The viscous behaviour of HES 130/0.4 (Voluven®) and HES 260/0.45 (Pentaspan®).

PURPOSE: Several fluids are available for volume therapy to address hypovolemia. We focus on two hydroxyethyl starches (HES) available for volume expansion in Canada, HES 130/0.4 (Voluven®) and HES 260/0.45 (Pentaspan®). Although information is available regarding their pharmacokinetic and risk/benefit profiles, this paper examines their viscous properties. METHODS: Dynamic viscosities of HES 130/0.4 and HES 260/0.45 were measured through capillary viscometry at 21°C and 37°C. The viscosities of the solutions were then measured through a closed flow loop at room temperature across physiologic

Perioperative hemodynamic monitoring with transesophageal Doppler technology.

Invasive cardiac output (CO) monitoring, traditionally performed with transpulmonary thermodilution techniques, is usually reserved for high-risk patients because of the inherent risks of these methods. In contrast, transesophageal Doppler (TED) technology offers a safe, quick, and less invasive method for routine measurements of CO. After esophageal insertion and focusing of the probe, the Doppler beam interrogates the descending aortic blood flow. On the basis of the measured frequency shift between the emitted and received ultrasound frequency, blood flow velocity is determined. From this v

Pressor therapy in acute ischemic stroke: systematic review.

BACKGROUND AND PURPOSE: Systolic blood pressure (SBP) levels below 140 mm Hg after acute stroke occur in 18% to 25% of patients, and may be associated with adverse outcome, in terms of death and disability. It has thus been proposed that BP elevation in acute ischemic stroke may be beneficial by increasing perfusion to the peri-infarct penumbra, though not only in those with low BP levels. METHODS: All articles studying BP elevation in the context of acute stroke were identified using a structured search strategy. RESULTS: Two reviewers independently searched the databases, and 12 relevant p

Principles of intravenous fluid replacement.

Infusates are used to expand the intravascular volume or to influence solute composition of body fluids. The choice of infusate depends on the underlying physiological disturbances. A variety of infusates are used and it is important that the consequences of over-infusion are understood.

Acceleration of fibrin gel formation by unrelated proteins.

Addition of gamma-globulin, serum albumin, hemoglobin, or ovalbumin in concentrations of 1-10 g/dl to solutions of purified fibrinogen results in a substantial (up to six-fold) decrease in the lag time preceding appearance of a firm fibrin gel following addition of thrombin at 24 degrees C. The effect does not appear to be due to a protein-induced enhancement in the enzymatic activity of thrombin, nor does it appear to be due to the co-condensation of the added protein with fibrin/fibrinogen. It is suggested that the observed effect is primarily due to nonspecific volume exclusion arising from
