Internuclear ophthalmoplegia. II. Pursuit, optokinetic nystagmus, and vestibulo-ocular reflex.

Smooth pursuit, optokinetic nystagmus (OKN) and the vestibulo-ocular reflex (VOR) were studied in four patients with internuclear ophthalmoplegia (two with bilateral and two with unilateral lesions). Horizontal smooth pursuit by an adducting eye on the side of a medial longitudinal fasciculus (MLF) lesion was normal in three of four patients; vertical pursuit was abnormal in all four. The horizontal VOR gain of slow components made by an adducting eye on the side of an MLF lesion was normal in all four patients. The vertical VOR gain was decreased in the two patients tested. It is concluded th

A new familial disease of saccadic oscillations and limb tremor provides clues to mechanisms of common tremor disorders.

Tremor disorders pose fundamental questions about disease mechanisms, and challenges to successful neurotherapeutics: What causes motor circuits to oscillate in disorders in which the central nervous system otherwise seems normal? How does inheritance 'determine' the clinical phenotype in familial tremor disorders? Here, we address these questions. Analogies between the neural circuits controlling rapid eye movements (saccades) and those controlling limb movements allow us to translate the interpretations from the saccadic systems to the limb movement system. Moreover, the relatively well unde

Visualization of radial nerve activity at the upper arm using magnetoneurography.

OBJECTIVE: To evaluate the function of the radial nerve in the upper arm using Magnetoneurography (MNG). METHODS: Eight asymptomatic male volunteers (age 25-63 years) and one 67-year-old female patient with radial nerve palsy were included. The radial nerve was electrically stimulated in the right wrist, and the evoked magnetic field was recorded using a 132-channel bio-magnetometer system with a superconducting quantum interference device positioned below the upper arm. The measurements were divided into distal and proximal sessions because of the extensive range of the nerve. RESULTS: MNG

Electrical stimulation over the human vertebral column: which neural elements are excited?

Supramaximal percutaneous electrical stimuli applied over the human cervical vertebral column produce maximal compound muscle action potentials (CMAPs) in abductor digiti minimi. It is important to know which neural elements are excited by these stimuli and experiments were performed to answer this question. With stimulating electrodes placed progressively lateral to the midline, submaximal CMAPs with the same latency are produced. With shocks over the cervical vertebrae in the midline, the threshold for excitation of arm muscles is much lower than for excitation of leg muscles. Comparison of

Combined transcranial direct current stimulation and mirror therapy for upper limb rehabilitation in patients with stroke: a systematic review and meta-analysis.

Upper limb impairment is among the common consequences of stroke. Combining transcranial direct current stimulation (tDCS) and mirror therapy (MT) have recently been used to improve neural plasticity and functional recovery. This review aims to evaluate the effects of combined tDCS and MT on upper limb function in patients with stroke. "PubMed, Scopus, Web of Science, CINAHL, PEDro, and EMBASE" were searched from their inception to March 2026. Randomized controlled trials investigating the combination of tDCS and MT in patients with stroke and assessing upper limb function were included. The q

Connections between expiratory bulbospinal neurons and expiratory motoneurons in thoracic and upper lumbar segments of the spinal cord.

Cross-correlation of neural discharges was used to investigate the connections between expiratory bulbospinal neurons (EBSNs) in the caudal medulla and expiratory motoneurons innervating thoracic and abdominal muscles in anesthetized cats. Peaks were seen in the cross-correlation histograms for around half of the EBSN-nerve pairs for the following: at T8, the nerve branches innervating internal intercostal muscle and external abdominal oblique muscle and a more distal branch of the internal intercostal nerve; and at L1, a nerve branch innervating internal abdominal oblique muscle and a more di

Electrophysiological features of POEMS syndrome and chronic inflammatory demyelinating polyneuropathy.

Polyneuropathy is often an initial manifestation of polyneuropathy, organomegaly, endocrinopathy, M protein and skin changes (POEMS) syndrome and therefore this disorder is frequently misdiagnosed as chronic inflammatory demyelinating polyneuropathy (CIDP). We reviewed electrophysiological data in 20 patients with POEMS syndrome and 36 matched patients with CIDP to compare the electrophysiological features of POEMS syndrome and CIDP. Compared with CIDP controls, POEMS patients demonstrated (1) less prolonged distal motor latency and less reduced motor nerve and sensory nerve conduction velocit

The rat caudal nerves: a model for experimental neuropathies.

This study provides a detailed investigation of the anatomy of the rat caudal nerve along its entire length, as well as correlated nerve conduction measures in both large and small diameter axons. It determines that rodent caudal nerves provide a simple, sensitive experimental model for evaluation of the pathophysiology of degeneration, recovery, and prevention of length-dependent distal axonopathy. After first defining the normal anatomy and electrophysiology of the rat caudal nerves, acrylamide monomer, a reliable axonal toxin, was administered at different doses for escalating time periods.

Effects of imeglimin on experimental diabetic neuropathy in streptozotocin-induced diabetic rats.

AIMS/INTRODUCTION: As a common chronic complication, diabetic neuropathy affects a substantial number of individuals with diabetes mellitus, with limited therapeutic options addressing the underlying pathogenesis. The glucose-lowering action of imeglimin is mediated through improved insulin sensitivity in peripheral organs, including the skeletal muscle and liver, along with augmented insulin release from β-cells. Imeglimin exerts its effects in part by modulating mitochondrial complex I, leading to the reduced production of reactive oxygen species and protection against metabolic stress. Desp

Characterizing Combined Central and Peripheral Demyelination-Insights From a Multimodal Comparison With Chronic Inflammatory Demyelinating Polyneuropathy and Multiple Sclerosis.

INTRODUCTION/AIMS: Combined central and peripheral demyelination (CCPD) is a rare dysimmune disorder sharing features with multiple sclerosis (MS) and chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). Direct comparisons of central and peripheral diagnostic findings across these entities remain limited. We, therefore, performed a systematic study assessing nervous system involvement in CCPD, using magnetic resonance imaging (MRI), nerve ultrasound (US), and nerve conduction study (NCS) and compared findings to MS and CIDP controls. METHODS: We conducted a descriptive case study

[Effect of ritmilen on myocardial refractility and cardiac impulse conduction in patients with paroxysmal tachycardia].

By choosing antiarrhythmic therapy, ritmilen, 3 mg/kg, was given to 30 patients with atrial, atrioventricular, reciprocal and ventricular tachycardias. The agent was found to have the highest effect on atrial myocardial refractoriness in patients with atrial tachycardias and on ventricular refractoriness in patients with ventricular tachycardias (longer effective and functional refractory periods). Intraventricular pathways are the major site of ritmilen's effect on anterograde conduction. Profound changes were found in patients with atrial and ventricular tachycardias. The cholinolytic action

Effect of Balance Enhanced Exercise Program (BEEP) on Balance, Balance Confidence, and Nerve Conduction Velocity in Patients with Diabetic Peripheral Neuropathy.

BACKGROUND: Type 2 diabetes mellitus (DM) accounts for the majority (>85%) of all DM cases; these patients have a higher risk of complications and a greater chance of acquiring chronic microvascular diseases, including diabetic neuropathy. It negatively affects the nerve function, leading to altered balance and posture, which demands a protocol to help improve both. OBJECTIVES: To study the effect of BEEP on balance, balance confidence, Nerve conduction velocity (NCV) and quality of life using limits of stability (LOS), the modified Clinical Test on Sensory Interaction of Balance (m-CTSIB), A

Dysregulation of the Schwann cell GABAergic system and Aβ-fibers conduction abnormalities in the sciatic nerve of the mdx mouse model of Duchenne muscular dystrophy.

Duchenne muscular dystrophy (DMD), a lethal X-linked disorder caused by loss of the full-length dystrophin Dp427, is characterized by progressive skeletal muscle degeneration, although increasing evidence indicates a broader neuromuscular dysfunction. Here, we identify impaired Schwann cell (SC) homeostasis in the sciatic nerve of dystrophic mdx mice. Integrated molecular, immunofluorescence, ultrastructural, and electrophysiological analyses demonstrate that sciatic nerves from 6 to 7-week-old mdx mice exhibit downregulation of key myelin proteins (MBP, P0, PMP22), compared with age-matched w

The Renin-Angiotensin system mediates the effects of stretch on conduction velocity, connexin43 expression, and redistribution in intact ventricle.

UNLABELLED: Effect of Stretch on Conduction and Cx43. INTRODUCTION: In disease states such as heart failure, myocardial infarction, and hypertrophy, changes in the expression and location of Connexin43 (Cx43) occur (Cx43 remodeling), and may predispose to arrhythmias. Stretch may be an important stimulus to Cx43 remodeling; however, it has only been investigated in neonatal cell cultures, which have different physiological properties than adult myocytes. We hypothesized that localized stretch in vivo causes Cx43 remodeling, with associated changes in conduction, mediated by the renin-angioten
