Atypical Osteonecrosis of Humeral Head FollowingAnterior Shoulder Dislocation.

INTRODUCTION: Osteonecrosis of the humeral head is not a very common entity. It is usually associated with comminuted proximal humerus fractures. We report a rare case of osteonecrosis of humeral head after anterior shoulder dislocation without any fracture of the proximal humerus. CASE REPORT: We report a case of a 24-year-old male who sustained a left-sided anterior subcoracoid shoulder dislocation following a road traffic accident. The shoulder was closed reduced, and a post-reduction radiograph was taken to confirm concentric reduction. At 6-month and 2-year follow-up, the radiographs sho

Patient-evaluated functional outcome is systematically different from physician-evaluated outcome after proximal humerus fracture fixation.

AIMS: The study aimed to investigate the agreement between Constant-Murley score (CMS) and the shoulder subjective value (SSV); and the association of poor self-evaluated outcome with shoulder pain, range of motion (ROM) limitation, and shoulder strength after fixation of proximal humerus fractures. METHODS: The study included 74 shoulders in 72 patients (mean age 56 years (SD 14), 40 male (56%)) who underwent locking plate fixation of displaced proximal humerus fractures between January 2015 and January 2024. We evaluated CMS, SSV, pain scores (visual analogue scale), and ROM measurements at

Patient related functional outcome of glenoid rim fractures treated with open reduction and internal fixation.

The aim of this study was to evaluate patient related outcome and shoulder stability following open reduction and internal fixation (ORIF) in patients with glenoid rim fractures. After a median follow-up of four years, 14 patients completed the Rowe Shoulder Stability Score and Quick DASH questionnaire. The median Rowe score was 90 (Q1: 88, Q3: 100). Results were graded excellent in 11 patients and good in three. The median DASH score was 4.6 (Q1: 0, Q3: 32). In conclusion this study showed that ORIF of type la and 2 glenoid rim fractures provided satisfactory results with respect to preventio

[Shoulder and arm injuries in alpine ski racing. A survey on the frequency of these injuries with athletes of the Swiss Ski cadres and the regional cadres].

The aim of this work was to examine the frequency and pattern of injuries of the upper extremity among Swiss Ski athletes and among regional team athletes. The injury pattern regarding shoulder and arm injuries in alpine skiers in Switzerland correlates with the previous literature. The shoulder joint is most frequently injured, followed by injuries of the fingers, particularly the thumb. Regarding injury frequency no difference between ladies and men was seen. On the other hand, an increased injury rate could be seen in regional team drivers. Most injuries happened for both genders and in all

PHILOS plate versus nonoperative treatment in 2-, 3-, and 4-part proximal humeral fractures: Comparison with healthy control subjects.

PURPOSE: There is no consensus in the literature on nonoperative treatment of displaced and multipart fractures of proximal humerus as those are normally treated operatively. Our aim was to compare the functional results of nonoperative management and open reduction internal fixation with the proximal humerus internal locking system of 2-, 3-, and 4-part proximal humerus fractures, among themselves and with a healthy control group. METHODS: Between 2014 and 2018, 92 proximal humerus fractures constituting a nonoperative group (n = 47) and an operative group (n = 45) together with healthy cont

Classification of proximal humeral fractures based on a pathomorphologic analysis.

BACKGROUND: The purpose of this study was to analyze the pathomorphology of proximal humeral fractures to determine relevant and reliable parameters for fracture classification. METHODS: A total of 100 consecutive acute proximal humeral fractures in adult patients were analyzed by 2 non-independent observers from a single shoulder department using a standardized protocol based on biplane radiographs and 3-dimensional computed tomography scans. A fracture classification system based on the most reliable key features of the pathomorphologic analysis was created, and its reliability was tested b

Comminuted Bilateral Simultaneous Fracture and Luxation of the Proximal Humerus with an Injury to the Right Axillary Artery.

INTRODUCTION: Among all of the shoulder dislocation cases, only 1% is related to fractures. The simultaneous bilateral fracture and dislocation happen very rarely and it is seldom related to a trauma. An injury to the axillary artery related to a humerus fracture is an exceptional event. CASE REPORT: A 58-year-old male with no particular background who, after falling from a 5 m height, presented a fracture and dislocation of both of his shoulders and showed no pulse in his right arm due to an injury to his axillary artery. The peculiarity of this injury increases when the axillary artery inju

Locking plate fixation of proximal humerus fractures in patients older than 60 years continues to be associated with a high complication rate.

BACKGROUND: Locking plate technology has increased the frequency of open reduction and internal fixation (ORIF) of proximal humerus fractures (PHF). A number of technical pearls have been recommended to lower the complication rate of ORIF. These pearls are particularly relevant for patients aged >60 years, when nonoperative treatment and arthroplasty are alternatives commonly considered. There have been few large, single-center studies on the modern application of this technology. METHODS: Between 2005 and 2015, a total of 173 PHFs in patients aged >60 years were treated at our institution wi

[Fractures of the glenoid surface of the scapula].

The communications of literature regarding osteosynthesis in glenoid fractures of the scapula are reviewed, and six own cases are presented. The results are better than expected even in case of comminuted fractures. The indication for surgical reconstruction should therefore be adopted in dislocated fractures of the shoulder joint surface. Although the adequate operation technique is still under discussion, there seems to be a trend towards screwing and a narrow DC plate.

Scapular fractures osteosynthesis via Judet's posterior approach: clinical and radiographic results.

PURPOSE: Scapular fractures are uncommon and mainly treated nonoperatively. Judet's posterior approach allows access to the fracture site through the infraspinatus fossa and may be a technical option when ORIF is decided. The aims of this study were to determine clinical and radiographic outcomes of patients who underwent scapular body and/or glenoid fractures fixation via Judet's posterior approach. METHODS: We conducted a retrospective single-centre study, and all patients admitted for scapular fracture who underwent osteosynthesis via Judet's approach between January 2014 and September 202

Reduction of anterior glenohumeral dislocations: a new closed reduction technique.

OBJECTIVE: A new closed reduction technique for anterior glenohumeral dislocations and tuberosity fracture dislocations is introduced. METHODS: Forty-one consecutive patients with an acute anterior glenohumeral dislocation or tuberosity fracture dislocation underwent closed reduction by an orthopaedic surgeon employing this new method. RESULTS: Closed reduction was successful in 88% of patients using the reduction maneuver. Associated fracture with glenohumeral dislocation did not influence the success rate of the reduction maneuver. An assistant was needed in 15% of cases. No complications

[Proximal humerus fracture: new aspects in epidemiology, fracture morphology, and diagnostics].

The incidence of proximal humerus fractures is rising and they constitute the third most frequent fracture in the elderly after femoral fractures in the hip area and radius fractures. They are caused by the age-related increase in osteoporosis. In contrast to young people, low-energy trauma involved in simple falls represents the mechanism that leads to the injury in older people. Numerous authors have introduced systems for the classification of proximal humerus fractures. After a thorough clinical examination of the affected extremity including assessment of circulation, motor function, and s

Minimally invasive fixation of displaced midclavicular fractures with titanium elastic nails.

OBJECTIVES: Clavicular fractures account for 2.6% of all fractures, and more than 80% involve the middle third of the clavicle. Plate fixation has been the most common method of fixation reported but has been associated with complications such as infection, wound breakdown, nonunion, implant failures, poor cosmetic outcome, and local skin numbness. We report on a series of cases receiving minimally invasive insertion of titanium elastic nails (TEN) to fix the displaced midclavicular fractures. DESIGN: Prospective, clinical study. SETTING: Regional referral center. PATIENTS/PARTICIPANTS: Fro

Arthroscopic Management of a Coracoid Fracture Associated With Acromioclavicular Dislocation: Technical Note.

Operative management of a coracoid process fracture is indicated in case of painful nonunion, displacement of more than 1 cm, or multiple disruptions of the superior shoulder suspensory complex. Several techniques have been described with open reduction of the fracture and internal fixation using cortical screws with or without additional fixation of the acromioclavicular joint. This Technical Note aims to introduce an alternative safe, minimally invasive method for arthroscopic fixation of a coracoid fracture with simultaneously reduction of the acromioclavicular joint. The described arthrosc
