Drug resistance of Mycobacterium tuberculosis in West Java, Indonesia.

Tuberculosis (TB) is currently one of the leading causes of infectious disease deaths globally, and Indonesia ranks 2nd in annual TB cases, below only India. Accurate TB diagnosis and detection of multidrug-resistant TB (MDR-TB) in real-world settings are crucial for prompt treatment and surveillance. We therefore compared multiple methods for TB detection and drug resistance profiling, including a cartridge-based nucleic acid amplification test (CBNAAT), line probe assay (LPA), and phenotypic drug susceptibility testing (pDST) with targeted long-read next generation sequencing (tNGS) and whol

Uncommon pulmonary mycobacterium wolinskyi infection in a hemodialysis patient.

In recent years, the detection rate of nontuberculous mycobacteria has increased, drawing attention in clinical settings. Here, we present a case of pulmonary Pseudomonas aeruginosa and Mycobacterium wolinskyi coinfection in a 60-year-old hemodialysis recipient. To the best of our knowledge, this is the first instance of isolating the M. wolinskyi strain from human respiratory samples, including a comprehensive exploration of its susceptibility to clinically available antimicrobials. This discovery deepened our understanding of the infection spectrum of M. wolinskyi, highlighting the need to c…

MmpL12 transports lipooligosaccharides and impacts virulence in Mycobacterium marinum.

Lipooligosaccharides (LOSs) are polar glycolipids found in the cell envelope of many pathogenic mycobacteria. Here, we show that LOS transport in Mycobacterium marinum requires mmpL12, a member of the resistance-nodulation-division family of membrane proteins. Deletion of mmpL12 resulted in a rough colony morphology and increased hydrophobicity. The △mmpL12 mutant accumulated three of the biosynthesis intermediates of LOSs (LOS-I, LOS-II and LOS-III) intracellularly and failed to produce the final product, LOS-IV, suggesting that final glycosylation of LOS-III to yield LOS-IV occurs extracellu…

Comparative Analysis of Evolutionary Distances Using the Genus Mycobacterium.

Infections caused by nontuberculous mycobacteria are becoming significant due to the increasing number of vulnerable individuals worldwide. Understanding the evolutionary relationships within the genus Mycobacterium is critical for improving species identification and, consequently, enhancing diagnosis, treatment, and epidemiological tracking. Pairwise comparisons of average nucleotide identity, genome-genome distance calculations, Mash values, multilocus sequence analyses, and average amino acid identities (AAIs) revealed that the AAI metric is the best to distinguish Mycobacterium from other…

Proteomic characterization of Mycobacterium tuberculosis subjected to carbon starvation.

Mycobacterium tuberculosis ( Mtb ) is the causative agent of tuberculosis (TB), the leading cause of infectious-disease related deaths worldwide. TB infections present as a spectrum from active to latent disease. In the human host, Mtb faces hostile environments, such as nutrient deprivation, hypoxia, and low pH. Under these conditions, Mtb can enter a dormant, but viable, state characterized by a lack of cell replication and increased resistance to antibiotics. These dormant Mtb pose a major challenge to curing infections and eradicating TB globally. In the current study, we subjected Mtb to …

Atypical Mycobacterium Skin Infection in a Heart Transplant Patient.

Atypical mycobacterium infections are an uncommon cause of cutaneous infection, and they are especially rare infections of the facial region. Immunocompromised patients, such as transplant patients, are at higher risk for infections of this nature with concurrent hematogenous spread to other organ systems. We report a patient with a previous heart transplant who developed an atypical mycobacterium infection of the skin with possible dissemination to the lung. The case outlines the difficulty in diagnosing this condition along with the challenges of prescribing an effective drug regimen to trea…

Asymptomatic Tuberculosis in Children With Household Exposure to Mycobacterium tuberculosis.

BACKGROUND: Asymptomatic tuberculosis (TB) is common in adult community prevalence surveys. Evidence to inform our understanding of asymptomatic TB in children is lacking, because diagnostic algorithms typically include only symptomatic children. METHODS: Children aged 2-60 months, without human immunodeficiency virus (HIV), with household exposure to an adult TB patient in the last year, were enrolled in South Africa. Symptom screening, chest radiography (CXR) and induced sputum were performed. Confirmed TB (requiring positive Xpert Ultra and/or MGIT culture) and unconfirmed TB (requiring 2

Proteomic insights into the adaptation of Mycobacterium bovis to hypoxic conditions.

Bovine tuberculosis (bTB) is an important cattle disease with major public health and economic impacts. Mycobacterium bovis, its causative agent, is thought to persist in non-replicative forms within the host, similar to Mycobacterium tuberculosis, leading to chronic or latent infection. In this study, we used the Wayne model-an in vitro system that gradually depletes oxygen-to mimic the hypoxic conditions M. bovis may encounter during latency. Growth analysis showed that part of the bacterial culture remained viable but non-replicative under hypoxia, while another fraction likely lysed, as in…

Advances in functional transcriptome analysis of Mycobacterium tuberculosis: a review.

Drug-resistant tuberculosis poses a significant global challenge necessitating the prompt advancement of novel therapeutic options. Nonetheless, disease prognosis is contingent upon multiple factors. mRNA and other small RNAs are essential for gene regulation and disease progression. Additionally, they are essential for the advancement of TB mRNA therapies. The review aims to evaluate the functions of mRNA and various small RNAs, including lncRNA, miRNA, circRNA, and ceRNA, as interconnected components within the mRNA-miRNA-circRNA axis in Mycobacterium tuberculosis. In this context, the analy…

Coinfection by Mycobacterium tuberculosis and Rhodococcus equi in advanced HIV.

A 28-year-old man with HIV (diagnosed eight years prior, last CD4 count 23 cells/mm3) and prior tuberculosis (cured, then confirmedly reinfected with treatment default) presented with cachexia, diarrhea, chest pain, and tachypnea with increased work of breathing that required orotracheal intubation. Admission chest CT showed extensive bilateral consolidations with air bronchograms, suggestive of previous mycobacterial infection and/or possible new bacterial infection. Initially managed as sepsis, blood cultures grew Rhodococcus equi (four samples). Molecular testing confirmed Mycobacterium tub…

Phylogenetic distribution and longitudinal persistence of plasmids in Mycobacterium abscessus.

Mycobacterium abscessus, a non-tuberculous mycobacterium, is a cause of severe respiratory infections, notably in individuals with underlying lung conditions. Its high levels of intrinsic and acquired antimicrobial resistance make it particularly difficult to treat and horizontally acquired genetic elements may facilitate the spread of resistance. A small number of plasmids have been identified in this species, but their distribution, transmission dynamics across subspecies and clonal lineages remain poorly characterized. We analysed short-read genomic data from 3,060 M. abscessus isolates, in…

A Case Report of Wound Infection Caused by Mycobacterium porcinum.

BACKGROUND: Mycobacterium porcinum is a rapidly growing non-tuberculous mycobacterium that requires 3 to 5 days of cultivation for colonies to become visible to the naked eye, with most exhibiting vigorous growth during this initial week. It does not readily grow in standard bacterial culture conditions, posing challenges that often result in missed or incorrect diagnoses, and necessitating an extended course of anti-infective treatment. CASE PRESENTATION: This case report describes a rare instance of wound infection caused by Mycobacterium porcinum in a 64-year-old female patient who underwe…
