A One Health systematic review of diagnostic tools for Echinococcus multilocularis surveillance: Towards equity in global detection.

Echinococcus multilocularis is a zoonotic cestode of canid definitive hosts that is emerging as a parasite of medical and veterinary concern in regions of North America, Europe and Asia. Infection with the metacestode stage (alveolar echinococcosis - AE) is life-threatening, especially for patients who reside in low resource countries and lack access to modern diagnostic tests and treatments. The overall objectives of this One Health review were to systematically describe the diagnostic tests currently employed in endemic countries to detect E. multilocularis in people, canids and the environm

The structural and health policy environment for delivering integrated HIV and substance use disorder treatments in Puerto Rico.

BACKGROUND: HIV prevalence in Puerto Rico is nearly twice that of the mainland United States, a level that was substantially fueled by injection drug use. Puerto Rico has a longstanding history of health provision by the public sector that directly affects how HIV and substance use disorder (SUD) treatment services are provided and funded. As part of pre-implementation research for a randomized trial of a community-level intervention to enhance HIV care access for substance users in San Juan, Puerto Rico, we sought to understand the structural and health policy environment for providing HIV an

Brucellosis as an emerging threat in developing economies: lessons from Nigeria.

Nigeria is the most populous country in Africa, has a large proportion of the world's poor livestock keepers, and is a hotspot for neglected zoonoses. A review of the 127 accessible publications on brucellosis in Nigeria reveals only scant and fragmented evidence on its spatial and temporal distribution in different epidemiological contexts. The few bacteriological studies conducted demonstrate the existence of Brucella abortus in cattle and sheep, but evidence for B. melitensis in small ruminants is dated and unclear. The bulk of the evidence consists of seroprevalence studies, but test stand

Chronic diseases and injuries in India.

Chronic diseases (eg, cardiovascular diseases, mental health disorders, diabetes, and cancer) and injuries are the leading causes of death and disability in India, and we project pronounced increases in their contribution to the burden of disease during the next 25 years. Most chronic diseases are equally prevalent in poor and rural populations and often occur together. Although a wide range of cost-effective primary and secondary prevention strategies are available, their coverage is generally low, especially in poor and rural populations. Much of the care for chronic diseases and injuries is

Where health care has no access: the nomadic populations of sub-Saharan Africa.

Nomadic and seminomadic pastoralists make optimal use of scarce water and pasture in the arid regions south of the Sahara desert, spreading from Mauretania in the west to Somalia in East Africa. We attempted to summarize the fragmentary evidence from the literature on the health status of these populations and to assess the best ways to provide them with modern health care. Infant mortality is higher among nomadic than among neighbouring settled populations, but childhood malnutrition is less frequent. Nomads often avoid exposure to infectious agents by moving away from epidemics such as measl

A reassessment of the expected development consequences of tsetse control efforts in Africa.

Since the World Food Conference (1974) recommended a long term program of tsetse control, increasing attention of governments and international agencies has focused on the problem. This paper briefly outlines the strategies potentially available for tsetse control operations before assessing the likely consequences of any large scale tsetse control endeavor. The implications for both natural and human systems are shown to be profound and, in many cases, counter-productive to development goals. Suggestions for more appropriate strategies of addressing the tsetse issue are included. The major co

Can hepatitis C be eradicated in the United States?

The advent of highly effective antiviral regimens will make the eradication of hepatitis C in high-income countries such as the United States technically feasible. But eradicating hepatitis C will require escalating our response to the epidemic in key domains, including surveillance and epidemiology, prevention, screening, care and treatment, policy, research, and advocacy. Surveillance must be nimble enough to quickly assess the magnitude of new transmission patterns as they emerge. Basic prevention strategies - community-based outreach and education, testing and counseling, and access to ste

The impact of migration on the sexual health, behaviours and attitudes of Central and East European gay/bisexual men in London.

BACKGROUND: Building on an earlier quantitative study which found that gay/bisexual men from Central and Eastern Europe were at greater risk of sexual ill health following migration to the UK, the aim of this qualitative study is to explore how the process of migration itself may have influenced the migrants' sexual behaviour and attitudes. METHODS: To address these questions, we conducted 17 in-depth interviews in London with gay/bisexual male migrants from Central and Eastern Europe, drawing on Fisher and Fisher's Information-Motivation-Behavioral Skills model as an interpretive framework.

Public opinions and attitudes toward a state monopoly: a study of the finnish gambling system.

BACKGROUND: Gambling regulated through a state monopoly is often justified for reasons of public health, that is, that monopolies are a more effective means of reducing potential harm. This focus on harm prevention has increased in recent years, particularly as a result of pressures arising from the growth of online gambling and of legislation designed to promote competition. While prior works have examined the role of stakeholders in influencing policy decisions and in public discussions of the monopoly systems, attention has been focused on those with direct financial interests; the opinions

Whose Knowledge? Examining the Relationship between the Traditional Medicine Sector and Environmental Conservation Using a Stakeholder Analysis: Perceptions on Warwick Herb Market Durban South Africa.

The South African traditional medicine sector is estimated to accommodate millions of citizens, despite it being informal. The existence of such a healthcare system embodies the dual system of both primary and traditional healthcare, with some preferring one and others utilising both systems. The gathering, harvesting, and selling of medicinal plant and animal species have inevitable environmental effects. The paradox between biodiversity conservation and livelihood sustenance is eminent in South Africa's contemporary environmental legislation. The purpose of the study was to highlight and exa

Counting indirect crisis-related deaths in the context of a low-resilience health system: the case of maternal and neonatal health during the Ebola epidemic in Sierra Leone.

Although the number of direct Ebola-related deaths from the 2013 to 2016 West African Ebola outbreak has been quantified, the number of indirect deaths, resulting from decreased utilization of routine health services, remains unknown. Such information is a key ingredient of health system resilience, essential for adequate allocation of resources to both 'crisis response activities' and 'core functions'. Taking stock of indirect deaths may also help the concept of health system resilience achieve political traction over the traditional approach of disease-specific surveillance. This study respo

Socio-economic and food system drivers of nutrition and health transitions in The Gambia from 1990 to 2017.

In common with many nations undergoing a nutrition transition, micronutrient deficiencies, undernutrition and overnutrition coexist in The Gambia. Addressing these challenges simultaneously would require transformational changes in the country's food system. However, the evidence base that would enable informed decision-making in the Gambian food system has been scant, despite several sources of routinely-collected data being available. This descriptive study brings together data from four open-access global databases on food supply, political, economic, and demographic variables, and nutritio

Nutrition Transition, Processed Foods, and Cardiometabolic Risk in Somalia: A Public Health Commentary.

BACKGROUND: Somalia is undergoing a nutrition transition in which traditional camel milk, sorghum, and legume-centred diets are being progressively displaced by imported processed foods high in refined sugars, saturated fats, and dietary cholesterol. This shift is occurring in a population already burdened by undernutrition and a fragile health system. PURPOSE: This narrative commentary synthesises published evidence on the biochemical mechanisms linking excess sugar and cholesterol intake to dyslipidaemia, type 2 diabetes mellitus (T2DM), and cardiovascular disease, and considers how these m

HINTs: Sensemaking on Large Collections of Documents With Hypergraph Visualization and INTelligent Agents.

Sensemaking on a large collection of documents (corpus) is a challenging task often found in fields such as market research, legal studies, intelligence analysis, political science, or computational linguistics. Previous works approach this problem from topic- and entity-based perspectives, but the capability of the underlying NLP model limits their effectiveness. Recent advances in prompting with LLMs present opportunities to enhance such approaches with higher accuracy and customizability. However, poorly designed prompts and visualizations could mislead users into falsely interpreting the v

Efforts Towards Polio Eradication in Madagascar: 1997 to 2017.

BACKGROUND: In 1988, the World Health Assembly launched the Global Polio Eradication Initiative. WHO AFRO is close to achieve this goal with the last wild poliovirus detected in 2014 in Borno States in Nigeria. The certification of the WHO African Region requires that all the 47 member states meet the critical indicators for a polio free status. Madagascar started implementing polio eradication activities in 1996 and was declared polio free in June 2018 in Abuja. This study describes the progress achieved towards polio eradication activities in Madagascar from 1977-2017 and highlights the rema

From discovery to preparedness: reframing antimicrobial resistance as a global governance challenge.

Almost a century after the discovery of penicillin, the antimicrobial resistance (AMR) crisis has become as much a failure of global governance as a biomedical challenge. Published Global Burden of Disease estimates for 2000-2021 show a persistent geographic hierarchy of mortality, a pattern consistent with AMR burden being embedded in health system capacity rather than due to pathogen evolution alone. Furthermore, the for-profit pharmaceutical model remains structurally misaligned with the clinical necessity of restricted antibiotic use. This Perspective argues that the next phase of AMR miti

The Infected Homo Sacer: The (Auto)Immunity and Biopolitics of Haitian Zombies in Junot Díaz's "Monstro".

This article reconsiders the figure of the zombie in Junot Díaz's "Monstro" by shifting attention to its role as a biopolitical mechanism that determines who is protected, abandoned, and left to die. While existing scholarship has tended to read the zombie as an allegory of colonial trauma and racial violence, this study argues that zombification operates as a material technology of governance through which populations are classified, regulated, and exposed to varying degrees of precarity within neoliberal regimes. Drawing on Agamben's concept of homo sacer and Mbembe's necropolitics, the arti

[Physical activity promotion among older adults in community settings: structures, implementation barriers and the role of political narratives].

Physical activity is one of the most effective measures for preventing non-communicable diseases and for maintaining independence and quality of life in older age. Current data indicate that the majority of adults in Germany aged 65 and older do not meet recommended levels of physical activity. Against this background, the article discusses why effective interventions should not focus solely on individual behaviour but also address the settings in which older adults live.Within Germany's federal system, a variety of structures for promoting physical activity exist at both federal and state lev

Selection into Advanced Training in Rheumatology in Australia: towards a programmatic and theory-informed model.

Selection into vocational medical specialty programmes such as Advanced Training in Rheumatology in Australia is fraught with complexity. Selection panels must identify suitable trainees while navigating eligibility standards set by the Royal Australasian College of Physicians (RACP), complying with state-based employment legislation and responding to broader social and political expectations. While functional, this federated system exhibits variation in selection strategy, methodology, governance, transparency and panel composition. Evidence for selection into vocational medical training is l
