Sedation and analgesia in children with cerebral palsy: a narrative review.

BACKGROUND: Patients with cognitive impairment due to cerebral palsy experience pain more often than healthy peers and frequently require diagnostic and therapeutic painful procedures. Analgesia and procedural sedation outside the operating room are often required, but they may not adequately be provided because of the inability to accurately recognize and classify the state of pain and for the perceived higher risk of complications. DATA SOURCES: We reviewed the available literature to highlight the specific risk factors and area of criticism, that should be further improved. We searched the

Paediatric pain management practice and policies across Alberta emergency departments.

BACKGROUND: Many children requiring acute care receive suboptimal analgesia. OBJECTIVES: To describe paediatric pain management practices a … Few Alberta EDs use policies and protocols to manage paediatric pain. Noninvasive methods to limit procedural pain are underutilized. Canadian paediatricians must advocate for improved analgesia to narrow this knowledge-to-practice gap.

Risk factors of pain in mammographic screening.

Mammography is an important tool in the secondary prevention of breast cancer. However, earlier research has pointed out that an unpleasant experience during a previous mammography can deter women from returning for mammography screening. It is known that mammography can be a painful examination for some women. The research presented in this article focuses on the experience of pain during and after mammography (247 patients). Firstly, the study sets out to determine the extent to which women actually experience pain as a result of mammographic examination. Secondly, it focuses on identifying

"Procedural sedation and analgesia in Italian pediatric emergency departments: a subgroup analysis in italian hospitals".

To date, pain and anxiety are the most common symptoms reported by children who refer to pediatric emergency department. Despite it is well known that the undertreatment of this condition has some negative consequences in a short term and long term of time, gaps in the management of pain in this setting still persist. This subgroup analysis aims to describe the current state of art of pediatric sedation and analgesia in Italian emergency departments and to identify existing gaps to solve. This is a subgroup analysis of a cross-sectional European survey of pediatric emergency departments sedati

Behavioral Strategies to Minimize Procedural Distress During In-Office Pediatric Tympanostomy Tube Placement Without Sedation or Restraint.

The purpose of this study was to evaluate behavioral strategies to minimize procedural distress associated with in-office tympanostomy tube placement for children without general anesthesia, sedation, or papoose-board restraints. 120 6-month- to 4-year-olds and 102 5- to 12-year-olds were treated at 16 otolaryngology practices. Mean age of children was 4.7 years old (SD = 3.18 years), with more boys (58.1%) than girls (41.9%). The cohort included 14% Hispanic or Latinx, 84.2% White, 12.6% Black, 1.8% Asian and 4.1% 'Other' race and ethnicity classifications. The in-office tube placement proced

Impact of carbon dioxide insufflation and water exchange on postcolonoscopy outcomes in patients receiving on-demand sedation: a randomized controlled trial.

BACKGROUND AND AIMS: Water exchange (WE) is the least painful insertion method during colonoscopy. Its impact on postcolonoscopy discomfort has not been well-described. Carbon dioxide (CO2) insufflation consistently reduced postcolonoscopy discomfort. We compared postcolonoscopy outcomes of various combinations of insertion and withdrawal techniques (insertion-withdrawal modality): WE-CO2, WE-air insufflation (WE-AI), and CO2-CO2. METHODS: A total of 240 patients undergoing on-demand sedation diagnostic colonoscopy were randomized to WE-CO2 (n = 79), WE-AI (n = 80), CO2-CO2 (n = 81), with pos

Pain in Intensive Care: A Narrative Review.

All critically ill adult patients in intensive care units (ICU) typically experience pain. Critically ill adults suffer pain of different intensities. It depends on individual characteristics, specific procedural interventions, and underlying diseases. Inadequate management of acute pain is a risk factor for the emergence of chronic pain, where the incidence is up to 33% into the ICU survivor population. For the management of pain, agitation, and delirium, several coexisting clinical practice guidelines have been published. The first problem is that the patient recovered in intensive care unit

Pre-emptive and preventive opioids for postoperative pain in adults undergoing all types of surgery.

BACKGROUND: Postoperative pain is a common consequence of surgery and can have deleterious effects. It has been suggested that the administration of opioid analgesia before a painful stimulus may improve pain control. This can be done in two ways. We defined 'preventive opioids' as opioids administered before incision and continued postoperatively, and 'pre-emptive opioids' as opioids given before incision but not continued postoperatively. Both pre-emptive and preventive analgesia involve the initiation of an analgesic agent prior to surgical incision with the aim of reducing intraoperative n

Sacroplasty for Zone III Sacral Insufficiency Fracture: A Case Report Utilizing a Midline Approach and Articulating Osteotome.

BACKGROUND: Sacral insufficiency fractures can be a debilitating cause of low back pain in the elderly population. While sacroplasty is recommended for the treatment of persistent pain from sacral insufficiency fractures, the anatomical location of Zone III fractures can put patients at a higher risk of neurological complications during repair. CASE REPORT: We present a case of a 71-year-old woman with a Denis Zone III sacral insufficiency fracture who underwent percutaneous sacroplasty. Shoulder anterior capsular (SHAC) block for analgesia in anterior shoulder dislocation: An emergency department case series.

BACKGROUND: Anterior shoulder dislocation is among the most common major joint dislocations presenting to the emergency department (ED) and is frequently managed with procedural sedation. Several analgesic and regional anesthesia strategies are available for shoulder reduction, each with distinct advantages and limitations. The Shoulder Anterior Capsular (SHAC) block is a superficial, ultrasound-guided interfascial plane technique that targets pericapsular nerve contributions to the glenohumeral joint via an anterior approach, offering a potentially motor-sparing and sedation-sparing alternati

Aspiration of sterile post-operative spinal fluid collections using low-dose computed tomography guidance.

Sterile postoperative seromas can develop after posterior spinal surgery and cause pain, weakness, and numbness. Management typically involves operative evacuation. We propose that these collections can be managed with percutaneous computed tomography (CT) guided aspiration, potentially saving the patient an additional surgery. Here, we evaluate the safety and efficacy of this approach. Patients who developed symptomatic postoperative seromas within 60 days following surgery for spinal canal stenosis and had stable neurologic exams were considered for CT-guided percutaneous aspiration. To be c

Understanding maternal perspectives of skin-to-skin contact for the management of acute pain in very and extremely preterm infants.

Skin-to-skin care (SSC) and skin-to-skin contact for procedural pain (SSCP) are recognized for their physiological and emotional benefits in the neonatal intensive care unit (NICU), including pain reduction in preterm infants. However, little is known about how birthing parents of very and extremely preterm infants (<32 weeks gestational age), a significantly more challenging preterm infant population to enact SSCP, perceive this intervention. This study aimed to explore birthing parents' experiences and perceptions related to the use of SSC and SSCP in the NICU with their very and extremely p…

Wet Tap-Induced Spinal Cord Stimulator Trial Failure in Failed Back Surgery Syndrome: A Case Report Highlighting Intrathecal Drug Delivery for Treatment-Resistant Pain.

A 59-year-old female with failed back surgery syndrome (FBSS) following multiple lumbar surgeries underwent a spinal cord stimulator (SCS) trial for chronic pain management. However, the trial failed due to severe nausea, vomiting, and postural headaches secondary to unintentional dural puncture (wet tap) during lead placement. This resulted in cerebrospinal fluid (CSF) leakage, low-pressure headaches, and subsequent trial intolerance, necessitating early lead removal. Given her failed response to conservative treatments, neuropathic medications, and systemic opioids, an intrathecal pump (ITP)
