Prioritizing Maternal Mental Health in Addressing Morbidity and Mortality.

IMPORTANCE: The rate of maternal mortality in the United States is 2-fold to 3-fold greater than that in other high-income countries. While many national initiatives have been developed to combat maternal mortality, these efforts often fail to include mental illness. OBJECTIVE: To highlight the underrecognized contribution of mental illness to maternal mortality, which is nearly double that of postpartum hemorrhage. EVIDENCE REVIEW: A topic outline was developed to include challenges in measuring perinatal mental conditions and mortality rates; contributions of social determinants of health

Sacrococcygeal teratomas: prenatal surveillance, growth and pregnancy outcome.

OBJECTIVE: Prenatal surveillance and growth characteristics are evaluated in a cohort of fetuses with sacrococcygeal teratomas (SCT) as part of risk assessment. METHODS: Retrospective review of 23 fetuses with SCT: prenatal diagnosis, surveillance, delivery, and early postnatal outcome are reported. RESULTS: Cardiac output failure physiology requires serial evaluation. The size of the SCT determines obstetrical risks and mode of delivery. An SCT growth rate approaching >150 cm(3) per week may be associated with increased perinatal mortality risks. Maternal morbidity is related mainly to poly…

Placental abruption in term pregnancies: clinical significance and obstetric risk factors.

OBJECTIVE: To determine the incidence, obstetric risk factors and pregnancy outcome of placental abruption at term. METHODS: A comparison o … rauterine growth restriction (IUGR), non-vertex presentation, hydramnios and advanced maternal age. Perinatal mortality was significantly higher in pregnancies complicated with placental abruption (OR = 30.0, 95% CI 19.7-45.6; p < 0.001). In order to assess whether the increased risk for perinatal mortality was due to the placental abruption or to its significant ass …

Key outcomes in childbirth: Development of a perinatal core outcome set for management of labor and delivery at or near term.

INTRODUCTION: Consistency and relevance of perinatal outcome measures are necessary basics for obstetric research, audit, and clinical counseling. Still, there is an unwarranted variation in reported perinatal outcomes, which impairs research synthesis, validity, and implementation, as well as clinical benchmarking and longitudinal comparisons. The aim of this study was to develop a short-term perinatal (fetal and neonatal) Core Outcome Set to be used in research and quality assurance of management of labor and delivery at or near term. MATERIAL AND METHODS: The methods were guided by the Cor

Trend of pregnancy outcomes in type 1 diabetes compared to control women: a register-based analysis in 1996-2018.

INTRODUCTION: In 1989, the St Vincent declaration aimed to approximate pregnancy outcomes of diabetes to that of healthy pregnancies. We aim … d control pregnancies using hierarchical Poisson regression. RESULTS: The frequency of stillbirth, perinatal mortality, large for gestational age, caesarean section, admission to neonatal intensive care unit (NICU), and low Appearance, Pulse, Grimace, Activity, and Respiration (APGAR) score was 2-4 times higher in type 1 diabetes compared to controls, while the risk …

Causes of stillbirth and death among children younger than 5 years in eastern Hararghe, Ethiopia: a population-based post-mortem study.

BACKGROUND: Child mortality is high in Ethiopia, but reliable data on the causes of death are scarce. We aimed to gather data for the contributory causes of stillbirth and child deaths in eastern Ethiopia. METHODS: In this population-based post-mortem study, we established a death-notification system in health facilities and in the community in Kersa (rural), Haramaya (rural) and Harar (urban) in eastern Ethiopia, at a new site of the Child Health and Mortality Prevention Surveillance (CHAMPS) network. We collected ante-mortem data, did verbal autopsies, and collected post-mortem samples via

The role of labor induction in modern obstetrics.

A primary goal of obstetrical practice is the optimization of maternal and perinatal health. This goal translates into a seemingly simple as … luding cesarean delivery, hypertensive disorders of pregnancy, neonatal respiratory impairment, and perinatal mortality, are less likely when induction is performed. This information underscores the importance of making the preferences of pregnant individuals for different birth processes and outcomes central to the approach to delivery timing.

A global reference for fetal-weight and birthweight percentiles.

BACKGROUND: Definition of small for gestational age in various populations worldwide remains a challenge. References based on birthweight are deficient for preterm births, those derived from ultrasound estimates might not be applicable to all populations, and the individualised reference can be too complex to use in developing countries. Our aim was to create a generic reference for fetal weight and birthweight that overcame these deficiencies and could be readily adapted to local populations. METHODS: We used the fetal-weight reference developed by Hadlock and colleagues and the notion of pr

Fetal growth restriction: a modern approach.

PURPOSE OF REVIEW: Fetal growth restriction is a complicated perinatal condition, with multiple causes. It shares common pathophysiologies with other important disorders, such as preeclampsia and abruption. As a group, these conditions associated with ischemic placental disease are responsible for a large percentage of indicated preterm births. The ability to accurately predict, diagnose and manage these pregnancies has significant and far-reaching implications, including potential effects on long-term adult health. RECENT FINDINGS: Placental ischemia is the most common cause of fetal growth

Terminal pattern: characteristics and management.

Terminal prepartum patterns from 31 patients were associated with 39% perinatal mortality rate and major degrees of perinatal asphyxia among survivors. Conditions most commonly associated with these patterns were intrauterine growth retardation, preeclampsia, and prolonged pregnancy. NST patterns in which there was absence of accelerations, absent or reduced variability, with or without variable or shallow late decelerations, portended very omnious outcomes when accompanied by absent fetal movements and/or oligohydramnios. Perinatal mortality was 39% under these circumstances. Immediate cesare…

Perinatal outcome of babies delivered to eclamptic mothers: a prospective study from a nigerian tertiary hospital.

BACKGROUND: Eclampsia is a leading cause of maternal and perinatal mortality in Nigeria. Preventive and interventional measures have been shown to reduce maternal mortality and morbidity with no significant beneficial effect on neonatal outcomes. The aim of this study is to assess the perinatal outcome of eclampsia at the University of Port-Harcourt Teaching Hospital (UPTH). MATERIALS AND METHODS: This prospective, cross-sectional study was conducted on 88 consecutive patients presenting with antepartum eclampsia at the UPTH between 1(st) January 2007 and 31(st) December 2008. A protocol was …

Incidence-based measures of birth, growth restriction, and death can free perinatal epidemiology from erroneous concepts of risk.

BACKGROUND: Traditional perinatal epidemiology appears to embrace fallacious concepts of risk. The use of incorrect denominators for perinatal rates is commonplace both for straightforward indices such as the gestational age-specific labor induction rate and also for the more conceptually challenging indices such as the gestational age-specific neonatal mortality rate. As a consequence, perinatology is beset by several conondrums including the paradox of intersecting perinatal mortality curves. PROPOSITION: These traditions are ideally replaced by alternative concepts that may be derived a pri…
