Valvular Regurgitation Using Portable Echocardiography in a Healthy Student Population: Implications for Rheumatic Heart Disease Screening.

BACKGROUND: There is increasing use of portable echocardiography as a screening test for rheumatic heart disease (RHD). The prevalence of valvular regurgitation in healthy populations as determined using portable echocardiography has not been well defined. Minimal echocardiographic criteria for RHD have recently been clarified, but the overlap of normal and abnormal valvular regurgitation warrants further study. The aim of this study was to determine the spectrum of echocardiographic findings using portable echocardiography in children from a population with low prevalence of RHD. METHODS: Sc

Does raising heart rate prior to a behavioural test enhance learning in cognitive therapy for anxiety? An experimental test for the treatment of fear of heights using virtual reality.

BACKGROUND: A key clinical issue is how to maximise the belief change central to cognitive therapy. Physiological arousal is a key internal cue confirming threat beliefs in anxiety disorders. Deeper extinction of anxiety may occur if catastrophizing responses to physiological arousal are inhibited prior to joint exposure with external phobic stimuli. The aim of the study was to test whether increasing physiological arousal using exercise increases the benefits of behavioural tests. METHODS: Sixty individuals with a fear of heights had one session of VR cognitive treatment. They were randomise

Intensified emotion perception in depression: Differences in physiological arousal and subjective perceptions.

People suffering from depression perceive themselves and their surroundings as more negative than healthy ones. An explanation might be that depressed individuals experience negative information as more stressful than non-depressed subjects and, consequently, respond in an amplified manner on a subjective and physiological level. To test this proposition, we presented 41 patients with recurrent depressive episodes and 42 controls with stimuli from the International Affective Picture System split into three valence categories while different parameters of physiological arousal (e.g., heart rate

The reduction potential of nitric oxide (NO) and its importance to NO biochemistry.

A potential of about -0.8 (+/-0.2) V (at 1 M versus normal hydrogen electrode) for the reduction of nitric oxide (NO) to its one-electron reduced species, nitroxyl anion (3NO-) has been determined by a combination of quantum mechanical calculations, cyclic voltammetry measurements, and chemical reduction experiments. This value is in accord with some, but not the most commonly accepted, previous electrochemical measurements involving NO. Reduction of NO to 1NO- is highly unfavorable, with a predicted reduction potential of about -1.7 (+/-0.2) V at 1 M versus normal hydrogen electrode. These re

Peripheral arterial disease and intermittent claudication: efficacy of short-term upper body strength training, dynamic exercise training, and advice to exercise at home.

OBJECTIVE: To compare the effect of two training programmes and advice to exercise at home on physiological adaptations in patients with peripheral arterial disease (PAD). DESIGN: 30 patients with a typical history of PAD and intermittent claudication were randomised to either an upper body strength training programme (UBST), a dynamic (walking, cycling, circuit) conventional exercise rehabilitation programme (CER), or advice to 'walk as much as possible at home' (CONT). Before and after intervention groups performed a standard graded treadmill exercise test (GTET) and a 6-minute walk test (S

Refinement of Replacement Fluid Using Human Serum Albumin Preparation to Enhance Biocompatibility in Therapeutic Plasma Exchange.

Human serum albumin (HSA) solution is widely used as a replacement fluid during therapeutic plasma exchange (TPE). However, the ionic concentrations in commercially available 5% HSA solutions differ significantly from physiological levels. In this study, we developed a simple method for preparing a replacement fluid with ion concentrations closer to physiological norms by mixing 25% HSA with lactated Ringer's solution, sodium chloride, and calcium gluconate hydrate. We prepared three types of replacement fluids, each with a 4.0% albumin concentration and total volumes of 1.83, 2.43, and 3.04 L

A Case for Hydrogen Sulfide Metabolism as an Oxygen Sensing Mechanism.

The ability to detect oxygen availability is a ubiquitous attribute of aerobic organisms. However, the mechanism(s) that transduce oxygen concentration or availability into appropriate physiological responses is less clear and often controversial. This review will make the case for oxygen-dependent metabolism of hydrogen sulfide (H2S) and polysulfides, collectively referred to as reactive sulfur species (RSS) as a physiologically relevant O2 sensing mechanism. This hypothesis is based on observations that H2S and RSS metabolism is inversely correlated with O2 tension, exogenous H2S elicits phy

High-performance liquid chromatographic-radioimmunoassay method for the measurement of angiotensin II peptides in human plasma.

A highly selective high-performance liquid chromatographic-radioimmunoassay method for the measurement of individual endogenous angiotensin peptides in human plasma is described. This method allows the complete resolution of the immunoreactive angiotensin II peptides. We have also measured the angiotensin peptide levels and compared them in both pooled and individual human plasma. The effects of inhibition of angiotensin-converting enzyme on the angiotensin peptide levels have also been observed in a patient with renovascular hypertension with the plasma angiotensin II level being reduced grea

Limitations of portable roentgenography of the chest in patients with acute respiratory failure.

The ability of the portable roentgenography of the chest to define the amount of physiologic shunting and the severity of noncardiogenic pulmonary edema was evaluated in 37 observations of 11 patients. Ten of the 11 patients had acute respiratory failure. The roentgenologic assessment of the amount of pulmonary edema and the severity of left ventricular failure was compared with the physiologic shunt fraction, tracer measured lung water and the pulmonary arterial wedge pressure. The roentgenologic scores for edema did not predict the shunt fraction or tracer measured lung water. The roentgenol

Hierarchical Cluster Analysis Identifies Distinct Physiological States After Acute Brain Injury.

BACKGROUND: Analysis of intracranial multimodality monitoring data is challenging, and quantitative methods may help identify unique physiological signatures that inform therapeutic strategies and outcome prediction. The aim of this study was to test the hypothesis that data-driven approaches can identify distinct physiological states from intracranial multimodality monitoring data. METHODS: This was a single-center retrospective observational study of patients with either severe traumatic brain injury or high-grade subarachnoid hemorrhage who underwent invasive multimodality neuromonitoring.

Evolution of ventricular hypertrophy and myocardial mechanics in physiological and pathological hypertrophy.

Left ventricular hypertrophy (LVH) is an adaptive response to physiological or pathological stimuli, and distinguishing between the two has obvious clinical implications. However, asymmetric septal hypertrophy and preserved cardiac function are noted in early stages in both cases. We characterized the early anatomic and functional changes in a mouse model of physiological and pathological stress using serial echocardiography-based morphometry and tissue velocity imaging. Weight-matched CF-1 male mice were separated into Controls ( n = 10), treadmill Exercise 1 h daily for 5 days/wk ( n = 7), a

Physiologic recovery of Mycobacterium tuberculosis from drug injury: A molecular study of post antibiotic effect in vitro and in vivo.

Post-antibiotic effect (PAE) describes the delay in bacterial growth that continues after antibiotics are cleared. The physiologic basis of PAE in Mycobacterium tuberculosis (Mtb) remains poorly understood. Here, we evaluated the long-standing hypothesis that PAE reflects the time required for bacteria to recover from drug-induced physiologic damage by comparing Mtb after varying durations of treatment with the four-drug isoniazid, rifampin, pyrazinamide, ethambutol combination in vitro and in BALB/c mice using two novel molecular readouts of bacterial health. In aerobic axenic culture and in

Association of idealized amphiphiles and protease inhibitors: Conferring antimicrobial peptides with stable antibacterial activity under physiological conditions to combat multidrug-resistant bacteria.

AIMS: The unstable antimicrobial activity of antimicrobial peptides (AMPs) under physiological conditions (especially the degradation instigated proteases) seems to be a persistent impediment for their successful implementation in clinical trials. Consequently, our objective was to devise AMP engineering frameworks that could sustain robust antibacterial efficacy within physiological environments. METHODS: In this work, we harvested AMPs with stable antimicrobial activity under the physiological barriers through the combination of idealized amphiphiles and trypsin inhibitors. RESULTS: We scr

Physiologic expansion of human heart-derived cells enhances therapeutic repair of injured myocardium.

BACKGROUND: Serum-free xenogen-free defined media and continuous controlled physiological cell culture conditions have been developed for stem cell therapeutics, but the effect of these conditions on the relative potency of the cell product is unknown. As such, we conducted a head-to-head comparison of cell culture conditions on human heart explant-derived cells using established in vitro measures of cell potency and in vivo functional repair. METHODS: Heart explant-derived cells cultured from human atrial or ventricular biopsies within a serum-free xenogen-free media and a continuous physiol

The involvement of sympathetic nervous system in essence of chicken-facilitated physiological adaption and circadian resetting.

AIM: The impact of the sympathetic nervous system (SNS) on the regulation of circadian rhythm and physiological functions is still not clear. Previous studies have found that essence of chicken (EC) supplementation facilitated the physiological adaption and circadian resetting in rats subjected to jet lag. Herein, the effects of SNS on the circadian clock and the hypothesis that EC-induced acceleration of circadian resetting is dependent on the SNS are investigated. MAIN METHODS: Male Wistar rats with superior cervical ganglionectomy (SCGx) were used to investigate the role of the SNS in circ

The influence of elevated hormone levels on physiologic accumulation of 68Ga-DOTATOC.

OBJECTIVE: PET/CT imaging with 68Ga-1,4,7,10-tetraazacyclododecane-N,N',N″,N‴-tetraacetic acid-D-Phe1-Tyr3-octreotide (DOTATOC) is useful in patients with neuroendocrine tumors (NETs). Functioning NETs by definition secrete abnormal levels of hormones, causing clinical symptoms. It is known that physiologic accumulation can be seen in some organs, but it remains unknown whether elevated hormone levels can affect the physiologic accumulation pattern of 68Ga-DOTATOC. We aimed to investigate the influence of higher hormone levels on physiologic accumulation of 68Ga-DOTATOC. METHODS: A total of 1
