The association of malnutrition with surgical morbidity and mortality is well recognized. The question of whether this relationship is causal or simply an association in sick patients has been hotly debated. The field of nutrition support has grown out of the belief that correcting malnutrition will modify associated risks for poor outcome. It has been easier to substantiate this belief in some clinical situations than in others. The evidence for nutrition support during the perioperative period is reviewed and recommendations are made about where nutrition support is most useful and where it

Respiratory failure is a life-threatening condition that occurs when the lungs are unable to deliver adequate oxygen to the blood or remove carbon dioxide from the body. Patients with respiratory failure often have difficulty maintaining oral nutrition intake because of associated breathlessness and are at increased risk of malnutrition, given both their metabolic response to stress and their dependence on nutrition support. Predictive equations assist clinicians in determining specific energy (caloric) needs for patients with respiratory failure; however, they are inaccurate, complex, and dif

Early oral feeding is the preferred mode of nutrition for surgical patients. Avoidance of any nutritional therapy bears the risk of underfeeding during the postoperative course after major surgery. Considering that malnutrition and underfeeding are risk factors for postoperative complications, nutritional therapy is mandatory for any surgical patient at nutritional risk, especially for those undergoing upper gastrointestinal surgery. The focus of this guideline is to cover nutritional aspects of the Enhanced Recovery After Surgery (ERAS) concept and the special nutritional needs of patients un

Although the current doctrine of IBD pathogenesis proposes an interaction between environmental factors and gut microbiota in genetically susceptible individuals, dietary exposures have attracted recent interest and are, at least in part, likely to explain the rapid rise in disease incidence and prevalence. The D-ECCO working group along with other ECCO experts with expertise in nutrition, microbiology, physiology, and medicine reviewed the evidence investigating the role of diet and nutritional therapy in the onset, perpetuation, and management of IBD. A narrative topical review is presented

Nutritional treatment in critical care patients is an important component of the complete treatment plan. Assessment of nutritional status and careful consideration of the disease course will help guide the selection of the most appropriate assisted-feeding method. Enteral nutrition is favored over parenteral nutrition when the gastrointestinal tract is functional. Common routes of enteral feeding include nasoesophageal, esophagostomy, gastrostomy, and jejunostomy feeding tubes. Parenteral nutrition is indicated for patients who cannot tolerate enteral feedings, or are poor anesthetic candidat

BACKGROUND & AIMS: Critically ill surgical patients are particularly vulnerable to inadequate nutritional intake due to surgery, trauma, treatment-related interruptions, and hypermetabolic stress. Clinical practice guidelines (CPGs) are important for guiding nutritional management in this population, but specific recommendations remain limited. Therefore, this systematic review aimed to evaluate the methodological quality of CPGs related to nutritional management in critically ill surgical patients and to synthesize their recommendations.
METHODS: Comprehensive bibliographic searches were con

BACKGROUND & AIMS: Refeeding syndrome (RFS) is characterized by electrolyte and metabolic disturbances that may occur following the initiation of nutritional support. Given the potential for these disturbances to contribute to clinically important complications, there is substantial interest in ensuring timely recognition of RFS and identifying patients at increased risk. This systematic review and meta-analysis aimed to systematically evaluate the incidence and risk factors of RFS in critically ill patients to support risk stratification and optimize nutritional management.

PURPOSE OF REVIEW: Our aims were to review literature describing the role and benefits of medical nutrition therapy (MNT) in cancer care, highlight barriers to the integration of MNT into standard clinical practice, and investigate strategies to improve access to MNT in the United States (US).
RECENT FINDINGS: Patients with cancer benefit from receiving MNT, including individualized nutrition counseling, oral nutrition supplements, enteral nutrition, and parenteral nutrition. Recent evidence highlighted the efficacy of MNT in reducing treatment-related toxicity, risk of sarcopenia, gastrointe

Sarcopenia, defined as age-related decline in skeletal muscle mass and functional capacity, is a hallmark nutritional abnormality observed in patients with moderate-to-advanced CKD. Uremic state and associated medical conditions also predispose older patients with CKD to protein-energy wasting, a nutritional abnormality that could include sarcopenia. Prevention of protein and energy depletion and replenishing the already low nutritional reserves elderly patients with CKD should focus on conventional and innovative strategies. This review aims to provide an overview of the mainstay of nutrition

Attending diligently to the nutrition and hydration needs of patients with short bowel syndrome (SBS) is a key tenet of their care, both postoperatively and in the years that follow. For, without each, patients are left to themselves to navigate the nutrition consequences of SBS, including malnutrition, nutrient deficiencies, renal compromise, osteoporosis, fatigue, depression, and impaired quality of life. The intent of this review is to discuss the initial nutrition assessment, oral diet, hydration, and home nutrition support for the patient with SBS.

Infants born premature or other medical complex infants are at high risk for developing long-term feeding problems that extends beyond infancy. Intensive multidisciplinary feeding intervention (IMFI) represents the standard of care for children with chronic and severe feeding issues, with a profession team that should involve, at a minimum, psychology, medicine, nutrition, and feeding skill expertise. IMFI seems to hold benefit for preterm and medically complex infants; however, there remains a need to develop and investigate new therapeutic pathways to reduce the number of patients who likely

Malnutrition is common in older people with fragility fractures and is associated with poor clinical outcomes and increased risk of complications. The UK National Health Service has published national standards for food and drink for patients, staff and visitors, in hospitals. These standards describe the methods to ensure quality and sustainability. We assessed these standards and report the nutritional status of older (70 years of age) patients admitted to hospital with fragility fracture, and weighed food trolley and plate waste after lunch and supper for five days. There were 19 older patien

Short bowel syndrome (SBS) is a rare disorder characterized by severe intestinal dysfunction leading to malabsorption of macronutrients and micronutrients that often results in permanent need of parenteral nutrition support. Patients can develop SBS because of massive intestinal resection or loss of intestinal function and consequently experience significant morbidity and increased healthcare utilization. The remaining anatomy and length of bowel after intestinal resection have important prognostic and therapeutic implications. Because patients with SBS constitute a heterogenous group, managem

Malnutrition, both protein energy malnutrition (PEM) and deficiencies in individual nutrients, is a frequent complication of alcoholic liver disease (ALD). Severity of malnutrition correlates with severity of ALD. Malnutrition also occurs in patients with cirrhosis due to etiologies other than alcohol. The mechanisms for malnutrition are multifactorial, and malnutrition frequently worsens in the hospital due to fasting for procedures and metabolic complications of liver disease, such as hepatic encephalopathy. Aggressive nutritional support is indicated in inpatients with ALD, and patients oft…

Inflammatory Bowel Diseases - ulcerative colitis and Crohn's disease- are chronic gastrointestinal inflammatory diseases of unknown etiology. Decreased oral intake, malabsorption, accelerated nutrient losses, increased requirements, and drug-nutrient interactions cause nutritional and functional deficiencies that require proper correction by nutritional therapy. The goals of the different forms of nutritional therapy are to correct nutritional disturbances and to modulate inflammatory response, thus influencing disease activity. Total parenteral nutrition has been used to correct and to preven

Malnutrition and dehydration are widespread in older people, and obesity is an increasing problem. In clinical practice, it is often unclear which strategies are suitable and effective in counteracting these key health threats. The objective of this guide was to provide evidence-based recommendations for clinical nutrition and hydration in older persons in order to prevent and/or treat malnutrition and dehydration. Further, to address whether weight-reducing interventions are appropriate for overweight or obese older persons. This guideline was developed according to the standard operating pro
