Transitioning adolescents and young adults with a chronic health condition to adult healthcare - an exemplar program.

Pediatric specialists have successfully improved the longevity and quality of life of many children with chronic health conditions. As these children reach adolescence and young adulthood, the scope of their concomitant medical problems often include those typically seen in older patients. As a result, these individuals need continuing quality health care in focused adult healthcare facilities. This article describes the effective partnership between pediatric and adult healthcare providers to create and implement an exemplar Spina Bifida Transition Program. The processes, strategies and tools

A model for evaluating the sustainability of community-directed treatment with ivermectin in the African Program for Onchocerciasis Control.

Onchocerciasis is controlled by mass treatment of at-risk populations with ivermectin. Ivermectin is delivered through community-directed treatment (CDTI) approach. A model has been developed to evaluate the sustainability of the approach and has been tested at 35 projects in 10 countries of the African Program for Onchocerciasis Control (APOC). It incorporates quantitative and qualitative data collection and analysis, taking account of two factors identified as crucial to project sustainability. These are (i) the provision of project performance information to partners, and (ii) evidence-base

Optimal incentives for allocating HIV/AIDS prevention resources among multiple populations.

Many agencies, such as the United Nations Program on HIV/AIDS (UNAIDS), the World Health Organization (WHO), the World Bank, the U.S. President's Emergency Plan for AIDS Relief (PEPFAR), and the Global Fund to Fight AIDS, Tuberculosis and Malaria, provide funding to prevent HIV/AIDS infections worldwide. These funds are allocated at multiple levels, resulting in a highly complicated distribution process. An oversight agency allocates funds to various national-level decision-makers who then allocate funds to regional-level decision-makers who in turn distribute the monies to local organizations

Sickle cell hospital unit: a disease-specific model.

American urban hospitals often serve large populations of sickle cell disease (SCD) patients. Those hospitals that choose to implement an adult SCD-specific inpatient unit have the opportunity to acquire multiple operational benefits. Such units may ultimately reduce patient morbidity and mortality; improve timely access to quality medical care in a cost-effective manner; reduce overcrowding in the emergency department; and increase patient, family, physician, and payer satisfaction. SCD is a serious, painful, genetic blood disorder that affects a growing population of adults in the United Sta

Reducing mortality from anthrax bioterrorism: strategies for stockpiling and dispensing medical and pharmaceutical supplies.

A critical question in planning a response to bioterrorism is how antibiotics and medical supplies should be stockpiled and dispensed. The objective of this work was to evaluate the costs and benefits of alternative strategies for maintaining and dispensing local and regional inventories of antibiotics and medical supplies for responses to anthrax bioterrorism. We modeled the regional and local supply chain for antibiotics and medical supplies as well as local dispensing capacity. We found that mortality was highly dependent on the local dispensing capacity, the number of individuals requiring

School-based programs for preventing eating disturbances.

This paper describes a framework for involving schools in primary and secondary prevention of eating disturbances. The issues of why, what, who, and how are considered. Research on the prevalence and consequences of obesity, anorexia and bulimia nervosa, unhealthy dieting, and behaviors such as binge eating and purging indicates why prevention is necessary. Research on the etiology of eating disturbances also provided a basis for determining what factors need to be addressed. However, research has not adequately addressed the question of who should be targeted for prevention and how the topic

Telestroke care in resource-limited settings: A World Stroke Organization scientific statement.

BACKGROUND: Telestroke services represent a crucial strategy for expanding access to acute stroke care in regions with limited local stroke expertise. However, existing recommendations and guidelines are predominantly derived from evidence generated in high-income countries and based on generally resource-intensive models. Such models and evidence may not be directly transferable to resource-limited settings. This Scientific Statement aims to summarize current knowledge on telestroke in resource-limited settings, highlight key considerations for establishing networks, and provide practical rec

The Right Care Initiative: Promoting Accountable Care to Prevent Cardiovascular Death.

OBJECTIVE: We describe the rationale for and implementation of the Right Care Initiative and its University of Best Practices (UBP), a model designed to close the implementation gap between clinical evidence and the real-world management of cardiovascular risk factors. The model aims to move beyond institutional silos toward an accountable care model that emphasizes interorganizational partnerships and the systematic sharing of best practices to eliminate preventable mortality. METHODS: We summarize lessons from a 17-year learning collaborative involving clinicians, pharmacists, health plans,

Governing AI through character: moral agency and corporate authority in Anthropic's public texts.

INTRODUCTION: Commercial AI laboratories increasingly govern language models through documents that prescribe values, name dispositions, stage moral conflicts, and define acceptable forms of objection. In this article, we ask how such writing makes an AI assistant into a governable character and redistributes moral authority among the model, its producer, and its publics. METHODS: We analyze 14 Anthropic texts published between December 2022 and April 2026, selected from a documented frame of 130 producer-side publications. Through close reading, we identify four operations: constitutional in

Implementation by simulation; strategies for ultrasound screening for hip dysplasia in the Netherlands.

BACKGROUND: Implementation of medical interventions may vary with organization and available capacity. The influence of this source of variability on the cost-effectiveness can be evaluated by computer simulation following a carefully designed experimental design. We used this approach as part of a national implementation study of ultrasonographic infant screening for developmental dysplasia of the hip (DDH). METHODS: First, workflow and performance of the current screening program (physical examination) was analyzed. Then, experimental variables, i.e., relevant entities in the workflow of sc

Understanding the impact of the unique stressors of National Guard service: the National Guard Occupational Demands Model.

The National Guard (NG) is a vital component of the U.S. Armed Forces, balancing federal and state responsibilities that serve and protect the Nation. Those who serve in the NG have a unique and understudied role within the military. As part-time service members, NG must balance the functional demands of their civilian roles and responsibilities with their military roles, and obligations. Despite their distinct service experience, NG research, intervention and policy often fail to recognize important factors that distinguish these service members from their active-duty military peers, as well

Toward a Holistic Framework for the Integration of Internationally Educated Nurses Into the Canadian Nursing Workforce: Insights From Key Partners in Nova Scotia.

Recruiting internationally educated nurses (IENs) has been recognized as a policy strategy to address Canada's longstanding nursing shortage. Despite collective efforts, IENs continue to face challenges in integrating into the Nova Scotia workforce. A key gap is the paucity of data on the integration experiences of IENs from both IENs' and systems partners' perspectives in Nova Scotia. Using the socioecological model as a theoretical underpinning, our mixed methods study addresses this gap by identifying key priorities and support strategies to address barriers to IENs' integration into the No

Density-Energy Balanced Selection of Chiral Honeycomb Superstructures from Deprotonated Carboxylate Molecules on Ag(111).

Here, we investigate the self-assembly of 4,4',4″-(1,3,3a1,4,6,7,9-heptaazaphenalene-2,5,8-triyl) tribenzoic acid on Ag(111) by scanning tunneling microscopy. Upon annealing, complete deprotonation drives the formation of a family of periodic honeycomb superstructures, denoted as HCN (N = 1-8), which are composed of triangular molecular sublattices and exhibit pronounced organizational chirality. By controlling the molecular coverage and annealing conditions, we identify the coexistence of HC4 and HC5 as the thermodynamically favored state near equilibrium. An analytical model incorporating th

Using Statistical Learning to Examine Variables that Contribute to Longer Hospital Stays After Endometrial Cancer Surgery.

Endometrial cancer, primarily arising from the lining of the uterus (endometrium), is the most common gynecological malignancy in many developed countries. Risk factors include obesity, age, and a family history of related cancers. The prognosis is generally favorable when detected early. One important measure of clinical results and resource use is the duration of hospital stay (LOS) after surgery. Understanding the causes of prolonged hospital stays is crucial for improving patient care because both organizational and patient-related factors can have a substantial impact on LOS. This study u

Neuroendocrine consequences of androgen excess in female rodents.

Androgens exert significant organizational and activational effects on the nervous system and behavior. Despite the fact that female mammals generally produce low levels of androgens, relative to the male of the same species, increasing evidence suggests that androgens can exert profound effects on the normal physiology and behavior of females during fetal, neonatal, and adult stages of life. This review examines the effects of exposure to androgens at three stages of development--as an adult, during early postnatal life and as a fetus, on reproductive hormone secretions in female rats. We exa
