[Perfusion machine for hypothermic pulsatile preservation of rat kidneys. Use in PUVA treatment of allogenic kidney transplants].

A machine for continuous hypothermic pulsatile perfusion preservation of rat kidneys is described in detail. The use of longwave ultraviolet irradiation during hypothermic pulsatile kidney perfusion using Euro-Collins solution containing 8-MOP did not prolong graft survival compared with untreated controls. Systemic application of 8-MOP to the kidney donor may be necessary for effective action of the combined photochemical treatment as a method of immunoalteration.

Left heart bypass without thoracotomy.

An effective, simple and rapidly installed temporary mechanical assistance to the heart is needed. It should be capable of sustaining adequate circulation for several weeks. Patients with extensive myocardial infarctions, complicated by cardiogenic shock, or in end-stage heart failure, and who are candidates for heart replacement by transplantation or by a mechanical device, would benefit from such circulatory assistance. A mediastinal route was developed in human cadavers and in dogs institute left heart bypass without thoracotomy. Anatomical studies demonstrated the accessibility of the left

In Vitro Tests of Antegrade Distal Limb Perfusion Modalities in Peripheral Veno-Arterial Extracorporeal Membrane Oxygenation.

Distal limb ischemia is a frequent complication of femoro-femoral veno-arterial extracorporeal membrane oxygenation (V-A ECMO) associated with high morbidity and mortality. The optimal distal limb perfusion strategies remain a challenge. Furthermore, during a prolonged weaning phase, a significant distal limb hypoperfusion due to the reduced ECMO-based flow may occur. This study evaluated the hemodynamics of different distal perfusion cannula (DPC) configurations (6Fr, 8Fr, or 10Fr introducer sheaths, 8Fr and 9Fr arterial pediatric cannulas [APC], and a 19Fr bidirectional cannula incorporating

[Flow in natural and artificial organs and vessels (author's transl)].

After introductory remarks about the flow phenomena in the human body (separation, dead waters, stagnation regions) and the model simularity conditions, the flow behaviour in arterial bifurcations at pulsatile flow are considered; they are accompanied by the formation of dead waters and secondary flows and therefore they give the danger of atherosclerosis. Then the flow pattern at arterio-venous anastomosis is interpreted with regard to the danger of thrombogenesis. At the selected example of ball prosthesis the flow pattern and the pressure loss of artificial heart valves (aortic as well as m…

Large-Artery Stiffness in Health and Disease: JACC State-of-the-Art Review.

A healthy aorta exerts a powerful cushioning function, which limits arterial pulsatility and protects the microvasculature from potentially harmful fluctuations in pressure and blood flow. Large-artery (aortic) stiffening, which occurs with aging and various pathologic states, impairs this cushioning function, and has important consequences on cardiovascular health, including isolated systolic hypertension, excessive penetration of pulsatile energy into the microvasculature of target organs that operate at low vascular resistance, and abnormal ventricular-arterial interactions that promote lef

Fetal cardiovascular changes during open and fetoscopic in-utero spina bifida closure.

OBJECTIVE: Fetoscopic closure of spina bifida using heated and humidified carbon dioxide gas (hhCO2) has been associated with lower maternal morbidity compared with open closure. Fetal cardiovascular changes during these surgical interventions are poorly defined. Our objective was to compare fetal bradycardia (defined as fetal heart rate (FHR) < 110 bpm for 10 min) and changes in umbilical artery (UA) Doppler parameters during open vs fetoscopic closure. METHODS: This was a prospective cohort study of 22 open and 46 fetoscopic consecutive in-utero closures conducted between 2019 and 2023. Bot

The effect of the physiological rest position of the mandible on cerebral blood flow and physical balance: an observational study.

AIMS: There has been much published evidence that balance can improve by changing the mandible's position relative to the maxilla as it comes together with the teeth (or oral device) as the endpoint. To help with the complexity of this topic, a definitions table* (in Appendix) has been included at the end of the manuscript for reference as needed. The aim of the current study is to evaluate whether the physiologic rest position of the jaw* (oral device overtop of the teeth as endpoint where the muscles of mastication are optimized) can have an effect on cerebral blood flow and physical balance

Assessment of cardio-ankle vascular index in patients with cardiac syndrome-X: an observational study.

OBJECTIVE: Arterial stiffness is associated with major adverse cardiovascular events. Cardio-ankle vascular index (CAVI), a novel marker of arterial stiffness, which is weakly influenced by systolic blood pressure, is a sensitive marker the atherosclerosis and arteriolosclerosis. The aim of this study is to investigate arterial stiffness by CAVI in patients with cardiac syndrome X (CSX). METHODS: The present study was observational and cross sectional, and involved 49 patients (26 male) with CSX (angina-like chest pain, positive electrocardiographic ischemic changes at treadmill exercise test

Non spiral and spiral (helical) flow patterns in stenoses. In vitro observations using spin and gradient echo magnetic resonance imaging (MRI) and computational fluid dynamic modeling.

AIM: Physiological blood flow patterns are themselves poorly understood despite their impact on arterial disease. Stable spiral (helical) laminar flow (SLF) has been observed in normal subjects. The purpose of the present study is to develop a method of magnetic resonance (MR) flow pattern visualization and to analyze spiral and non-spiral flow patterns with and without luminal narrowing in vitro. The flow conditions were then modeled using computational fluid dynamics (Star-CD). METHODS: Laminar integrity was examined in a flow-rig using spin and gradient echo magnetic resonance imaging (MRI

[Arterial vascular diagnosis by Doppler pulse curve analysis of the first dorsal metatarsal artery].

In patients with peripheral arterial occlusive disease diagnostics by Doppler-sonography at the toe arteries and by Doppler-pressure measurement at the ankle arteries is often difficult and sometimes impossible. Using the first dorsal metatarsal artery for Doppler-waveform analysis yields exact information on haemodynamically relevant circulatory disturbances. Painful ulcers, gangrene, oedemata, mediasclerosis, burns, plaster casts, posttraumatic or postoperative conditions prevent the application of cuffs at the lower leg or foot. This Doppler-waveform analysis evidently represents the degree

The use of Doppler umbilical artery waveforms in placental abruption; a report of two cases.

In a prospective study, the predictive value of Doppler umbilical arterial velocimetry waveforms in detecting the fetus at risk was examined in ten patients with abruptio placenta. One patient suffered intra-uterine fetal death within 8 h of obtaining a normal reactive cardiotocograph and normal umibilical artery waveforms. In another patient with severe placental abruption, following 2500 ml of blood loss, the umbilical artery waveforms were normal. In this study, umbilical artery waveforms failed to predict fetal outcome in cases of placental abruption.

The National Heart, Lung, and Blood Institute Pediatric Circulatory Support Program.

Options for the circulatory support of pediatric patients under the age of 5 years are currently limited to short-term extracorporeal devices, the use of which is often complicated by infection, bleeding, and thromboembolism. Recognizing this void, the National Heart, Lung, and Blood Institute solicited proposals for the development of novel circulatory support systems for infants and children from 2 to 25 kg with congenital or acquired cardiovascular disease. Five contracts were awarded to develop a family of devices that includes (1) an implantable mixed-flow ventricular assist device design
