. However, the increased experience with MRONJ management suggests that surgical therapy can halt disease progression, and can allow a histology-based diagnosis of osteonecrosis. Surgical approach with Er:YAG laser is

The use of endoprotheses for patients with complex asymmetry: A clinical case study.

Facial asymmetry requires multidisciplinary management. In some cases, predictable problems due to congenital abnormalities can be corrected by early orthodontic treatment. Orthodontics is nevertheless often part of an orthosurgical treatment plan. The solutions are conventional and familiar: maxillomandibular rotation osteotomy and genioplasty. Many cases can be treated in this way, but there is often persistent residual asymmetry due to loss of volume that is not corrected by these techniques. The use of maxillo-acial endoprostheses designed on the basis of three-dimensional craniofacial ana

Haploinsufficiency of SF3B2 revealed by a craniofacial microsomia with atypical presentation: a case report.

Otomandibular syndrome is primarily characterised by craniofacial microsomia (CFM). Pathogenic loss-of-function variants of the SF3B2 gene, … described previously. Particularly low maxillary and mandibular bone density was observed during bimaxillary osteotomy, necessitating adaptation of the surgical protocol. This case expands the phenotypic spectrum of anomalies associated with the SF3B2 gene by indicating potential subclinical acral and bone involvement. It also emphasises the importance of genetic an …

Osteoplastic maxillotomy approach for infraorbital nerve schwannoma, a case report.

BACKGROUND: Extracranial schwannomas can readily occur in the head and neck region and rarely involve the trigeminal nerve. As a rule, their treatment is surgical and dictated by the location of the tumor and nerve of origin. METHODS: We describe a case of a 14-year-old boy with a mass invading right nasal fossa, maxillary sinus, orbital floor, pterygopalatine fossa, and infratemporal fossa. The diagnosis of a nerve sheath tumor was evoked after angiography showed no vascular blush. RESULTS: The tumor was removed through a Weber-Fergusson incision with subciliary extension followed by maxill

Horizontal alveolar distraction of the narrow maxillary ridge for implant placement.

PURPOSE: The purpose of this report was to describe a surgical technique for performing horizontal alveolar distraction of the knife-edge maxillary ridge. PATIENT AND METHODS: The patient was a 17-year-old woman with atrophy of the alveolar rim in the anterior upper jaw, which had inadequate width for implant placement. The transport segment was constructed by the osteotomy of the labial cortex of the alveolus. A transport plate of a distractor (LEAD system; Stryker Leibinger, Kalamazoo, MI) was placed on the transport segment. The distraction rod was inserted horizontally, and put in contact
