Trustworthy deep learning framework for the detection of abnormalities in X-ray shoulder images.

Musculoskeletal conditions affect an estimated 1.7 billion people worldwide, causing intense pain and disability. These conditions lead to 30 million emergency room visits yearly, and the numbers are only increasing. However, diagnosing musculoskeletal issues can be challenging, especially in emergencies where quick decisions are necessary. Deep learning (DL) has shown promise in various medical applications. However, previous methods had poor performance and a lack of transparency in detecting shoulder abnormalities on X-ray images due to a lack of training data and better representation of f

Upper Sternal Cleft Associated With Congenital Cardiac Defect: Single Stage Surgical Correction.

Sternal cleft is a rare congenital malformation of anterior chest wall resulting from the failure of mesodermal migration to the midline during early embryological development. It may occur as an isolated anomaly or in connection with other congenital defects, most commonly of the cardiovascular system. The authors report the case of a 6-month-old girl with an upper sternal cleft combined with ventricular and atrial septal defects, who underwent single-stage surgical correction. The cosmetic and functional outcomes of the reconstruction were very good, with a stable anterior chest wall. The ch

Reinforcing the vascular disruption theory of the genesis of Poland's syndrome: a rare association of diaphragmatic eventration in a preterm infant with severe musculoskeletal defects.

A preterm female infant was admitted at birth with respiratory distress. On examination, she had an asymmetric right chest wall and ipsilateral small hand. Air entry was reduced over the right chest. A clinical diagnosis of Poland's syndrome was made based on the hypoplasia of the right pectoral muscles, absent nipple, deformed ribs and symbrachydactyly of the ipsilateral hand. Chest X-ray suggested and ultrasound confirmed eventration of the right hemidiaphragm. 'Subclavian artery supply disruption sequence' (SASDS) theory by Bavnick and Weaver remains the most accepted pathogenic mechanism i

Expanding the phenotypic and genotypic spectrum of Wiedemann-Steiner syndrome: First patient from India.

Wiedemann-Steiner syndrome (WWS) is a rare disorder characterized by hypotonia, postnatal growth restriction, striking facial dysmorphism, and hirsutism. It is caused by heterozygous pathogenic variants in KMT2A. This gene has an established role in histone methylation, which explains the overlap of WWS with syndromes caused by genes involved in chromatin remodeling. We describe an infant with a novel single base pair deletion in KMT2A with features consistent with WWS, as well as additional features of stenosis of aqueduct of Sylvius and broad toes. The usefulness of Face2Gene as a tool for i

Osteochondral Abnormalities on Three-Dimensional Ultrashort Echo Time MRI Scans Are Associated with Knee Cartilage Degradation.

Background The calcified cartilage layer and subchondral bone plate (SBP) contribute to osteoarthritis development. Three-dimensional (3D) ultrashort echo-time (UTE) MRI can help to evaluate calcified cartilage and SBP in various stages of cartilage degradation. Purpose To compare calcified cartilage and SBP abnormalities using 3D UTE MRI with cartilage degradation and osteochondral junction (OCJ) abnormalities observed at proton-density fast spin-echo with fat suppression (PDFS) MRI. Materials and Methods Between April 2018 and March 2019, 143 participants were prospectively enrolled in this

Skeletal muscle abnormalities and genetic factors related to vertical talus.

BACKGROUND/RATIONALE: Congenital vertical talus is a fixed dorsal dislocation of the talonavicular joint and fixed equinus contracture of the hindfoot, causing a rigid deformity recognizable at birth. The etiology and epidemiology of this condition are largely unknown, but some evidence suggests it relates to aberrations of skeletal muscle. Identifying the tissue abnormalities and genetic causes responsible for vertical talus has the potential to lead to improved treatment and preventive strategies. QUESTIONS/PURPOSES: We therefore (1) determined whether skeletal muscle abnormalities are pres

Clinical-genetic analysis of selected genes involved in the development of the human skeleton in 128 Czech patients with suspected congenital skeletal abnormalities.

BACKGROUND: Congenital skeletal abnormalities are a heterogeneous group of diseases most commonly associated with small or disproportionate growth, cranial and facial dysmorphisms, delayed bone maturation, etc. Nonetheless, no detailed genotype-phenotype correlation in patients with specific genetic variants is readily available. Ergo, this study focuses on the analysis of patient phenotypes with candidate variants in genes involved in bone growth as detected by molecular genetic analysis. METHODS: In this study we used molecular genetic methods to analyse the ACAN, COL2A1, FGFR3, IGFALS, IGF

Magnetic Resonance Imaging of Axial and Peripheral Disease in Psoriatic Arthritis: A Report From the 2020 GRAPPA Annual Meeting.

Psoriatic arthritis (PsA) presents with diverse features of musculoskeletal inflammation that affect both axial and peripheral joints as well as entheses, tenosynovium, and bursae. Magnetic resonance imaging (MRI) is the imaging modality that is uniquely capable of identifying pathology in all these structures. The Group for Research and Assessment of Psoriasis and Psoriatic Arthritis (GRAPPA) Magnetic Resonance Imaging Working Group has increasingly explored diverse MRI methodologies for the purposes of quantifying inflammatory and structural abnormalities in clinical trials and research. The recent outbreak of ZIKV in Brazil called the attention of the world because the effects of viral infection in the brain under development in fetuses. Consequences of vertical infection comprise brain malformation, especially microcephaly, eye and musculoskeletal abnormalities, among others. In adults, outcomes of infection include meningoencephalitis and Guillain-Barré Syndrome. Recent data specific suggest that neural progenitor cells are the main targets of ZIKV infection, causing massive cellular death and impairment in the neurogenesis process. Here we review the fetal and adult brain …

MR imaging appearance of rheumatoid arthritis in the foot.

Although conventional radiographs remain the initial mainstay for imaging of the foot in patients with rheumatoid arthritis (RA), magnetic resonance (MR) imaging has afforded the ability to detect early signs of the disease (i.e., synovitis, tenosynovitis, bone lesions, and bursitis), especially at the forefoot. In addition, the relatively symmetric distribution of the imaging abnormalities depicted in the metatarsophalangeal joints and the frequent involvement of the retro-calcaneal bursitis are almost specific for RA. In more advanced stages of the disease, MR imaging is well suited to evalu

Left-sided gastroschisis and pseudoexstrophy: a rare combination of anomalies.

Pseudoexstrophy is a rare variant of the exstrophy/epispadias complex, comprised of the musculoskeletal defects of classic exstrophy but with an intact bladder and urethra. We present a case of a neonate with left-sided gastroschisis and pseudoexstrophy, the first reported combination of these two anomalies. The anomalies in this case were unusual but highlight the importance of careful clinical evaluation before and during surgery so as to carefully define the anatomy of the abnormalities, which will assist in planning surgery and further postoperative management. Habitual toe-walking: evaluation and approach to treatment.

Habitual toe-walking has been presented as a prolongation of a normal stage of development that requires conservative treatment to prevent or ameliorate associated gait abnormalities such as tripping and falling. An approach to the evaluation of a child with toe-walking should include (1) medical history (prenatal, intrapartum, and postnatal), (2) gait evaluation, (3) musculoskeletal examination, and (4) neurologic examination. Pathologic entities producing toe-walking have been explored in order to differentiate those conditions from idiopathic (habitual) toe-walking. The most common etiologi

Approach to Adult Patients with Musculoskeletal Complaints and Normal Findings: A Guide for Clinical Practice.

BACKGROUND: Musculoskeletal (MSK) complaints are among the most common reasons for visits to primary care and outpatient clinics. While many cases are attributed to identifiable pathology, a significant proportion of patients present with persistent MSK symptoms despite normal physical examinations and imaging findings. These presentations pose diagnostic and management challenges and often result in patient frustration, clinician uncertainty, and overutilization of healthcare resources. OBJECTIVE: This narrative review aims to provide a comprehensive, evidence-informed, and practical framewo

Ultrasound Versus Magnetic Resonance Imaging in the Evaluation of Shoulder Pain in End Stage Renal Disease Patients on Chronic Hemodialysis.

BACKGROUND: Musculoskeletal pain is common in hemodialysis (HD) patients and may be related to articular or periarticular amyloid deposition. The shoulder is one of the most common afflicted joints, but not all causes of shoulder pain are detectable on radiography, and magnetic resonance imaging (MRI) is not always available. The aim of this study was to evaluate the validity of musculoskeletal ultrasound (MSUS) to properly detect shoulder disorders in HD patients by identifying US abnormalities in the shoulder and comparing them to those identified on MRI, with MRI serving as the gold standar

Spasmodic muscle cramps and weakness as presenting symptoms in Wilson disease.

Wilson disease (WD) is an autosomal-recessive disorder of hepatic copper metabolism that has tremendous variability in its presentation. Phenotypic diversity of the disease can lead to delayed diagnosis. We describe a case of WD in a 10-year-old boy presenting with 3 months of increasingly intense, spasmodic lower extremity muscle cramps. Physical examination revealed tenderness on calf palpation and dark flat lesions over his ankles, knees, and elbows. Initial testing revealed creatine kinase of 302 IU/L (normal 24-248 IU/L), hemoglobin of 8.9 g/dL (11.5-15.5 g/dL), aspartate aminotransferase
