Finite element analysis of a novel patellar fracture fixation method.

BACKGROUND: Patellar fractures present challenges in treatment, with traditional methods often leading to complications such as loss of reduction and implant failure. This study aimed to compare a novel suture fixation technique with the traditional tension band method using finite element analysis. METHODS: CT images of a healthy 35-year-old male were used to construct 3D patellar models. Two fixation methods were modeled: the novel suture technique using polyester sutures and traditional tension band fixation with Kirschner wires and cerclage wires. Finite element analysis was conducted to

The composite vastus medialis-patellar complex osseomuscular flap as a salvage procedure after complex trauma of the knee--an anatomical study and clinical application.

BACKGROUND: In the setting of severe perigenicular trauma or complicated endoprosthetic knee surgery, primary knee fusion may be the last resort for salvage of the limp. In this case, the patella looses its destination as an anterior knee stabilizer and can become a substantial donor of bone substance, especially if osseous defects are involved. PATIENTS AND METHODS: 12 formalin fixated cadavers were studied in terms of vascular anatomy, pedicle reliability, arc of rotation and their relation to sex, age, and height. Moreover, the operation was performed on a suitable patient. RESULTS: The q

Patellar dislocation and osteochondral fractures.

Follow-up study of nine patients with patellar dislocation in combination with osteochondral fractures. Three of these patients with special problems in relation to knee pathology are discussed in detail. This group of knee injuries is often overlooked because of frequent spontaneous reposition. Etiology, diagnosis and treatment are discussed. Arthroscopy is recommended to visualize chondral defects. Results are presented in combination with follow-up. Four patients still complain of instability of the knee after surgery. Reconstruction is the treatment of choice. The kind of fixation depends

Identifying predictors of surgical site infection in closed patellar fracture surgery: a multicenter study.

Surgical site infection (SSI) is a prevalent complication in orthopedic surgery, leading to significant morbidity and potentially severe outcomes. This study aimed to identify predictors of SSI in patients undergoing closed patellar fracture surgery. We retrospectively analyzed 2664 patients treated for closed patellar fractures at two hospitals from November 2013 to January 2023. Patients were divided into SSI and non-SSI groups. We employed univariate analysis, logistic regression, and receiver operating characteristic (ROC) curve analyses to evaluate demographic data, comorbidities, and pre

The Comparison of Postoperative Outcomes Open and Closed Reduction for Patellar Fractures.

Patellar fractures account for approximately 1% of all human body fractures. This study aimed to compare the surgical outcomes of open reduction and closed reduction for patellar fractures. This retrospective study included 62 patients (63 cases) who underwent surgical treatment of patellar fractures from 2008 to 2013. Of the 63 cases, open and closed reductions were performed in 42 and 21 cases, respectively. Plain radiography was used to assess fracture healing. Comminuted patellar fracture was the most common fracture type in both groups. Tension band wiring and cannulated screw fixation we

Minimally invasive Unicondylar Knee Arthroplasty in a patellectomized patient.

Patellectomized patients may have less satisfactory clinical outcomes following total knee arthroplasty due to a decreased extensor mechanism efficiency and potential instability. The literature only reports 3 case series of patients who had a previous patellectomy undergoing a Unicondylar Knee Arthroplasty (UKA) with mixed results. A Minimally Invasive fixed-bearing UKA was performed in a patellectomized patient with excellent postoperative knee stability and clinical outcomes after 5 years. MIS UKA may be a viable option for patellectomized patients with isolated medial compartment osteoarth

Negative pressure wound therapy for the treatment of infected wounds with exposed knee joint after patellar fracture.

Treatment of soft tissue defects with exposed bones and joints, resulting from trauma, infection, and surgical complications, represents a major challenge. The introduction of negative pressure wound therapy has changed many wound management practices. Negative pressure wound therapy has recently been used in the orthopedic field for management of traumatic or open wounds with exposed bone, nerve, tendon, and orthopedic implants. This article describes a case of a patient with a large soft tissue defect and exposed knee joint, in which negative pressure wound therapy markedly improved wound he

Quadriceps Tendon Rupture in an Emergency Department Patient with Left Knee Pain.

Point-of-care ultrasound (POCUS) is a dynamic imaging modality that can diagnose musculoskeletal and tendon injuries to facilitate appropriate orthopedic care and improve patient outcomes. This manuscript illustrates the value of dynamic POCUS to distinguish between complete and partial tendon injuries at the patient's bedside, which is important for management and disposition planning in emergency department (ED) or clinic patients with acute trauma. In this case, a healthy 34-year-old male presented with acute left knee pain and swelling after a fall. He had a deformity superior to his patel

Global burden, trends and forecast analysis of extremity fractures based on GBD 2021.

BACKGROUND: A quite common kind of clinical trauma, extremity fractures (EFs) have a significant impact on patients and society. Thus, the global burden of disease (GBD) trend for EFs was analyzed for the first time in this study using the GBD 2021 database, which serves as a crucial foundation for the creation of future prevention and treatment strategies and aids in the sensible distribution of health care resources. METHODS: We retrieved incidence, prevalence, and years lived with disabilities (YLDs) for seven EFs by area, age, and sex from the GBD 2021 database for 1990-2021. Based on the
