Single upper-pole percutaneous access for treatment of > or = 5-cm complex branched staghorn calculi: is shockwave lithotripsy necessary?

BACKGROUND AND PURPOSE: Percutaneous nephrolithotomy for staghorn calculi is reported to have a residual stone rate of 28%, while shockwave lithotripsy alone results in residual stones in approximately 50% of cases. Combination therapy, sandwich therapy, and multiple percutaneous accesses have also been advocated for staghorn stones. We believe these stones can often be removed with a staged procedure via a single upper-pole percutaneous access using flexible nephroscopy and the holmium:YAG laser. Our experience is reviewed. PATIENTS AND METHODS: The hospital records of patients having a cumu

High attenuation value in non-contrast computer tomography can predict pyonephrosis in patients with upper urinary tract stones.

To evaluate whether the higher attenuation value [Hounsfield unit (HU)] in non-contrast CT can predict pyonephrosis in patients with upper urinary tract stones (UTS). Between October 2019 and October 2021, patients with hydronephrosis or pyonephrosis secondary to upper UTS were retrospectively searched in our study. All patients with UTS were treated with percutaneous nephrostomy, percutaneous nephrolithotomy, retrograde ureteral stent or transurethral ureteroscope lithotripsy. We excluded patients treated with extracorporeal shock-wave lithotripsy. Patients whose CT was not performed in our h

Squamous cell carcinoma of the renal pelvis presenting as an integumentary neoplasm of the flank: A case report.

RATIONALE: Although chronic pyelonephritis and urolithiasis are established risk factors for squamous cell carcinoma (SCC), only a minority of patients with chronic urolithiasis eventually develop SCC. It is believed that the chronic irritation leads to squamous cell metaplasia that may subsequently develop into SCC. Although studies show that SSC generally spreads locally with associated symptoms of lymphadenopathy, metastasis to the lungs and liver have also been reported. However, cases spreading to the flank have yet to be reported. Therefore, the use of reconstructive techniques for the r

Are there any predictors of pyonephrosis in patients with renal calculus disease?

The objective of the study is to identify factors predicting development of pyonephrosis in patients of renal calculus disease (RCD), as this knowledge is largely unknown. Patients of RCD without pyonephrosis (Group 1) or with pyonephrosis (Group 2) presenting between December 2013 and November 2015 were evaluated. All patients of RCD who had undergone either percutaneous nephrostomy (PCN) or surgical management (percutaneous nephrolithotomy/pyelolithotomy/nephrectomy) were included. Patients treated conservatively, by extracorporeal shock-wave lithotripsy and patients of bilateral RCD were ex

Laparoscopy assisted percutaneous stone surgery can be performed in multiple ways for pelvic ectopic kidneys.

Pelvic kidney stones remain a unique challenge to the endourologists. Treatment options include open surgery, extracorporeal shockwave lithotripsy, percutaneous nephrolithotomy (PNL), retrograde intrarenal surgery, and laparoscopy assisted PNL (LA-PNL). As a minimal invasive option, LA-PNL can decrease the risk for bowel and major vessel injury. Here, we describe our experience using the LA-PNL procedures with different combinations, to treat kidney stones in multiple patients with a pelvic ectopic kidney (PEK). Eight patients, with PEK, kidney stones, and no other treatment choice, but open s

Cross-sectional imaging of iatrogenic complications after extracorporeal and endourological treatment of urolithiasis.

Extracorporeal shock wave lithotripsy (ESWL), percutaneous nephrolithotomy (PCNL) and ureteroscopy (URS) currently represent the mainstay treatment options for the vast majority of patients with urolithiasis, with limited contraindications and high success rates. However, minimally invasive extracorporeal and endourological treatments are associated with a non-negligible morbidity including occasional life-threatening occurrences. These complications represent a source of concern for urologists since they may result in prolonged hospitalisation, need for surgical, endoscopic or interventional

A Rare Case of Emphysematous Pyelonephritis Following Retrograde Intrarenal Surgery in an Elderly Man.

BACKGROUND Emphysematous pyelonephritis (EPN) is a severe, rare urinary tract infection (UTI) primarily affecting patients with diabetes or compromised immune function. Characterized by nonspecific manifestations including fever, flank pain, and pyuria, it frequently overlaps with other renal or abdominal conditions, leading to diagnostic delays and increased mortality risk. Notably, EPN following retrograde intrarenal surgery (RIRS) - a minimally invasive technique for renal and ureteral stone management - is extremely rare, with no prior documented cases in the literature. CASE REPORT A 74-y

Retrograde vs. antegrade fl exible nephroscopy for detection of residual fragments following PNL: A prospective study with computerized tomography control.

INTRODUCTION: The main aim of stone surgery is to establish stone free status. Performing fl exible nephroscopy is an effective tool in this manner. The aim of this study was to evaluate the role of retrograde fl exible nephroscopy for detection of residual fragments following percutaneous nephrolithotomy (PNL) in comparison with antegrade approach. MATERIALS AND METHODS: Data of 137 patients underwent ECIRS was collected prospectively. In all cases following stone clearance, collecting system was checked for residual fragments. First antegrade than retrograde fl exible nephroscopy was perfor

Pulsed solid-state thulium: YAG laser compared to holmium: YAG laser during mini-PCNL.

BACKGROUND: Holmium: yttrium-aluminum-garnet (Ho: YAG) laser technology has long been employed for urinary stone fragmentation; however, its limitations have prompted the exploration of alternative systems such as thulium lasers. This study aims to compare a novel pulsed solid-state Thulium: YAG (Tm: YAG) laser to the standard Ho: YAG laser in Mini-percutaneous nephrolithotomy (Mini-PCNL) lithotripsy for renal calculi. METHODS: In total, 100 patients undergoing Mini-PCNL were enrolled in this clinical trial. Fifty patients treated with a holmium laser were selected from a retrospective databa

[Non-vascular interventional radiology nonvascular interventional radiology of the urinary tract].

Several kinds of nonvascular interventional radiology of the urinary tract are reviewed. Transurethral balloon dilation of the prostate (TU-DP) is a newly developed nonsurgical treatment for benign prostate hyperplasia, which is performed under local anesthesia with minimal morbidity and requires no hospitalization. The TUDP technique involves dilating the prostatic urethra and bladder neck to 75 F for 10 minutes under fluoroscopic control using a high-pressure dilating balloon. Long-term follow-up studies are required. Percutaneous nephrolithotomy (PNL) and transurethral ureterolithotomy (TUL

Navigating the Complexities of Symptomatic Hydronephrosis in Pregnancy.

The management of symptomatic hydronephrosis presents substantial challenges due to the absence of consensus within clinical guidelines concerning pain management, diagnostic approaches, therapeutic interventions, and follow-up protocols. This literature review focuses on complexities involving diagnostic challenges that arise from the difficulty in distinguishing physiological from pathological obstruction and treatment complexities that involve deciding on the most appropriate pain management medications and safe interventions while minimizing risks to both the mother and foetus. To address

Laparoscopy and ureteroscopy cooperative surgery for high burden stones in solitary kidney and ureter in one stage: A case report.

Treatment of kidney and ureter multiple calculi is a difficult procedure in urology. It is especially difficult to eliminate the high burden stones in a one-stage operation. When a patient has had only one kidney since he/she was born (a condition termed 'solitary kidney'), the conservation of the renal function is especially important. A series of combined surgery techniques have been developed, including endoscopic combined intrarenal surgery, extracorporeal shock wave lithotripsy sandwich therapy and laparoscopy-assisted percutaneous nephrolithotomy, but not laparoscopy or endoscopy coopera

Current Perspectives on Urolithiasis: Pathogenesis, Clinical Management, and Treatment.

Urolithiasis is a common and recurrent urological condition with significant clinical and economic implications worldwide. In light of new evidence on evaluating and managing urolithiasis, this review summarizes the current literature and guidelines. The development of urolithiasis is influenced by a complex interplay of dietary, metabolic, environmental, and genetic factors, with calcium-based stones constituting the majority of cases. Patients may present with a wide spectrum of symptoms, ranging from severe renal colic to incidental, asymptomatic findings. Accurate diagnosis requires the ap

Urinothorax Secondary to Obstructive Uropathy: A Rare Cause of Pleural Effusion.

Urinothorax is an exceptionally rare cause of pleural effusion characterized by the abnormal accumulation of urine in the pleural space, typically secondary to genitourinary trauma or obstructive uropathy. We report the case of a 54-year-old male who presented with acute right-sided chest pain, colicky abdominal pain, and rapidly worsening respiratory distress. Serial imaging demonstrated a rapidly refilling massive loculated pleural effusion alongside a 3.3 cm right pelviureteric junction calculus, causing moderate hydroureteronephrosis. Ultrasound-guided thoracentesis yielded an exudative fl

Incidental giant renal schwannoma misdiagnosed as angiomyolipoma and low-grade spindle cell neoplasm: A case report.

INTRODUCTION AND IMPORTANCE: Renal schwannomas are extremely rare benign tumors of the peripheral nerve sheath, accounting for less than 1 % of renal masses. Due to their nonspecific clinical and radiologic features, they are frequently misdiagnosed, posing a diagnostic challenge. CASE PRESENTATION: We report a 63-year-old male with chronic right flank pain and a right staghorn calculus. During preoperative evaluation for percutaneous nephrolithotomy (PCNL), a large retroperitoneal mass adjacent to the left kidney was incidentally found. Initial core needle biopsy suggested angiomyolipoma, la

A novel method of inducing thrombosis in target arteries using coronary stents for therapeutic occlusion.

OBJECTIVES: Peripheral artery embolizations are often required for therapeutic purposes. The therapeutic embolizations are usually performed using embolization particles and coils. These procedures are also associated with complications. Coils and their delivery cables, and the expertise are not always available in all catheterization centers. Hence, a novel, simple controlled technique for arterial closure would be useful in emergency settings. RESULTS: Following is a report of seven cases where embolization was performed successfully on an emergency basis after stenting using coronary stent
