Perimesencephalic nonaneurysmal subarachnoid haemorrhage (PN-SAH) is characterised by accumulation of blood around the midbrain, normal angiographic findings and an excellent prognosis. The etiology of PN-SAH has not yet been determined. Therefore we decided to compare the venograms of patients with PN-SAH with those of patients with aneurysmal SAH (A-SAH) in order to examine the relationship between PN-SAH and venous structures. We retrospectively studied 6 patients with PN-SAH and 102 cases of angiographically evaluated A-SAH during the past 12 years by reviewing their venograms for possible

Unilateral third nerve palsy with bilateral superior rectus paresis and bilateral ptosis is a typical condition for nuclear oculomotor nerve syndrome. We report a case of nuclear oculomotor nerve syndrome due to midbrain hemorrhage, as a rare cause. A 73-year-old man presented with an abrupt onset of double vision and difficulty opening his eyes. He had uncontrolled hypertension in his history. Neurological examination revealed right oculomotor palsy with impairment of bilateral upward gaze and bilateral ptosis. MRI showed a mesencephalic area of increased T1 signal and decreased T2 signal con

OBJECTIVE: A critical review of the literature on the incidence, presentation, diagnosis, and prognosis of perimesencephalic nonaneurysmal subarachnoid hemorrhage.
METHODS: Review of the relevant literature.
CONCLUSION: The importance of early computed tomography (< 3 d), anatomy of the perimesencephalic and neighboring cisterns, and adequate four-vessel angiography are discussed. Treatment strategies, including the avoidance of repeated angiographic studies and surgical exploration, are presented.

Spontaneous, nontraumatic, nonhypertensive, midbrain hemorrhage is an uncommon cause of the dorsal midbrain syndrome. We describe a patient with this syndrome in whom the initial neuroimaging studies failed to clearly identify the lesion. Subsequent magnetic resonance imaging studies disclosed a dorsal midbrain hemorrhage. The patient experienced gradual spontaneous resolution of her dorsal midbrain signs and symptoms over several months. The evolution on serial neuroimaging studies of the dorsal midbrain hemorrhage in this patient is described.

Bilateral ophthalmoplegia may be an unusual sign of vertebrobasilar ischemia. We report the cases of two patients (75 and 73 years old), who suddenly developed drowsiness, bilateral ophthalmoplegia with bilateral ptosis and mild right hemiparesis. In both patients, MRI revealed bilateral thalamic and midbrain infarcts, ECG showed the presence of atrial fibrillation and Doppler study of the extracranial and intracranial vertebral arteries found no significant alterations. Ischemia involving the midbrain and thalamic paramedian regions may cause bilateral ophthalmoplegia and consciousness distur

Compelling evidence exists that tobacco-smoking represents a form of drug addiction to nicotine. Like several drugs of abuse, nicotine activates the mesolimbic dopamine system and this effect appears to be of critical importance for the reinforcing properties of the drug. Specifically, nicotine has been shown to increase burst activity in dopamine neurones of the ventral tegmental area, i.e. a mode of firing pattern in these cells which is physiologically associated with basic motivational processes underlying learning and cognitive behaviour. The stimulatory action of nicotine on mesolimbic d

We report a case of a 54-year-old man with sudden-onset double-vision. On admission, neurological examination showed upward and downward gaze palsy on voluntary and smooth pursuit movements, and vertical oculocephalic maneuver elicited a full upward and downward response. Bell's phenomenon, horizontal eye movements and convergence were not impaired. Based on these findings, supranuclear dissociated vertical gaze palsy was diagnosed. T1-weighted MR images revealed low intensity on the medial side of the right thalamo-mesencephalic junction, which impaired the rostral interstitial nucleus of the

A 65-year-old white female presented with the sudden onset of headaches, bilateral ptosis, and complete ophthalmoplegia. Other than a mild decrease in mental status, she was neurologically intact. Magnetic resonance imaging (MRI) and computed tomography (CT) examinations showed a midline hemorrhage extending from the caudal diencephalon to the pontomesencephalic junction affecting the oculomotor complex, the medial longitudinal fasciculus (MLF), and the rostral parapontine reticular formation (PPRF). Ischemic changes were also noted in the midline pontine tegmentum possibly affecting root fibe

A 31-year-old Hispanic laborer sought medical attention for a four-day history of posterior headaches and double vision. The headache spontaneously resolved, but the patient continued to experience vertical diplopia. The neuro-ophthalmic exam revealed minimal anisocoria and subtle deficits of vertical motion of the left eye. MRI scanning demonstrated a lesion within the midbrain on the left side. Lumbar puncture revealed cells consisting primarily of lymphocytes and eosinophils. Both serum and cerebrospinal fluid revealed glycoproteins indicative of a cysticercosis infection. The patient was t

We have used cultured ventral mesencephalic and cerebellar granule cells to test the toxicity of extracts of cycad seeds (genus Cycas) and cycad-derived flours traditionally prepared in Guam. There was no significant difference in the toxicity of extracts prepared from the female gametophyte tissue of C. circinalis, C. revoluta, and C. media, common wheat flour, and 13 of 17 cycad flour samples. However, extracts prepared from 4 of 17 Guamanian flour samples exhibited marked dose-dependent neurotoxicity to mesencephalic and granule cell cultures. There was no correlation between toxicity and 2

The tonic increased influence of the superior colliculus (SC) on the formation of visual responses of the sensorimotor cortex (SMC) was shown on the alert rabbits. It was shown that SC influence was realized through tectothalamocortical (nucleus lateralis posterior) canal relation. It was established that SMC of the alert rabbits in its turn exerts the inhibitory phasic influence on SC function. The obtained data are discussed in the light of participation of the colliculo-cortical and cortico-collicular interactions in the organization of the visual-motor coordination necessary for realizatio

Primary aqueductal stenosis is one of the main causes of congenital hydrocephalus in humans and experimental models. The congenitally hydrocephalic rat strain LEW/Jms is one such model. In this report, we describe further detailed histological features of periaqueductal structure, including the posterior commissure, subcommissural organ (SCO), and ependyma, and discuss the changes in these structures in relation to the cause of hydrocephalus. Coronal sections of the aqueduct in normal rats showed that the usual ependyma was absent in the center of the base facing the dorsal side, which was rep

A 65-year-old female developed peduncular hallucinosis 3 days after rupture of a basilar-superior cerebellar artery aneurysm. There were no neurological deficits except slight anisocoria when she first complained of hallucinations. Vasospasm of the perforating arteries to the upper brainstem, rather than direct brainstem damage caused by the bleeding, was probably the cause. Peduncular hallucinosis is possibly the only localizing sign of ruptured upper posterior circulation aneurysm.

We describe a patient with a dissociated vertical nystagmus and an internuclear ophthalmoplegia. The vertical nystagmus consisted of a left downward nystagmus with a synchronous right intorting nystagmus when the patient looked down and to the left. This rare type of nystagmus has been described both in isolation and in association with an internuclear ophthalmoplegia. Previous authors postulated a lesion in the midbrain in the region of the medial longitudinal fasciculus. In our patient, a discrete midbrain infarction was demonstrated on magnetic resonance imaging in the hypothesized location

Intracranial lipomas are rare, usually do not have clinical expression and are located more frequently in the corpus callosum. Other locations include the spinal cord, midbrain tectum, superior vermis, tuber cinereum, infundibulum and more rarely cerebellopontine angle, hypothalamus, superior medullary velum and insula. We report the case of a lipoma of the left inferior colliculus which was a post-mortem finding in a woman who died of breast cancer. Although there are reports of intracranial lipomas in patients with malignant tumors there is no explanation for the co-existence of the two tumo

A 62 year old man suddenly presented with a pure left sensory deficit involving only pain and temperature sensations. CT scan and MRI showed a small haemorrhage in the dorsolateral part of the right midbrain. This suggested a lesion limited to the right dorsal spinothalamic tract.

On the awake rabbits and cats under nembutal anesthesia it has been shown that the reticular nucleus of the thalamus takes considerable part in the formation of reticulogeniculate response of the lateral geniculate body (LGB) to electrostimulation of the mesencephalic reticular formation. It is assumed that the reticular nucleus of the thalamus takes basic part in a realization of "rapid" physical influences of the reticular formation on the LGB.

A quantitative analysis of the oculomotor, trochlear, and abducens nuclei was carried out in the mutant microphthalmic rat. In this strain of rat, the eyeball is reduced to about one-third in diameter, and there is no optic nerve. Nevertheless, this strain possesses all the types of extraocular muscles; however, the volume of these muscles is reduced from 33 to 74% of the normal values. The oculomotor, trochlear, and abducens nuclei in this mutant rat were found in the same location as in the normal ones. Moreover, the neurons in these nuclei in the microphthalmic rat did not appear to be any

Injections of horseradish peroxidase, wheat germ agglutinin and various amino acids into the optic tectum in both the turtle and rat, and into the nucleus isthmi magnocellularis (IsM) in the turtle were used to analyse the connections of the isthmus-tegmentum complex. The connectivities and the selective retrograde transport properties in certain tectal pathways were taken as a basis to define more accurately isthmus-tegmentum complex subdivisions. There were several main findings. In the turtle the projection from the tectum to the IsM originated in the stratum griseum periventriculare, where
