Seven of eight dogs that had been previously infested with Sarcoptes scabiei var. canis and then cured, expressed protective immunity when experimentally reinfested with scabies. All seven dogs that expressed resistance were spontaneously cleared of scabies by 64 days after they were experimentally reinfested. Five of the eight dogs were free of scabies by 24 days. Crusted scabies is a rare manifestation of scabies characterized by uncontrolled proliferation of mites in the skin. In immunocompromised patients, this infestation is characterized by crusted lesions. The occurrence of the disease in human immunodeficiency virus-infected patients and the widespread use of immunosuppressive agents has led to a renewed interest in the disease. Early recognition and treatment is necessary to avoid an outbreak of scabies. This review highlights the pathogenesis, predisposing factors, clinical features and treatment of crusted scabies.

We present a 35-year-old patient with acquired immunodeficiency syndrome who had demodicidosis on his face, characterized by multiple papules and papulopustules, associated pruritus, numerous mites on skin-surface biopsy and in biopsy specimens, and rapid response to topical treatment with permethrin. It seems likely that Demodex infestation does not manifest unless local or systemic immune function is altered, leading to the proliferation of the organism and subsequent disease.

A patient who had recently had renal transplant was on a maintenance regimen of azathioprine and prednisone. She developed a florid, scaling, papular eruption clinically identical to Darler disease. Biopsy specimens and skin scrapings, however, showed a scabietic infestation. We believe that this highly atypical presenation, which had several features found in Norwegian scabies, was due to muted inflammatory response that permitted a great proliferation of the mites.

A 30-year-old Thai woman with systemic lupus erythematosus developed crusting, erythematous scaling plaques on the scalp and neck. Histologic section showed numerous mites in the stratum corneum. The diagnosis was further confirmed by identification of Sarcoptes scabiei in scraping and potassium hydroxide preparations made from the cutaneous lesions. After applications of 25 per cent benzyl benzoate for one month, there was complete resolution of the parasitic infestation.

Demodicosis caused by infestation with Demodex folliculorum hominis is a parasitic acariasis disease which may affect nasal skin. Acne rosacea with demodicosis must be distinguished from nasal furunculi which require antiinflammatory, hyposensitizing treatment. If the latter fails, it is acna rosacea with demodicosis that must be suspected. The diagnosis is made by investigation of nasal skin scrapings. If the mites are found, the patient should be referred to dermatologist for specific treatment.
