Medical malpractice after treatment of Peyronie's disease.

BACKGROUND: Peyronie's Disease is a fibrotic tunica albuginea disorder resulting in penile deformity. AIM: This analysis provides an important investigation of medical malpractice cases related to the treatment of Peyronie's Disease. METHODS: The LexisNexis+ and Westlaw databases were used to access all federal and state cases as well as jury verdicts and settlements. The databases were queried for the term "Peyronie Disease" and "Peyronie's Disease" between the years 1980 and 2024. One author manually reviewed all cases to find cases involving a patient suing the urologist or medical system

Extent of disability following traumatic extremity amputation.

Extremity amputation is a devastating injury. Forty-two patients who sustained traumatic limb amputation were contacted 3-57 months (mean, 25 months; median, 24 months) after injury to evaluate residual disability and to determine what factors were associated with a good recovery. There were 35 men and 7 women with ages ranging from 5 to 73 years (mean, 34 years). Amputation was the result of a motor vehicle crash in 18, work-related injury in 11, motorcycle crash in 9, and other causes in 4. There were 46 amputations done: 8 above-knee (AK), 25 below-knee (BK), 5 above-elbow (AE), and 9 below

Osteomyelitis of the Hip due to Granulicatella adiacens: A Case Report of an Extremely Rare Cause of Bone Infection.

BACKGROUND: Granulicatella adiacens (G. adiacens) is a nutritionally variant Streptococcus generally observed in the human mouth microbiota. Although G. adiacens is involved in bacterial endocarditis and bacteremia, bone and joint infections are extremely rare. In the present study, we reported the first known case of hip osteomyelitis related to G. adiacens. Moreover, a commentary on the available evidence was also performed. CASE PRESENTATION: A 59-year-old male with a former substance use disorder who initially complained of low back pain with a medical history negative for any other comor

Partial replacement of tricuspid valve using cryopreserved homograft.

BACKGROUND: The optimal choice of prosthesis for tricuspid valve (TV) replacement is yet to be determined. Partial replacement of the TV using a homograft atrioventricular valve might offer resistance to infection, good durability, and excellent functionality, in addition to avoiding prosthesis-related morbidity. METHODS: We present 14 patients who underwent replacement of the TV using a homograft between 1997 and 2008. The mean age at operation was 32 years, including 5 patients younger than the age of 10. All patients preoperatively showed severe TV regurgitation as a result of active infec

Complications of reverse shoulder arthroplasty: a concise review.

Reverse shoulder arthroplasty is an ideal treatment for glenohumeral dysfunction due to cuff tear arthropathy. As the number of patients treated with reverse shoulder arthroplasty is increasing, the incidence of complications after this procedure also is increasing. The rate of complications in reverse shoulder arthroplasty was reported to be 15%-24%. Recently, the following complications have been reported in order of frequency: periprosthetic infection, dislocation, periprosthetic fracture, neurologic injury, scapular notching, acromion or scapular spine fracture, and aseptic loosening of pr

Mitral valve re-replacement for impaired bioprosthesis after 19 years in a patient undergoing steroid treatment.

A 59-year-old man underwent closed mitral commissurotomy for mitral stenosis at the age of 19 years, and mitral valve replacement (MVR) with a Hancock standard bioprosthesis and tricuspid annuloplasty at the age of 40 years. Two years after the second operation, the patient developed human T-lymphotropic virus type-1 infection, manifesting as myelopathy/tropical spastic paraparesis, and has required steroid therapy for 17 years. He was well for 18 years after the MVR, but has recently developed severe congestive heart failure, orthopnea and enlarged liver. Doppler echocardiography showed sever

Cementless total replacement for old tuberculosis of the hip.

We report the results of cementless total joint replacement in 18 patients with old tuberculosis of the hip, performed, on average, 34 years after the onset of infection. Mean follow-up was 3.5 years. Only seven of the patients had antituberculous drugs during or after the operation. Using the Mayo hip score, 15 patients had excellent or good results and two had a fair rating. One patient had the prosthesis removed more than one year postoperatively for late haematogenous staphylococcal infection and had a poor rating. All the patients had relief of hip-related pain. Despite the absence of any

[Surgical treatment with the freestyle bioprosthesis and omentopexy for prosthetic valve endocarditis after aortic root replacement].

A 60-year-old woman, who had undergone aortic root replacement with composite graft 5 months previously, suffered from anemia and slight fever. Transthoracic echocardiography showed pseudoaneurysm in the aortic root, and blood culture was positive. She was diagnosed with prosthetic valve endocarditis, and surgical intervention was planned. Intraoperatively necrotic tissue and dehiscence of the suture line in the aortic annulus were found. Re-aortic root replacement with Freestyle bioprosthesis and re-hemiarch graft replacement were performed with the omentopexy around the aortic root and the n

Syme Amputation: A Systematic Review.

BACKGROUND: In the decision-making process toward an amputation of the lower extremity, knowledge about patient-related outcomes after amputation and rehabilitation is important. We have not found a systematic review that provides this knowledge for Syme amputation. The aim of this study was to present an overview of outcomes after a Syme amputation grouped according to the International Classification of Functioning, Disability and Health (ICF) model, focusing on body structures and functions, activities, and participation. METHODS: A systematic literature search was performed. Included stud

Distinct vasculotropic versus osteotropic features of S. agalactiae versus S. aureus implant-associated bone infection in mice.

Osteomyelitis is a devastating complication of orthopaedic surgery and commonly caused by Staphylococcus aureus (S. aureus) and Group B Streptococcus (GBS, S. agalactiae). Clinically, S. aureus osteomyelitis is associated with local inflammation, abscesses, aggressive osteolysis, and septic implant loosening. In contrast, S. agalactiae orthopaedic infections generally involve soft tissue, with acute life-threatening vascular spread. While preclinical models that recapitulate the clinical features of S. aureus bone infection have proven useful for research, no animal models of S. agalactiae ost

Optimizing combination rifampin therapy for staphylococcal osteoarticular infections.

Staphylococcus spp. causes more than half of all osteoarticular infections of native structures or implanted material. The ability of Staphylococcus spp. to persist within infected bone tissue and to produce a bacterial biofilm, most notably in infections of implanted material, can lead to treatment failures and microbiological relapses. Rifampin is a cornerstone of the treatment of staphylococcal osteoarticular infections, particularly those of implanted material. Rifampin is a bactericidal antibiotic that diffuses very well within bone tissue and bacterial biofilms. The mechanism of action i

Transcatheter Aortic Valve Implantation for Pseudoaneurysm of the Mitral-Aortic Intervalvular Fibrosa Causing Severe Aortic Regurgitation.

Pseudoaneurysm of the mitral-aortic intervalvular fibrosa (P-MAIVF) is a rare complication following infective endocarditis or valve surgery. We report the case of a 65-year-old male who developed a P-MAIVF following surgical mitral valve replacement for infective endocarditis. Shortly after the surgery, although the patient remained afebrile and blood cultures were negative, routine transthoracic echocardiography revealed new-onset severe aortic regurgitation. Multimodality imaging confirmed a P-MAIVF extending into the non-coronary sinus and the left ventricular outflow tract, causing valvul

Factors influencing the outcome of deep infection following total knee arthroplasty.

BACKGROUND: Whereas clinical studies have revealed a number of important risk factors for postoperative infection following total knee arthroplasty (TKA), it is unclear which factor influences clinical results. A multicenter study was therefore performed by reviewing 51 patients with deep infection after TKA and seeking for the factors related to clinical outcomes. METHODS: Fifty-one deep infections after TKA were treated from 2000 to 2011 at the multicenter including a university or three arthroplasty centers. Among them, two clinical outcome measures including retention of prosthesis at ini

The New Challenge for Heart Endocarditis: From Conventional Prosthesis to New Devices and Platforms for the Treatment of Structural Heart Disease.

Infective endocarditis is a sinister condition with considerable morbidity and mortality. Its relevance in the current era is compounded by the increased use of implanted devices such as replacement valves or cardiac implantable electronic devices. These infections are caused by multiple different bacteria with different virulence, pathogenicity, and antimicrobial resistance. Unlike in native endocarditis, the presence of foreign tissue permits sustenance by inflammatory and thrombotic processes as the artificial surfaces promote inflammatory responses and hypercoagulability. Prevention of the

Early complications after revision total hip arthroplasty with cemented dual-mobility socket and reinforcement ring.

Encouraged by the success of dual-mobility sockets in achieving implant stability in primary hip replacement, surgeons have started to use the implant in revision hip arthroplasty. However, very little is known yet about the postoperative complication rate of this type of implant when fixation in a reinforcement ring is required. In our department, 37 dual-mobility sockets were cemented in a reinforcement ring for revision hip arthroplasty in 36 patients over a period of two years. The mean follow-up period was 16 months (range, 6-27 months). Indications for revision hip arthroplasty included,

Clinical and radiographic results associated with a modern, cementless modular cup design in total hip arthroplasty.

BACKGROUND: First-generation cementless modular cups reproducibly achieved fixation to bone but were associated with unacceptable rates of pelvic osteolysis and mechanical failure. Consequently, second-generation cups were developed with shells that had a limited number of holes (or no holes) as well as improved locking mechanisms, a polished inner surface, and increased conformity with the liner. The purpose of the present study was to evaluate the clinical and radiographic results associated with the use of a second-generation acetabular component for primary total hip arthroplasty. METHODS
