Pharmacokinetics and safety of fosfomycin and flomoxef administered as part of neonatal sepsis treatment (NeoSep1 Part 1).

Neonatal doses for the off-patent antibiotics fosfomycin and flomoxef, which offer coverage against many extended-spectrum beta-lactamase (ESBL)-producing organisms, are based on limited data. We performed a pharmacokinetic (PK) and safety study of fosfomycin and flomoxef to confirm proposed neonatal dosing before further investigation in a trial (NeoSep1, ISRCTN48721236). Neonates with suspected sepsis, weighing more than 1,000 g, were sequentially enrolled into three antibiotic treatment cohorts: fosfomycin and amikacin (Cohort 1), flomoxef and amikacin (Cohort 2), and flomoxef and fosfomyci

Trends in neonatal sepsis: bacteriological profile, and antibiotic resistance pattern at a tertiary care hospital in Egypt.

INTRODUCTION: Neonatal sepsis is a life-threatening bloodstream infection that occurs within the first 4 weeks of life and represents a significant cause of illness and death, especially in developing countries. Regular assessment of the local causative agents and their resistance patterns is important for effective management. This study aimed to determine the microbiological profile of neonatal sepsis and its antibiotic resistance patterns. METHODOLOGY: Our study was conducted on 237 neonates suspected of sepsis. Blood samples were collected and inoculated into BACT/ALERT blood culture bott…

Trends of neonatal sepsis and its etiology at Hawassa, Ethiopia: a five year retrospective cross-sectional study.

BACKGROUND: Neonatal sepsis is a significant cause of morbidity and mortality in low- income countries. Neonatal sepsis is classified as early-onset neonatal sepsis (EONS) and late-onset neonatal sepsis (LONS). Etiologies responsible for EONS are mostly acquired vertically from the mother during or before birth with the possibility of prevention. The burden and etiology of neonatal sepsis is not uniform across the globe with huge disparities based on the income level of the countries. This study aimed to determine neonatal sepsis trends, prevalence, and etiologies at Hawassa University Compreh…

Clinical and Microbiological Profile of Culture-Positive Neonatal Sepsis in a Tertiary Care Center in North India.

Background Sepsis in newborns is a major clinical concern, especially in resource-limited settings. Although blood culture remains the primary method for confirming infection and guiding treatment, its detection rate is often suboptimal. The objective of this study was to evaluate the incidence, clinical characteristics, maternal and neonatal risk factors, and antimicrobial resistance patterns linked to culture-confirmed neonatal sepsis in a tertiary care facility in North India. Materials and methods This prospective observational study was carried out over 18 months in the neonatal intensive…

Evaluation of Empiric Nafcillin Versus Vancomycin for Suspected Late Onset Sepsis in the Neonatal Intensive Care Unit.

OBJECTIVE: To compare clinical outcomes in neonates receiving empiric vancomycin or nafcillin containing regimens for neonatal late onset sepsis (nLOS). METHODS: This retrospective study evaluated neonates > 72 hours of age initiated on vancomycin/gentamicin (VANC) or nafcillin/gentamicin (NAF) for suspected nLOS. The primary composite endpoint was clinical worsening, defined as a composite of increased respiratory support or vasopressor requirement, both within the first 24 hours, or prolonged bacteremia. Secondary endpoints included acute kidney injury, methicillin-resistant Staphylococcus

Tiny Joints, Big Clues: Neonatal Septic Polyarthritis as a Rare Presentation of Late-Onset Streptococcus agalactiae Sepsis.

Group B Streptococcus (GBS), or Streptococcus agalactiae, is a significant cause of neonatal morbidity and mortality, with late-onset disease (LOGBS) typically manifesting as bacteremia or meningitis. Osteoarticular infections, particularly septic polyarthritis, are exceedingly rare but potentially devastating complications of LOGBS. We report a rare case of LOGBS presenting as septic polyarthritis in a 28-day-old term male neonate. The infant presented with a 15-day history of progressive swelling in the left knee and right shoulder, accompanied by irritability and feeding difficulties. Ultra

Fulminant Early-Onset Neonatal Sepsis Due to Streptococcus gallolyticus (Group D): A Report of a Fatal Case.

Streptococcus enterococci group D is a rare cause of neonatal sepsis, and it is associated with significant morbidity and a very high fatality rate. The infection is usually acquired intra-partum, from the colonization of the maternal genital tract. Most affected neonates have an early-onset presentation of symptoms, usually within the first 48 hours after birth, which is similar to other causes of neonatal sepsis such as Streptococcus agalactiae or Escherichia coli. We describe an early preterm infant with a fatal, early-onset sepsis caused by Streptococcus D.

Preventing Early Onset GBS Neonatal Sepsis - Are Second Line Intrapartum Antibiotics Equivalent to No Antibiotics at All?

Strongly encourage pregnant women with the history of a penicillin allergy to undergo prenatal allergy testing as first-line intrapartum antibiotic prophylaxis for group B strep (GBS) is superior to second-line or no antibiotics, which have similar rates of early onset sepsis (EOS).

Development of risk prediction nomogram for neonatal sepsis in Group B Streptococcus-colonized mothers: a retrospective study.

Neonatal clinical sepsis is recognized as a significant health problem, This study sought to identify a predictive model of risk factors for clinical neonatal sepsis. A retrospective study was conducted from 1 October 2018 to 31 March 2023 in a large tertiary hospital in China. Neonates were divided into patients and controls based on the occurrence of neonatal sepsis. A multivariable model was used to determine risk factors and construct models. The utilization and assessment of model presentation were conducted using Norman charts and web calculators, with a focus on model differentiation, ca…

Timing of Symptoms of Early-Onset Sepsis after Intrapartum Antibiotic Prophylaxis: Can It Inform the Neonatal Management?

The effectiveness of "inadequate" intrapartum antibiotic prophylaxis (IAP administered < 4 h prior to delivery) in preventing early-onset sepsis (EOS) is debated. Italian prospective surveillance cohort data (2003-2022) were used to study the type and duration of IAP according to the timing of symptoms onset of group B streptococcus (GBS) and E. coli culture-confirmed EOS cases. IAP was defined "active" when the pathogen yielded in cultures was susceptible. We identified 263 EOS cases (GBS = 191; E. coli = 72). Among GBS EOS, 25% had received IAP (always active when beta-lactams were administe

Antimicrobial Resistance Pattern and Empirical Antibiotic Treatments in Neonatal Sepsis: A Retrospective, Single-Center, 12-Year Study.

Neonatal sepsis is an important cause of morbidity and mortality in neonatal intensive care units (NICUs). Continuous evaluation of antimicrobial resistance (AMR) profiles is advised to implement antimicrobial stewardship (AMS) programs and establish effective empiric antibiotic protocols. AMS may reduce AMR in NICUs and improve sepsis outcomes. In this retrospective observational study, we report data on culture-positive neonatal sepsis, assessing differences after the implementation of an AMS program (2011-2016 vs. 2017-2022). A total of 215 positive bacterial cultures from 169 infants were …

Gram-negative Late Onset Neonatal Sepsis in a Tertiary Care Center From Central India: A Retrospective Analysis.

BACKGROUND: Neonatal sepsis has been a major cause of neonatal mortality and morbidity globally. Late onset sepsis is on the rise mostly due to better health care services and improved survival of premature neonates. Gram-negative sepsis has emerged as a major public health problem constituting significant morbidity and mortality. There is limited data on gram-negative late onset sepsis from the central part of India, therefore this study was conducted at a tertiary care center from rural part of India. OBJECTIVES: To determine the clinical profile and outcome among neonates with gram-negativ…

Case report: Fatal neonatal sepsis associated with Escherichia fergusonii infection in a common bottlenose dolphin (Tursiops truncatus).

A 25-day-old male common bottlenose dolphin (Tursiops truncatus) died suddenly while swimming at a dolphinarium. The gross examination revea … th in this animal was E. fergusonii sepsis. To the best of our knowledge, this is the first case of neonatal sepsis associated with E. fergusonii infection in a dolphin, and suggests E. fergusonii as an opportunistic pathogen associated with sepsis in dolphins.

Reliability of Interleukin-6 Alone and in Combination for Diagnosis of Early Onset Neonatal Sepsis: Systematic Review.

Neonatal sepsis is a major cause of morbidity and mortality in both preterm and term infants. Early-onset neonatal sepsis (EONS) presents within the first 72 h of life. Diagnosis is difficult as signs and symptoms are non-specific, and inflammatory markers are widely used to confirm or rule out neonatal sepsis. Interleukin-6 (IL-6) is part of the fetal inflammatory response syndrome (FIRS) and therefore an interesting early marker for neonatal sepsis. The main objective for this review was to assess the diagnostic potential of IL-6, alone and in combination, for diagnosis of early neonatal sep…

Bacteriological Profile and Outcome of Culture-Positive Neonatal Sepsis in a Special Newborn Care Unit Setting, Odisha.

INTRODUCTION: Nearly one-third of neonatal mortality in India is due to neonatal sepsis and death occurs in 30% of culture-positive neonates. Pathogens such as Klebsiella pneumoniae and Escherichia coli are the most common bacteria responsible for neonatal sepsis in India and South Asia. MATERIALS AND METHODS: It was an observational study, conducted in special newborn care units (SNCUs) of Capital Hospital in Bhubaneswar, Odisha from May 2017 to October 2019. All neonates (<28 days of life) with blood culture-positive sepsis were included in this study. Blood cultures were sent in all the ba…

Detection of pathogens associated with early-onset neonatal sepsis in cord blood at birth using quantitative PCR.

Background: Early onset neonatal sepsis (EONS) typically begins prior to, during or soon after birth and may be rapidly fatal. There is paucity of data on the aetiology of EONS in sub-Saharan Africa due to limited diagnostic capacity in this region, despite the associated significant mortality and long-term neurological impairment. Methods: We compared pathogens detected in cord blood samples between neonates admitted to hospital with possible serious bacterial infection (pSBI) in the first 48 hours of life (cases) and neonates remaining well (controls). Cord blood was systematically collected…

nSeP: immune and metabolic biomarkers for early detection of neonatal sepsis-protocol for a prospective multicohort study.

INTRODUCTION: Diagnosing neonatal sepsis is heavily dependent on clinical phenotyping as culture-positive body fluid has poor sensitivity, and existing blood biomarkers have poor specificity.A combination of machine learning, statistical and deep pathway biology analyses led to the identification of a tripartite panel of biologically connected immune and metabolic markers that showed greater than 99% accuracy for detecting bacterial infection with 100% sensitivity. The cohort study described here is designed as a large-scale clinical validation of this previous work. METHODS AND ANALYSIS: Thi…

Ralstonia mannitolilytica sepsis in neonatal intensive care unit - Be(a)ware of the multidrug resistant nosocomial bug.

Ralstonia mannitolilytica, an emerging opportunistic pathogen is rarely isolated in neonatal units. We have elaborated our experience of managing its recent septic outbreak affecting four neonates in our intensive care unit over a span of five days. Three of the four had extremely low birth weight and had secondary clinical deterioration in the form of recurrent apnoea requiring intubation. All had thrombocytopenia. Their median age at clinical deterioration was 8 (4-12) days. The organism was multidrug resistant, but uniformly sensitive to flouroquinolones and co-trimoxazole, on which all rec
