Unilateral pulmonary artery agenesis is a rare congenital anomaly often associated with other cardiovascular abnormalities. It is usually diagnosed and surgically treated in childhood. A 71-year-old man underwent a completion pneumonectomy for a recurrent carcinosarcoma of the left upper lobe. A single lumen endotracheal tube was used for intubation. During dissection the right main bronchus became blocked by a tumor mass. Due to severe adhesions it was impossible to open the airway from inside the operation field. This neither was also not possible by bronchoscopic maneuvers and the patient died. The case is similar to six cases described in the literature since 1966 [4-7, 9, 10]. Recommendations for preventing and treating the complications are given.

A 2-day-old baby with severe respiratory distress and cyanosis was admitted to the hospital with a totally opacified right pleural cavity. Preoperative studies demonstrated absence of the right pulmonary artery, and a patent ductus arteriosus, and a left pulmonary artery pressure of 85/45 mm. Hg. The hypoplastic right lung received its entire blood supply from multiple vessels through the diaphragm. The infant's condition improved initially after a right pneumonectomy, but she died of progressive respiratory insufficiency at the age of 7 months.

We herein report a case of an 18-year-old male with left postpneumonectomy syndrome who underwent a bullectomy for right pneumothorax. The patient underwent a left pneumonectomy at the age of 1 year. At the age of 18 years, he developed right pneumothorax, and radiological findings revealed apical bullae in the right pleural cavity extending into the left atrophic thoracic cavity beyond the upper mediastinum. The right thoracoscopic bullectomy was successful. Modifications of selective lobar ventilation during surgery and thoracoscope position were described.

We report successful surgery for malignant pleural mesothelioma (MPM) in a 14-year-old boy with right aortic arch. Pleural biopsy by video-assisted thoracic surgery yielded a diagnosis of MPM, epithelial type. As the disease was not changed after combination chemotherapy with three cycles of cisplatin and gemcitabine, we performed left extrapleural pneumonectomy, including resection of the pericardium and diaphragm for MPM, and aortopexy for right aortic arch. The postoperative course was uneventful, and the patient has remained alive without disease for 10 months postoperatively.

Splanchnicectomy has been known for years as a treatment for refractory pain in patients with pancreatic cancer or chronic pancreatitis. We report herein the performance of a videothoracoscopic left splanchnicectomy in a patient with a previous right pneumonectomy who suffered intractable pain from an irresectable left adrenal metastasis associated with metastatic retroperitoneal lymph nodes. Immediate pain relief was obtained, but abdominal pain of middle intensity recurred 6 weeks later. Although infrequently required, this procedure might be of value in some patients with refractory pain.

Granular cell myoblastoma of of the right main bronchus in a 5-year-old girl is reported. The tumor extended to the lower trachea from the right main bronchus which was occluded completely by the tumor. The diagnosis was established histologically by a obtaining bronchoscopic biopsy specimen. Right pneumonectomy was carried out to control pulmonary suppuration caused by the tumor obstruction of the bronchus. External radiotherapy and interstitial brachytherapy were successful to control the growth of the residual tumor. She has been free from symptoms for about 12 years after the treatment.
