Case Report: Concurrent esophageal and spinal cord compression in cervical spondylosis: integrated anterior osteophytectomy and zero-profile ACDF for dual pathology decompression.

Cervical spondylotic dysphagia (CSD) and cervical spondylotic myelopathy (CSM) represent two distinct clinical entities within degenerative cervical pathology. Their co-occurrence creates diagnostic and therapeutic dilemmas due to overlapping pathophysiological mechanisms. CSD primarily stems from anterior cervical osteophytes mechanically compressing the esophageal lumen, resulting in progressive dysphagia and pharyngeal discomfort. Conversely, CSM develops through spinal cord compression mediated by posterior osteophytic growth, intervertebral disc herniation, or ossification of the posterio…

Costovertebral joint involvement in radiographic axial spondyloarthritis: A case-series computed tomography study.

AIMS: To evaluate structural changes of costovertebral joints (CVJ) in patients with radiographic axial spondyloarthritis (rAxSpA) using computed tomography (CT) studies. METHODS: Available chest or thoracic spine CT studies of 17 patients with rAxSpA and 17 patients with rheumatoid arthritis (RA) were analyzed. Ankylosis, erosions, joint space narrowing, and osteophytes were assessed. RESULTS: The groups were similar by patients' average age, but the rAxSpA group included more males (11/17) compared to the RA group (4/17, p = 0.036). In all, 748 CVJ were assessed in each patient group, incl…

What is a normal pharynx? A videofluoroscopic study of anatomy in older adults.

PURPOSE: Structural and functional pharyngeal changes occur with age. How these affect swallowing in healthy older adults is not well define … e spinal changes. 8% of participants demonstrated cricopharyngeal bars and 12% of subjects revealed osteophytes. Age positively correlated with the presence of a cricopharyngeal bar (rs = 0.281, p < 0.001) and presence of osteophytes (rs = 0.334, p < 0.001). The incidence of cricopharyngeal bars in adults over 70 years old was 16%. CONCLUSIONS: A significan …

Differences in descriptions of Kellgren and Lawrence grades of knee osteoarthritis.

OBJECTIVE: Correct application of the Kellgren and Lawrence (K&L) classification system is difficult due to inexact wording of the descripto … escriptions. In grade 2, often used as a cut-off to classify OA, one description replaced "definite osteophytes and possible narrowing of joint space" (K&L) by "definite osteophyte, unimpaired joint space". Another description for grade 2 was "minimal osteophytes, possible narrowing, cysts, and sclerosis". In some cohort studies, descriptions changed during …

Diffuse idiopathic skeletal hyperostosis: an unusual cause of difficult intubation.

A case is reported in which anterior osteophytes on the cervical vertebra, in combination with a subglottic stenosis, resulted in distortion of the airway and led to unexpected difficulties during intubation. The osteophytes, associated with the syndrome of diffuse idiopathic skeletal hyperostosis (DISH) were centred at the midcervical level and resulted in anterior displacement of the larynx with an acute angulation of the trachea just below the larynx. This acute angulation, immediately above an unrecognized subglottic stenosis, rendered it impossible to pass all but the smallest endotrachea…

In vivo protective effects of prophylactic treatment with tiaprofenic acid or intraarticular corticosteroids on osteoarthritic lesions in the experimental dog model.

We examined the in vivo effects of prophylactic intraarticular corticosteroid injections and nonsteroidal antiinflammatory drugs (NSAID) (tiaprofenic acid) oral administration on cartilage lesions of osteoarthritis (OA) and osteophyte formation in 18 dogs in which the anterior cruciate ligament had been sectioned. Six of the dogs received no treatment; 5 had 3 injections of 5 mg triamcinolone hexacetonide at 0, 4 and 8 weeks postsurgery; and 7 were treated orally with 5 mg/kg/day tiaprofenic acid. The untreated dogs developed cartilage lesions on the tibial plateaus and femoral condyles with p…

The recognition of craniomandibular disorders--a comparison between clinical and radiographic findings in eighty-nine subjects.

A comparison has been made between clinical and radiographical findings in a study of eighty-nine patients suffering from craniomandibular disorders. In general it was found that condylar changes were more common in these patients than in earlier studies concerning the severe problems of craniomandibular disorders. A statistically significant correlation between clinical and radiographical findings was found for crepitation with osteophyte formation of the condyle (P less than 0.01). After clinical separation of the patients into two groups based upon the origin of pain, myogenous versus arthr…

Diffuse idiopathic skeletal hyperostosis causing dysphagia in a young patient.

Diffuse idiopathic skeletal hyperostosis (DISH) is an under-diagnosed condition producing flowing ossification of the antero-lateral ligaments of the spine. Affecting predominantly males over 65 years old, it is an unusual cause of dysphagia and dysphonia. We report a 45-year-old mole with a three years history of dysphonia and three months of dysphagia. The initial diagnosis was gastroesophageal reflux, and an endoscopy ruled out esophageal luminal pathology. Cervical spine radiographs showed ossification of the cervical anterior longitudinal ligament with large, prominent osteophytes from C3…

Diffuse idiopathic skeletal hyperostosis inducted stridor and dysphagia.

Diffuse Idiopathic Skeletal Hyperostosis (DISH) is a condition characterized by calcification and ossification of soft tissues, mainly ligaments and enthesis. Dysphagia is the commonest complaint, stridor secondary to osteophyte compression has rarely been documented. The osteophytes may cause symptoms by mechanical compression or by inducting inflammatory reaction. When an upper segment of the C-spine is involved, particular C3 - C4 level, the larynx may be affected. This could be result of hoarseness, stridor, laryngeal stenosis and obstruction. Sometimes vocal fold paralysis may result from…

Growth factors in experimental osteoarthritis: transforming growth factor beta pathogenic?

Osteoarthritis (OA) is characterized by focal cartilage destruction and osteophyte formation. Triple injections of transforming growth factor beta (TGF beta) in normal murine knee joints revealed that this growth factor can elicit osteophytes at sites characteristic for OA. Moreover, enhanced cartilage proteoglycan synthesis is induced and cartilage proteoglycan content is increased. Exuberant repair reactions may be pathogenic and indeed focal cartilage proteoglycan loss and disorganized chondrocyte spacing is noted in the tibial plateau after 1 month. Although TGF beta is immunosuppressive a…

Upper zone of growth plate and cartilage matrix associated protein protects cartilage during inflammatory arthritis.

BACKGROUND: ADAMTS aggrecanases play a major role in cartilage degeneration during degenerative and inflammatory arthritis. The cartilage-specific secreted protein Upper zone of growth plate and cartilage matrix associated protein (Ucma) has been shown to block ADAMTS-triggered aggrecanolysis in experimental osteoarthritis. Here we aimed to investigate whether and how Ucma may affect cartilage destruction and osteophyte formation in the context of inflammatory arthritis. METHODS: Ucma-ADAMTS5 protein interactions were studied using slot blot and solid phase binding assays. Chondrocyte culture…
