Preclinical Evaluation of the Alexis Retractor as a Temporary Abdominal Closure Device in Damage Control Surgery Using a Dead Tissue Training Model.

Temporary abdominal closure (TAC) is a critical component of damage control surgery. Negative pressure wound therapy (NPWT), particularly when combined with continuous fascial traction, is regarded as the preferred TAC strategy because of improved delayed fascial closure rates. However, dedicated equipment, consumables, and technical expertise may be unavailable in military, humanitarian, mass-casualty, or resource-limited settings, where simpler techniques such as the Bogotá bag remain relevant. The Alexis retractor provides circumferential retraction and wound-edge protection but has not pre

Do Junior Entrants to the UK Armed Forces have worse outcomes than Standard Entrants?

INTRODUCTION: The UK is the only permanent member of the UN Security Council that has a policy of recruiting 16 and 17 year old individuals into its regular Armed Forces. Little is known about the consequences of enlisting as a Junior Entrant (JE), although concerns have been expressed. We compare the mental health, deployment history, and pre-enlistment and post-enlistment experiences of personnel who had enlisted as JEs with personnel who joined as Standard Entrants (SEs). METHOD: Participants from a large UK military cohort study completed a self-report questionnaire between 2014 and 2016

Transfusion of rare cryopreserved red blood cell units stored at -80 degrees C: the French experience.

The technology allowing freezing of RBC units has been available for many decades. The high-glycerol method for RBC storage at -8 degrees C is predominantly used. Several studies have shown satisfactory results regarding the in vitro viability and function of cryopreserved RBCs. RBC freezing is nowadays mostly encountered in rare blood programs and military deployments. Preservation time of frozen RBCs appears to be virtually indefinite, but most countries apply a 10-year outdate. There is no mandatory time restriction in France. The National Rare Blood Bank currently includes 962 (17.5%) RBC

Risk of First Adjustment Disorder in U.S. Army Soldiers.

INTRODUCTION: Adjustment disorder is the most common mental health diagnosis in the U.S. military and one of the most common globally, yet its patterns and predictors are rarely studied. Identifying career phases and characteristics associated with increased risk of first adjustment disorder (AD) diagnosis among U.S. Army soldiers can inform targeted prevention and intervention efforts. MATERIALS AND METHODS: This longitudinal, retrospective cohort study analyzed data from the Study to Assess Risk and Resilience in Servicemembers-Longitudinal Study (STARRS-LS). Administrative medical and pers

Instructor and Trainee Judgments of Cohesion in Mixed-Gender Military Teams.

INTRODUCTION: In integrating women into combat roles, one key question has been whether women will reduce the cohesion of small teams, which is their ability to stick together and perform well under pressure. Among other things, team cohesion protects the mental health of military personnel during initial training as well in operational zones. To provide new converging evidence on the cohesion of all-male versus mixed-gender recruit teams at the end of training, cohesion ratings were obtained from both recruits and their instructors. MATERIALS AND METHODS: The recruits consisted of 83 females

Risk Indicators of Urgent and Extensive Dental Treatment Needs in U.S. Air Force Recruits.

INTRODUCTION: Dental Readiness Classifications (DRCs) enable the Military Health System to prioritize dental care in garrison, minimizing dental emergencies and mission degradation during deployments. Over half (52.4%) of 2008 military recruits presented with high-priority urgent needs classified as DRC3 upon initial dental examination and 18.1% required extensive treatment, needing 7 or more restorations, in order to achieve operational dental readiness. The purpose of this study is to identify risk indicators for urgent and extensive dental treatment needs in current U.S. Air Force (USAF) re

Feasibility and usability of a combat medical early warning system for simulated mass casualty triage during a naval exercise - A pilot study.

INTRODUCTION: Mass casualty incidents (MASCAL) in modern naval large-scale combat operations (LSCO) present unique triage challenges due to the confined environments, operational constraints, and saturation of medical resources. Combat Medical Early Warning Systems (CMEWS)-integrating wearable biosensors with automated early warning scores (EWS)-may support dynamic triage, but their use in a naval military setting has not been previously studied. MATERIALS AND METHODS: This prospective observational pilot study was conducted aboard a French Navy amphibious helicopter carrier during the POLARI

Veteran Reports of Anxiety and Depression Before, During, and After COVID-19: Associations With Race/Ethnicity, Gender, and Traumatic Exposures.

The COVID-19 pandemic was a world-wide health emergency that resulted in individuals experiencing challenges in numerous life domains. Life domains affected included physical and mental health, finances, and social isolation. Many health and research professionals evidenced concern that veterans were more likely than civilians to experience COVID-19 related problems due to their "at-risk" health status. Veterans are at-risk for health problems due to encountering unique military experiences such as traumatic exposures, development of trauma-related mental health symptoms or disorders, combat-r

Risk of longer-term endocrine and metabolic conditions in the Deepwater Horizon Oil Spill Coast Guard cohort study - five years of follow-up.

INTRODUCTION: Long-term endocrine and metabolic health risks associated with oil spill cleanup exposures are largely unknown, despite the endocrine-disrupting potential of crude oil and oil dispersant constituents. We aimed to investigate risks of longer-term endocrine and metabolic conditions among U.S. Coast Guard (USCG) responders to the Deepwater Horizon (DWH) oil spill. METHODS: Our study population included all active duty DWH Oil Spill Coast Guard Cohort members (N = 45,224). Self-reported spill exposures were ascertained from post-deployment surveys. The Psychological Consequences of Combat Injury Among U.S. Navy Health Care Personnel.

INTRODUCTION: Military health care personnel face numerous risks to mental health, including those associated with combat injury, although no study has described combat injuries within this subgroup or assessed their impact on mental health outcomes. MATERIALS AND METHODS: Male U.S. Navy-enlisted health care personnel, or corpsmen, with combat injury were identified from clinical records. Noninjured corpsmen were matched to injured corpsmen with a 4:1 ratio on year of deployment end or injury and location (Iraq or Afghanistan). The final study population included 2,025 corpsmen (405 injured a

Association of mild traumatic brain injury, post-traumatic stress disorder, and other comorbidities on photosensitivity.

SIGNIFICANCE: Photosensitivity is common after mild traumatic brain injury. However, this study demonstrates that photosensitivity is also impacted by common comorbidities that often occur with mild traumatic brain injury. Understanding how physical and psychological traumas impact photosensitivity can help improve provider care to trauma survivors and guide novel therapeutic interventions. PURPOSE: This study aimed to characterize the association between mild traumatic brain injury and common comorbidities on photosensitivity in post-9/11 veterans. METHODS: Existing data from the Translatio

Combat-related foot injuries: impact on gait and functional outcome.

INTRODUCTION: Prior to deployment of the Netherlands Army Task Force Urozgan in Afghanistan, the Dutch Military and civilian healthcare systems had limited experience in treating blast injuries and their long-term consequences. This meant that guidelines for treatment and rehabilitation were lacking. The aim of this cohort study was to quantify kinematic and kinetic abnormalities in service members with foot injuries in relation to functional outcome using gait analysis. METHOD: In nine service members with combat-related talus, calcaneus and/or navicular bone (TCN) fractures and nine control

Role of Pre-Morbid Factors and Exposure to Blast Mild Traumatic Brain Injury on Post-Traumatic Stress in United States Military Personnel.

Mild traumatic brain injury (mTBI), the signature injury of the recent wars in Afghanistan and Iraq, is a prevalent and potentially debilitating condition that is associated with symptoms of post-traumatic stress/post-traumatic stress disorder (PTS/PTSD). Prior mTBI, severity and type of injury (blast vs. non-blast), and baseline psychiatric illness are thought to impact mTBI outcomes. It is unclear if the severity of pre-morbid PTS/PTSD is a risk factor of post-injury levels of PTS and mTBI symptoms. The objective of the study was to examine predictors of post-injury PTS/PTSD, including pre-m

Ready before the call: Integration of civilian surgeons into a combat casualty care clinical readiness program for large-scale combat operations.

Large-scale combat operations will generate casualty volumes and injury patterns that fundamentally differ from routine civilian trauma, yet contingency planning increasingly relies on civilian surgeons and hospitals to deploy, absorb, and definitively treat combat-injured patients through frameworks such as the National Disaster Medical System and Health Care Personnel Delivery System. Decades of experience, from World War II and the Global War on Terrorism to the current war in Ukraine, demonstrate that civilian surgical volume alone does not reliably translate into readiness for combat casu

Examining associations between disordered eating and harmful substance use in a nationally representative sample of US veterans.

OBJECTIVE: The association between eating disorders (EDs) and harmful substance use (substance use that causes psychosocial impairment) is well recognized in the literature, and military veterans may be at heightened risk for both issues due to deployment-related stressors. However, little is known about which ED-related symptoms are associated with harmful substance use in veterans, and whether gender plays a differential role in this relationship. Our aims were to: (1) examine gender differences in ED-related symptoms; and (2) examine whether ED-related symptoms differentially predict harmfu
