Pioglitazone in Skin Fibrosis: Mechanistic Rationale and Therapeutic Potential of Pioglitazone for Scarring Dermatoses.

Cutaneous fibrosis - encompassing keloids, hypertrophic scars, localized scleroderma (morphea), and scarring alopecias - remains fundamentally undertreated, with conventional interventions frequently yielding incomplete therapeutic responses and high rates of recurrence. Pioglitazone, a Food and Drug Administration (FDA)-approved peroxisome proliferator-activated receptor γ (PPAR-γ) agonist traditionally utilized for glycemic control in type 2 diabetes mellitus, represents a highly compelling candidate for dermatologic repurposing. Beyond its canonical metabolic functions, robust target engage

Anti-collagen type v: a marker of early systemic sclerosis?

OBJECTIVE: To evaluate the frequency of anti-collagen type V in humans with early systemic sclerosis (SSc) compared to defined SSc patients and healthy controls, since collagen type V was shown to be overexpressed in early SSc patients' skin and there is no data concerning the presence of this antibody in early stages of human SSc. Experimental studies showed that animal models immunized with collagen type V developed a disease similar to human systemic sclerosis (SSc), with antibodies production, mainly in early stages post-immunization. METHODS: Eighty-one female SSc patients were included

Specific management of lipodermatosclerosis (sclerotic hypodermitis) in acute and chronic phase.

Lipodermatosclerosis or sclerotic hypodermitis is presented as a complication of venous insufficiency and in particular of post-thrombotic syndrome with a high risk of progression to leg ulcers. However, it has also been described in obese patients without venous insufficiency, and even in the course of various systemic diseases including scleroderma. It most often affects middle-aged women and is usually bilateral, with a typically "inverted champagne bottle" leg appearance. The pathogenic role of venous hypertension explains why compression with bands or stockings is the basis of treatment.

Recent progress in studies of miRNA and skin diseases.

miRNA is a family of small non-coding RNA that consists of 22 nucleotides on average. miRNA are implicated in various cellular activities such as cell proliferation or migration via the modulation of gene expression, and also are linked to the pathogenesis of human diseases. This paper reviews recent research progress about the contribution of miRNA to the pathogenesis of various skin diseases, and possible application of miRNA as the disease markers in each disease. For example, downregulated miR-424-5p in psoriatic skin causes the overexpression of MEK1 and cyclin E1 in psoriatic keratinocyt

Phototherapy in the age of biologics.

Dermatologists are presented with a diversity of therapeutic modalities for the treatment of inflammatory, sclerosing, and neoplastic conditions, but with the development of various new irradiation devices that utilize specific parts of the electromagnetic spectrum, phototherapy has become a more viable, accessible, and efficacious option in the treatment of these conditions. The ultraviolet (UV) range (10-400 nm) is further subdivided into UVA and UVB, each of which has been particularly useful in a number of skin conditions. The most commonly used forms of UV irradiation are UVA1, psoralen p

[Necrobiosis lipoidica over appendectomy scar in a patient with morphea of the breast].

Morphea is a sclerosing skin disease that may be clinically confused with a carcinoma when it involves the breast. It has been described in association with earlier radiotherapy and with silicone breast implants. We describe the case of a woman with morphea of the breast who developed necrobiosis lipoidica over the scar of an appendectomy performed 38 years earlier. Necrobiosis lipoidica over surgical scars is infrequently found in the literature. We review the published cases and discuss the association of necrobiosis lipoidica with morphea.

Associated localization of morphea and lichen planus of the lip in a patient with vitiligo.

Morphea, lichen planus and vitiligo are common diseases, but seldom associated. Morphea or lichen planus in the lips have not frequently been reported, and an associated localization of both lesions in the lip mucosa is indeed a rare event. A case of morphea and lichen planus localized in the upper lip mucosa in a patient with vitiligo is reported. Immunohistochemistry revealed a clear prevalence of T-lymphocytes in the inflammatory infiltrates of morphea and in the band-like infiltrate of the lichen, a finding consistent with previous reports of morphea, lichen planus, and vitiligo. The inver

Keratoderma with pseudo-Quincke's pulse.

The physical examination sign known as Quincke's capillary pulse is elicited by pressing lightly on the distal edge of a fingernail. When the vasculature of the nailbed is partly blanched, Quincke's pulse appears as alternating flashes of red and white that reveal arterial pulsations. This sign is seen in many normal persons, but it is most prominent in individuals with widened pulse pressures, such as aortic insufficiency. This case report describes a patient with keratoderma and sclerodactyly whose fingernails exhibited rapid color changes, mimicking Quincke's pulse. The visible pulsations w

Generalized morphoea secondary to porphyria cutanea tarda.

We report a patient with generalized morphoea secondary to porphyria cutanea tarda, who responded to treatment with cyclosporin. The differences between the sclerodermatous skin changes seen in porphyria cutanea tarda and generalized morphoea are discussed.

Linear scleroderma: a response to neurologic injury? Report and literature review.

A 28-year-old white man presented with neurologic symptoms and skin changes. Subsequent evaluation led to the diagnosis of transverse myelitis of the cervical spine (C8) and linear scleroderma. The progression of neurologic abnormalities prompted treatment with corticosteroids. Neurologic symptoms diminished and the progression of linear skin lesions halted. A review of the literature uncovered considerable evidence for underlying abnormalities of the spine and spinal cord in many patents with linear scleroderma and a paucity of immunologic abnormalities characteristic of progressive systemic

Acquired Brown's syndrome in a patient with combined lichen sclerosus et atrophicus and morphoea.

A 49-year-old woman with generalised lichen sclerosus et atrophicus and morphoea developed bilateral Brown's syndrome. Some of the skin lesions were in the vicinity of the trochlea. A characteristic feature of morphoea is subcutaneous fibrosis, so we postulate that the cause of the Brown's syndrome was mechanical tethering of the superior oblique tendon by deep subdermal fibrosis. Histopathological diagnosis was made from biopsies of similar lesions on the patient's face.
