BACKGROUND: Increasing healthcare spending is a significant issue, with the aging population contributing to a rise in patients needing renal replacement therapy. The cost of peritoneal dialysis (PD) is substantial, particularly in upper-middle-income countries like Serbia. We aimed to identify the direct costs and influencing factors of treating peritoneal dialysis-associated peritonitis (PD associate peritonitis) in Serbia.
METHODS: A retrospective observational study was conducted on consecutive patients admitted due to PD-associated peritonitis in five tertiary care hospitals across Serbi…

Peritonitis is an important complication of peritoneal dialysis (PD), affecting up to 40% of patients at some point of their PD treatment. Here, we describe a case of PD-associated peritonitis due to an unusual pathogen, Brevibacterium casei. The patient was treated with intraperitoneal antibiotics, successfully preserving the PD catheter. To the best of our knowledge, only four cases of peritonitis due to B. casei have been previously documented worldwide, with catheter preservation achieved in just one other case.

In settings where intraperitoneal vancomycin is unavailable or off-label (e.g., Japan), intravenous vancomycin can be a viable treatment option for peritoneal dialysis-associated peritonitis. Due to limited pharmacokinetic data regarding intravenous dosing, pragmatic strategies involving more frequent therapeutic drug monitoring-measuring both serum levels for systemic safety and dialysate levels for intraperitoneal antimicrobial exposure-can help ensure therapeutic efficacy while maintaining patient safety.

Patients undergoing long-term peritoneal dialysis (PD) frequently develop peritoneal fibrosis and angiogenesis, leading to membrane dysfunction. Transglutaminase 2 (TG2) stabilizes the extracellular matrix against proteases. In an animal model, inhibition of TG2 reduced peritoneal fibrosis, angiogenesis, and inflammation. We investigated the expression of TG2 in 163 human peritoneal membrane tissue samples, including controls, tissues exposed to conventional acidic or low-glucose degradation product (GDP) pH-neutral solutions, and those with peritonitis or encapsulating peritoneal sclerosis (E…

A 63-year-old woman undergoing peritoneal dialysis (PD) presented to our hospital with abdominal pain, diarrhea, and cloudy PD effluent. An elevated white blood cell count in the PD effluent led to a diagnosis of PD-associated peritonitis. She was subsequently started on intraperitoneal cefazolin and ceftazidime, after which her condition improved rapidly. The peritonitis resolved after 21 days of therapy, with no subsequent relapse. Centrifuged PD effluent samples identified Kocuria rhizophila as the causative organism. Reports of PD-associated peritonitis caused by K. rhizophila are rare, wi…

Methylobacterium populi is a fastidious, pink-pigmented, Gram-negative bacterium that has been isolated from poplar trees that are found throughout the Northern Hemisphere in both temperate and subtropical regions. Herein, we report a novel case of M. populi peritonitis associated with continuous ambulatory peritoneal dialysis (CAPD). A 45-year-old Japanese man with a history of hypertension and chronic kidney disease was diagnosed with CAPD-associated peritonitis. After starting broad-spectrum antibiotics, white blood cell (WBC) count in the CAPD effluent remained elevated. On day 7, the CAPD…

A 65-year-old woman with a history of peritoneal dialysis undergoing hemodialysis at our hospital presented with a fever after experiencing gastroenteritis symptoms. She had an implanted peritoneal dialysis catheter for draining chylous ascites. After commencing empirical treatment with meropenem, peritoneal effluent samples revealed an increased white blood cell count, and peritonitis was diagnosed. Enterococcus gallinarum was detected in blood and effluent cultures. Meropenem was changed to vancomycin based on susceptibility testing but subsequently restarted and thereafter changed to ampici…

It is challenging to differentiate between tuberculous peritonitis and peritoneal carcinomatosis due to their insidious nature and intersecting symptoms. Computed tomography (CT) is the modality of choice in evaluating diffuse peritoneal disease. We conducted an ambispective analysis of patients suspected as having tuberculous peritonitis or peritoneal tuberculosis between Jan 2020 to Dec 2021. The study aimed to identify the clinical and radiological features differentiating the two entities. We included 44 cases of tuberculous peritonitis and 45 cases of peritoneal carcinomatosis, with a med…

Peritoneal dialysis (PD)-related peritonitis is a frequent complication. PD units should be aware of all possible pathogens and share their experience about prevention and optimal management. Uncommon bacteria, a special group with crescent incidence in PD practice, may require singular considerations. A case of peritonitis due to Ewingella americana, a rare human pathogen, is reported, with a favorable outcome. To date, only three other cases have been described in the literature. New evidence is necessary for a better understanding of this pathogen and its consequences in PD modality.

BACKGROUND: Encapsulating peritoneal sclerosis (EPS) is a catastrophic complication of peritoneal dialysis (PD) with high mortality. Our aim is to develop a novel noninvasive microRNA (miRNA) test for EPS.
METHODS: We collected 142 PD effluents (EPS: 62 and non-EPS:80). MiRNA profiles of PD effluents were examined by a high-throughput real-time polymerase chain reaction (PCR) array to first screen. Candidate miRNAs were verified by single real-time PCR. The model for EPS prediction was evaluated by multiple logistic regression and machine learning.
RESULTS: Seven candidate miRNAs were identi

Peritonitis is an important cause of morbidity and technique failure in peritoneal dialysis. Herein, we report peritonitis related to Acinetobacter lwoffi in two patients on peritoneal dialysis. The first case is a 63-year-old patient treated by automated peritoneal dialysis admitted with abdominal pain. The peritoneal effluent White Blood Cells count consisted of 280 cells/mm3. Then culture identified a multisensitive Acinetobacter lwoffi. Treatment with ceftazidime and ciprofloxacin had been started. The control dialysate culture was sterile after three weeks. The second patient is a 59-year…

BACKGROUND: Peritonitis is an important complication and cause of morbidity in patients undergoing peritoneal dialysis (PD). Corynebacterium species, often considered skin and mucosal contaminants, are a rare cause of PD-associated peritonitis and have been acknowledged in published guidelines for the diagnosis and treatment of PD peritonitis only over the last decade.
CASE-DIAGNOSIS/TREATMENT: We present two children with difficult-to-treat episodes of PD peritonitis due to Corynebacterium amycolatum. Episodes were associated with fever, abdominal pain and cloudy dialysate, high dialysate po…
