We report four cases of idiopathic retroperitoneal fibrosis (IRPF) effectively treated with steroid therapy. Computed tomographic (CT) scan showed the density of soft tissue mass enveloping the abdominal aorta in four cases. From radiographic findings we made a diagnosis of IRPF. Management with steroid therapy over three months improved general symptoms and radiographic findings. Prominent calcification in the wall of the abdominal aorta indicated that the arteriosclerosis was related to IRPF. We measured serum antibodies for Chlamydia pneumoniae in four cases.

Retroperitoneal fibrosis (RF) is an uncommon disease with an estimated incidence of 1 case on 200.000 inhabitants. Idiopathic RF (2/3 of cases) must be distinguished from secondary RF, caused by hexogen substances assumptions, radiotherapy and various diseases. The treatment of RF is not yet univocally defined. From 1984 to 2002 we observed 15 patients (1 case with idiopathic retroperitoneal fibrosis) i.e. 27 ureteral units (1 patient was previously nephrectomized and 2 showed a monolateral ureteral involvement). We report the first case of localized, unilateral, idiopathic retroperitoneal fibrosis encasing the left renal vein, which resulted in secondary renal vein hypertension. The patient presented with sudden and dramatic gross hematuria. Surgical release and excision of the surrounding localized retroperitoneal fibrosis resulted in prompt resolution of the hematuria.

Pergolide is a dopaminergic agonist used to treat Parkinson's disease but is associated with the development of retroperitoneal fibrosis (RPF). Newer nonergot agents (pramipexole, ropinirole) may not carry this same risk. A patient with a history of pergolide-induced RPF was treated successfully with ropinirole for 1 year without complications.

53 year old men suffered from abdominal pain, vomiting, lack of appetite, loss of weight, with ESR > 100 mm Hg and moderate anaemia was presented. Many diagnostic procedures were without results and finally laparotomy was taken. The microscopic examination of specimen of periaortic tissue revealed retroperitoneal fibrosis accompanying atherosclerotic aortae. Corticotherapy and antilipemic drugs were given. After 3 years a complete remission of periaortic pathologic mass was observed. This case could prove the autoimmunological hypothesis of retroperitoneal fibrosis.

A 25-year-old woman sought medical attention because of iliocaval manifestations of retroperitoneal fibrosis while she was taking methysergide. Laboratory studies yielded substantially increased serum procollagen III levels and anticardiolipin antibodies accompanied with anti-beta(2) glycoprotein I, findings not previously described with this disorder. Clinical and laboratory manifestations resolved after cessation of methysergide therapy.

Two cases of idiopathic retroperitoneal fibrosis (IRPF) were reported. The first case, in a 72-year old man, was admitted for hydronephrosis with indistinct origin. Following percutaneous nephrostomy, biopsy of retroperitoneal tissue was performed, and IRPF was diagnosed. The treatment consisted of bilateral ureterolysis, intraabdominal transposition of ureters and oral administration of prednisolone for 3 months. A patient with S. japonicum infestation of the liver presented with acute renal failure due to retroperitoneal fibrosis, together with the presence of low complement levels, anti-nuclear and anti-DNA antibodies. Renal failure responded to intravenous bolus methylprednisone and later definitive surgery, with complement levels returning to normal after the acute episode. The findings are consistent with the hypothesis that retroperitoneal fibrosis may be a local manifestation of a systemic immune disease, in which the associated S. japonicum infestation may have a role.

After 3 sessions of haemodialysis, renal function returned to normal, but the bilateral ureteral obstruction recurred 11 months later with signs of compression of the inferior vena cava. Corticosteroid therapy in doses of 0.5 mg/kg/day resulted in disappearance of the pyeloureteral dilatation at intravenous urography and regression of the fibrosis on CT sections. From a review of the literature the authors describe the clinical symptoms, diagnostic methods and main treatments of perianeurysmal retroperitoneal fibrosis, a not uncommon disease.

A patient with Stage IB squamous cell carcinoma of the cervix associated with benign retroperitoneal fibrosis and hydroureters is reported. It is suggested that staging laparotomy is indicated in such a case to assess whether hydroureters are related to benign fibrosis or cancer metastasis.

The ordinary manifestations of idiopathic retroperitoneal fibrosis (IRF) with involvement of the ureters and urinary obstruction are well known. Quite a number of publications have made it clear that this disease process can often also affect organ systems other than the urinary tract--from the trachea and mediastinum to the rectum. The present case report calls attention to the fact that IRF may also be a surgically treatable cause of obstructive icterus.
