The pathogenesis of retinal detachment with morning glory disc and optic pit.

A child with nonrhegmatogenous retinal detachment associated with morning glory disc underwent first a vitrectomy and then, some months later, an optic nerve sheath fenestration. The latter procedure led to retinal reattachment. It also produced a biopsy specimen that confirmed the perineural herniation of poorly differentiated retinal tissue in this condition, similar to that in congenital pit of the optic nerve. It demonstrated continuity of the vitreous cavity with the perineural space, both histologically and by the fact that gas injected through the pars plana into the vitreous cavity bub

A hydrogel ionic circuit electrical stimulation system to restore denervated muscle following long-gap peripheral nerve injury.

Electrical stimulation (EStim) has been used to exercise denervated muscle pending nerve regeneration, to prevent muscle atrophy. However, higher EStim intensity is required to achieve maximal therapeutic efficacy, which is challenging because conventional electrodes generate heat and alter pH, potentially causing tissue damage. Here, we developed and utilized a novel hydrogel ionic circuit (HIC)-based EStim system that is ionically conductive and prevents tissue damage. This HIC was safely used to deliver a high EStim intensity of 32 mA both in vitro and in vivo without substantial cell death

Adenoid cystic carcinoma of the parotid gland presenting as temporal bone neoplasm: a case report.

PROBLEM: Temporal bone tumours are rare head and neck cancers that may involve both the parotid and the neck. In cases with concomitant temporal bone and parotid tissue involvement, tumour origin determination can be challenging. A tumour with parotid and temporal bone involvement is usually of temporal origin. The tumour may also be a rare parotid neoplasm with intratemporal invasion through the facial nerve, such as adenoid cystic carcinoma. These are slow-growing, yet locally aggressive tumours marked by late-stage distant metastases. CASE REPORT: We describe a case of parotid adenoid cyst

The ultrastructure of sensory nerve endings in the human knee joint capsule.

The ultrastructure of sensory nerve endings in the human knee joint capsule was studied. Three types of nerve endings were found: free nerve endings (FNE), Ruffini corpuscles and Pacini corpuscles. In the joint capsule, FNE are located below the synovial layer and within the fibrous layer near blood vessels. These nerve terminals derive from myelinated A delta-fibres or from unmyelinated C-fibres. Their structure is almost identical to FNE in human hairy and non-hairy skin. Ruffini corpuscles are present within the fibrous layer and the ligaments of the capsule in three variations: small Ruffi

Next-generation RNA sequencing elucidates transcriptomic signatures of pathophysiologic nerve regeneration.

The cellular and molecular underpinnings of Wallerian degeneration have been robustly explored in laboratory models of successful nerve regeneration. In contrast, there is limited interrogation of failed regeneration, which is the challenge facing clinical practice. Specifically, we lack insight on the pathophysiologic mechanisms that lead to the formation of neuromas-in-continuity (NIC). To address this knowledge gap, we have developed and validated a novel basic science model of rapid-stretch nerve injury, which provides a biofidelic injury with NIC development and incomplete neurologic reco

Analysis of biological functional networks during sciatic nerve repair and regeneration.

Peripheral nerve injury is a common disease with a low recovery rate. A better understanding of the molecular changes underlying peripheral nerve injury and regeneration may contribute to the development of novel therapies for the treatment of peripheral nerve injury. In the current study, we analyzed differentially expressed genes in rat sciatic nerve stumps at 1, 4, 7, and 14 days post nerve crush and built biological functional networks at each time point. Our outcomes suggested that "Neurological Disease" involved networks were significant at 1 day post nerve crush, "Cellular Assembly and

Sympathectomy does not affect the early ectopic discharge from myelinated fibres in ferret inferior alveolar nerve neuromas.

Ectopic neural activity from damaged axons is thought to contribute to the development of sensory disorders following nerve injury. Here we have studied the role of sympathetic fibres in the generation or potentiation of this abnormal activity by determining the effect of predegenerating these fibres. Twelve adult ferrets were used in the study and six of them underwent sympathectomy by removal of the left superior cervical ganglion. Electrophysiological recordings were made from myelinated axons in fine filaments dissected from the inferior alveolar nerve, 3 days after it had been ligated fur

Organization of nitric oxide-producing nerves in the rat pyloric sphincter.

The architecture of nitric oxide (NO)-producing nerves in the rat pylorus was studied in relation to the muscular structure. The musculature of the rat pylorus was observed to be composed of two discrete muscle loops (proximal and distal sphincters). Connective tissue septa containing neural elements divided the thick musculature of the distal sphincter into many bundles. The myenteric nerve plexus of the stomach with a subpopulation of NO-producing nerves was continuous with that of the duodenum. Nitrinergic nerve fibers which originated from the antral myenteric plexus ran through the connec

Gracilis microneurovascular transfer for facial paralysis.

Facial nerve dysfunction occurs in varying degrees of severity due to several causes, and leads to asymmetric or absent facial movements. Regardless of the etiology, facial nerve dysfunction can be functionally and psychologically devastating. Many techniques to restore facial symmetry both at rest and with motion have been pursued throughout history. Within the past 30 years, free muscle microneurovascular transfer techniques have been developed to provide symmetric motion to the face. The aim of this article is to describe one of the most common and reliable techniques to restore midface mob

Multi-functional self-powered nanofiber conduit for peripheral nerve regeneration and muscle atrophy prevention.

Peripheral nerve injury (PNI) often leads to limited functional recovery in over 50% of patients, with current clinical strategies primarily relying on nerve grafts or conduits to bridge injury gaps. In the search of alternative solutions, self-powered biomaterials show significant potential in tissue engineering applications. This study designed a self-powered polycaprolactone/gelatin (PCL/gelatin) nanofiber conduit for sciatic nerve regeneration. The electrospun PCL/gelatin nanofiber conduit was characterized for its self-powering capability, mechanical strength, biocompatibility, and degrad

Chiral MoS2@BC fibrous membranes selectively promote peripheral nerve regeneration.

BACKGROUND: Molybdenum disulfide (MoS2) has excellent physical and chemical properties. Further, chiral MoS2 (CMS) exhibits excellent chiroptical and enantioselective effects, and the enantioselective properties of CMS have been studied for the treatment of neurodegenerative diseases. Intriguingly, left- and right-handed materials have different effects on promoting the differentiation of neural stem cells into neurons. However, the effect of the enantioselectivity of chiral materials on peripheral nerve regeneration remains unclear. METHODS: In this study, CMS@bacterial cellulose (BC) scaffo

Analysis of Facial Reanimation Procedures Performed Concurrently With Total Parotidectomy and Facial Nerve Sacrifice.

IMPORTANCE: Facial reanimation procedures share the same surgical field as a parotidectomy and are most easily accomplished at the time of facial nerve sacrifice. Early reanimation would also reduce the duration of paralysis and may lead to better functional outcomes. OBJECTIVE: To assess the incidence and types of facial nerve reanimation performed concurrently with total parotidectomy and facial nerve sacrifice using the American College of Surgeons National Surgical Quality Improvement Project (ACS-NSQIP) database. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study identified 2

Nanofibrous nerve guidance conduits decorated with decellularized matrix hydrogel facilitate peripheral nerve injury repair.

Rationale: Peripheral nerve injury (PNI) is a great challenge for regenerative medicine. Nerve autograft is the gold standard for clinical PNI repair. Due to its significant drawbacks, artificial nerve guidance conduits (NGCs) have drawn much attention as replacement therapies. We developed a combinatorial NGC consisting of longitudinally aligned electrospun nanofibers and porcine decellularized nerve matrix hydrogel (pDNM gel). The in vivo capacity for facilitating nerve tissue regeneration and functional recovery was evaluated in a rat sciatic nerve defect model. Methods: Poly (L-lactic acid…

Supraligamentous Approach of Lateral Release in Endoscopic Lateral Soft Tissue Procedure.

Many surgical methods are available for hallux valgus, but consensus regarding the best management has yet to be established. Minimally invasive techniques are gaining popularity with the potential advantages of better cosmetic outcome, less postoperative pain, and decreasing recovery time because surgical exposure and deep soft tissue dissection are less extensive and possibly gentler. Recently, techniques of the endoscopic distal soft tissue procedure have been reported, in which an endoscopic approach of the classic distal soft tissue procedure is described. The intermetatarsal ligament, ad
