BACKGROUND: Fractures of the scaphoid bone are common but can easily be overlooked in standard X‑rays. Inadequate diagnostics and therefore inappropriate treatment of scaphoid fractures often leads to problems in healing with formation of non-union and painful osteoarthritis of the wrist.
OBJECTIVE: This review summarizes the current practical recommendations in the diagnostics and treatment of acute scaphoid fractures.
METHODS: An analysis and review of selected literature including the current S3 guidelines were performed.
RESULTS: The main statements are that in cases of a clinically sus…

INTRODUCTION: Over the past years, different fixation techniques focused on rotational stability in order to increase stability and stimulate union rates. Additionally, extracorporeal shockwave therapy (ESWT) has gained importance in the treatment of delayed and nonunions. Purpose of this study was to compare the radiological and clinical outcome of two headless compression screws (HCS) and plate fixation in scaphoid nonunions, in combination with intraoperative high energy ESWT.
MATERIALS AND METHODS: Thirty-eight patients with scaphoid nonunions were treated by using a nonvascularized bone

This study evaluated the effects of sectioning the scapholunate interosseous ligament, radioscaphocapitate ligament, and scaphotrapezial ligament on the kinematics of the scaphoid and lunate. Eight cadaver upper extremities were placed in a wrist joint simulator and moved in continuous cycles of flexion-extension and radial-ulnar deviation. Positional data of the scaphoid and lunate were obtained in the intact state, after the scapholunate ligament was cut; after the scapholunate and scaphotrapezial ligaments were cut; after the scapholunate, scaphotrapezial, and radioscaphocapitate ligaments

Tenderness in the anatomical snuff box (ASB) has long been accepted as an indicator of a possible scaphoid fracture. The longitudinal axis of the scaphoid lies in an anteroposterior plane, perpendicular to the remainder of the carpal bones, in the radially deviated wrist. The scaphoid tubercle can therefore be easily and accurately palpated on the palmar aspect of the radially deviated wrist. This study was devised to determine if tenderness over the scaphoid tubercle (ST) was superior to ASB tenderness in identifying definite fractures. Over a 10-month period 246 patients were seen who were s

PURPOSE OF THE STUDY To compare the treatment outcome of scaphoid facture fixation with one versus two Herbert screws (HBS). MATERIAL AND METHODS 72 patients underwent open reduction internal fixation (ORIF) following acute scaphoid fracture, and were followed prospectively by one surgeon. All fractures were Herbert & Fisher classification type B, the most common fracture lines being oblique (n=38) and transverse (n=34). Fractures with similar fracture lines were randomly assigned into two groups; fractures stabilized with one HBS (n=42) and fractures stabilized with two HBS (n=30). A specific

Melorheostosis is a rare mesodermal disease affecting the skeleton and adjacent soft tissues. Often it is incidentally detected on radiographs. In the standard radiology and orthopedics literature, melorheostosis is described as a "flowing hyperostosis, resembling dripping candle wax as an incidental radiographic finding." A 22-year-old man presented with a 2-year history of right-hand pain. Radiologic evaluation of the hand showed massive sclerotic changes in the first and second metacarpal and phalangeal bones on the right side. Further radiographic evaluation of the right upper extremity re

Proximal pole fractures of the scaphoid have a high incidence of nonunion and avascular necrosis. Because of their poor prognosis, the treatment of these fractures remains controversial. 102 patients with symptomatic nonunion of the proximal pole were treated by simple osteosynthesis, using retrograde screw fixation through a direct dorsal approach to the scaphoid. Fracture preparation and bone grafting were kept to a minimum, in order to preserve as much bone stock as possible, and to avoid damage to the already compromised vascularity of the proximal fragment. No postoperative splinting was

PURPOSE: The effect of scaphoid nonunion deformity on wrist function is uncertain due to the lack of reliable imaging tools. Advanced three-dimensional (3-D) computed tomography (CT)-based imaging techniques may improve deformity assessment by using a mirrored image of the contralateral intact wrist as anatomic reference. The implementation of such techniques depends on the extent to which conventional CT is currently used in standard practice. The purpose of this systematic review of medical literature was to analyze the trend in CT scanning scaphoid nonunions, either unilaterally or bilatera

BACKGROUND: The scaphoid is the most commonly fractured carpal bone, often going into non-union and secondary complications. Despite this, no explicit guidelines exist for the management of its non-union till date. This study quantitatively analyses the existent research on scaphoid nonunions from 2013 to 2024 using bibliometric methodology.
MATERIALS AND METHODS: The Web of Science database was used to search for and extract publications using the keywords 'scaphoid' AND 'nonunion'. Articles were then filtered based on their eligibility, and the metadata of the remaining articles was analyse

The treatment choice in scapholunate (SL) injury depends on the extent of the SL ligament tear, chronicity of injury, quality of the ligament remnants, reducibility of carpal malalignment, and cartilage status of the radiocarpal and midcarpal joints. In the absence of degenerative changes with chronic reducible dissociation, the optimal treatment would be the reconstruction of the SL interosseous ligament. Various SL reconstruction techniques via open or arthroscopic approaches have been described over the years; they include tendon reconstructions, volar/dorsal capsulodesis, SL allografts, bo

Preiser's disease or idiopathic avascular necrosis of the scaphoid is a rare condition where ischemia and necrosis of the scaphoid bone occurs without previous fracture. It is thought to be caused by repetitive micro trauma or side effects of drugs (e.g., steroids or chemotherapy) in conjunction with existing defective vascular supply to the proximal pole of the scaphoid. Wrist radiography or CT coupled with MRI is the imaging modality of choice in the diagnosis of this rare entity. Here, we report a case of Preiser's disease of the left wrist in a 17-year-old female patient who presented with…

Scaphoid nonunions in children requiring operative treatment are rare. The aim of this study was to analyze our own patient population of the last 13 years to assess the postoperative outcome of these operations. Seven patients, under aged 14 years, were treated for scaphoid nonunions. Patients were clinically and radiologically examined and subjective outcomes were measured, such as the visual analogue pain scale (VAPS), Disability of the Arm, Shoulder and Hand (DASH) score, and Krimmer score. All patients showed an excellent outcome, as determined by the DASH score, Krimmer score, and VAPS.

We report four cases of late avascular necrosis (AVN) of the proximal part of the scaphoid following apparent healing of acute scaphoid fractures. One patient had been treated conservatively, by plaster immobilization, and the other three had undergone internal fixation of their acute fractures. The onset of symptoms associated with AVN varied, being as late as 2 years in one patient. Late AVN following healing of a scaphoid fracture does not appear to have been previously recognized, perhaps due to the fact that patients are seldom followed up for long enough. We feel that this condition is i

For the treatment of scaphoid non-unions, innumerable curative and palliative operations have been recommended. Of the curative operations, the various methods of bone grafting give the best results, whereas screw fixation is not satisfactory. Bone pegs with a cortical graft are no longer used because of technical difficulties. Corticocancellous grafts (Matti-Russe) yield bony union in almost 90% of cases. The combination of corticocancellous grafts with plate stabilization gives the highest rate of bony union (99%). A cure should therefore always be attempted in all non-union cases, with the

BACKGROUND: The all-dorsal scapholunate reconstruction technique is indicated for the treatment of scapholunate injuries in cases in which the carpus is reducible and there is no arthrosis present. The goal of this procedure is to reconstruct the torn dorsal portion of the scapholunate ligament in order to stabilize the scaphoid and lunate.
DESCRIPTION: A standard dorsal approach to the wrist, extending from the third metacarpal distally to the distal radioulnar joint, is utilized. The extensor pollicis longus is transposed and retracted radially, and the second and fourth extensor compartmen

INTRODUCTION: Scaphoid fractures are one of the most common carpal bone fractures, with most fractures involving the waist. When surgery is indicated, internal fixation with screws is the standard method for the fixation of these fractures. Accurate length and trajectory of the screw are two crucial parameters essential for optimal internal fixation. Determining the accurate screw length required for fixation can be challenging, especially in an intraoperative setting. Preoperative determination of the screw length can improve surgical precision, thereby contributing to an improved functional

A retrospective review of 160 cases of scaphoid nonunion treated by internal fixation using a Herbert screw with bone grafting was conducted at an average followup of 24 months. Definite radiographic union was achieved in 90% of cases. Based on Cooney's clinical scoring system, 80 cases had an excellent result, 37 had a good result, 33 had a fair result, and 10 had a poor result. Failure of union was related to the existence of avascular changes of the proximal fragment, instability of the fracture fragment, the prolonged delay in surgery, and the location of the fracture site. In the united s

At the transitional zone from the forearm to the hand the insertion tendon of the m.flexor carpi radialis (FCR) glides on a fibrous and fatty cushion, which is connected dorsally with the joint capsule of the radiocarpal articulation. The tendon distally crosses the palmar side of the scaphoid tubercle and enters the dorsally curved rim of the trapezoid tubercle. At the level of the wrist joint the narrow tendon sheath begins, which extends to the insertion at the metacarpus. Immediately after entering the gliding tunnel the tendon branches off radially as a rule with an accessory fibre strand
