[Fractures of the humeral head. Conservative and surgical differential therapy].

The Neer classification for fractures of the proximal humerus is a system based on accurate identification of the four major fragments and their relationship to each other. There is a specific type of displaced 4-fragment fracture of the proximal humerus which consists in valgus impaction of the head fragment. This type of fracture is combined with a lower rate of avascular necrosis than that of other displaced 4-fragment fractures. In addition, the outcome is different according to whether a displaced 4-fragment fracture affects the surgical or the anatomical neck. Those through the surgical

Reverse shoulder arthroplasty for proximal humerus fracture using a dedicated stem: radiological outcomes at a minimum 2 years of follow-up-case series.

BACKGROUND: Complex proximal humeral fractures are very difficult to treat particularly in patients older than 65 years with an osteoporotic bone and tuberosities compromised. The goal of this paper is to evaluate radiological outcomes at mid-term follow-up of proximal humerus fractures treated with reverse shoulder arthroplasty using a dedicated fracture stem. MATERIALS AND METHODS: The study population included 98 patients who underwent reverse shoulder with a dedicated fracture stem for an acute proximal humerus fracture; 87/98 patients were available for analysis. There were 62 female and

Complications of shoulder arthroplasty.

Early and mid-range followup studies of shoulder arthroplasty have been encouraging, showing good and excellent results in > 90% of shoulders. Despite this success, complications in shoulder replacement surgery are inevitable, with an incidence of approximately 14%. Numerous complications have been identified and include the following factors in order of decreasing frequency: instability, rotator cuff tear, ectopic ossification, glenoid component loosening, intraoperative fracture, nerve injury, infection, and humeral component loosening. Successful treatment of these difficulties requires car

Optimizing Infection Prevention in Primary Shoulder Arthroplasty: Evidence-Based Strategies and Best Practices.

» Based on the available evidence, there are numerous strategies that may reduce infection risk in primary shoulder arthroplasty. Preoperatively, optimization of iron deficiency anemia and smoking cessation are associated with lower rates of periprosthetic joint infection and perioperative complications. In addition, risk of infection may be mitigated by deferring shoulder arthroplasty for at least three months following a corticosteroid injection. Management of biologic and targeted immunosuppressive therapies should be coordinated with medical specialists and tailored to the specific agent's

Short stem humeral components in reverse shoulder arthroplasty: stem alignment influences the neck-shaft angle.

INTRODUCTION: Shorter humeral reverse total shoulder arthroplasty (RTSA) stems may reduce stress shielding, however, potentially carry the risk of varus/valgus malalignment. This radiographic study's purpose was to measure the incidence of stem malalignment and thus the realized neck-shaft angle (NSA). The hypothesis was that malalignment of the stem is a frequent postoperative radiographic finding. METHODS: Radiographs of an uncemented curved short stem RTSA with a 145° NSA were reviewed. The study group included 124 cases at a mean age of 74 (range 48-91) years. The humeral stem axis was me

Shoulder hemiarthroplasty for steroid-associated osteonecrosis.

Currently, there is little information on the outcome of humeral head replacement for steroid-associated osteonecrosis of the humeral head. The purpose of this study was to evaluate the outcome of patients who underwent humeral head replacement for steroid-associated osteonecrosis to determine the results, risk factors for an unsatisfactory outcome, and rates of revision surgery. Between 1980 and 2000, 32 shoulder hemiarthroplasties were performed for steroid-associated osteonecrosis. We included 31 hemiarthroplasties in 25 patients with a minimum 2-year follow-up (mean, 12.0 years) in the stu

Reverse Shoulder Arthroplasty Is Superior to Plate Fixation at 2 Years for Displaced Proximal Humeral Fractures in the Elderly: A Multicenter Randomized Controlled Trial.

BACKGROUND: Almost one-third of patients with proximal humeral fractures are treated surgically, and the number is increasing. When surgical treatment is chosen, there is sparse evidence on the optimum method. The DelPhi (Delta prosthesis-PHILOS plate) trial is a clinical trial comparing 2 surgical treatments. Our hypothesis was that reverse total shoulder arthroplasty (TSA) yields better clinical results compared with open reduction and internal fixation (ORIF) using an angular stable plate. METHODS: The DelPhi trial is a randomized controlled trial comparing reverse TSA with ORIF for displa

Is failure of tuberosity suture repair in hemi-arthroplasty for fracture mechanical?

BACKGROUND: Tuberosity suture repair in hemiarthroplasty for fracture carries risk of malunion giving poor results. Is failure mechanical or biochemical? We investigated the mechanical aspect with repetitive loading in cadaveric repairs. MATERIALS AND METHODS: Tests were carried out in line with U.K. Human Tissue Authority regulations. A 4-part fracture was created in 8 cadaver shoulders by osteotomizing the tuberosities. A standard hemiarthroplasty implant was cemented in at correct height and retroversion, and standardized repairs applied. Initial firmness of repair was confirmed by attempt

Replantation of an autoclaved autogenous segment of bone for treatment of chondrosarcoma. Long-term follow up.

Seven patients who had a low-grade chondrosarcoma of the proximal part of the humerus or femur were treated by resection, autoclaving of the excised segment, and reinsertion of that segment with supplementary fresh autogenous grafts. The length of follow-up ranged from fourteen to twenty-four years, and sequential roentgenograms were made during the follow-up period. A biopsy of the autoclaved segments was done in one patient eleven years after the operation, and examination of the specimen showed predominantly live bone; there were very few spicules of dead bone. Although the principal roentg

Biomechanics of Reverse Shoulder Arthroplasty:
Current Concepts.

The evolution of reverse shoulder arthroplasty has provided surgeons with new solutions for many complex shoulder problems. A primary goal of orthopaedics is the restoration or re-creation of functional anatomy to reduce pain and improve function, which can be accomplished by either repairing injured structures or replacing them as anatomically as possible. If reconstructible tissue is lacking or not available, which is seen in patients who have complex shoulder conditions such as an irreparable rotator cuff-deficient shoulder, cuff tear arthropathy, or severe glenoid bone loss, substantial pr

Short-stem uncemented primary reverse shoulder arthroplasty: clinical and radiological outcomes.

Short uncemented humeral stems can preserve humeral bone stock and facilitate revision surgery but may be prone to malalignment or loosening, especially when used in reverse total shoulder replacement (TSR). We undertook a retrospective review of 44 patients with a mean age of 76 years (59 to 92) who underwent primary reverse TSR with a short uncemented humeral stem. There were 29 females. The indications for joint replacement included cuff tear associated arthropathy (33), avascular necrosis (six), post-traumatic arthritis (two), and inflammatory arthritis (three). At a mean follow-up of 27 m

Etiology of prosthetic joint infections in a tertiary care centre in Italy.

Prosthetic joint infections (PJIs) represent a severe complication in orthopaedics. Coagulase-negative staphylococci (CoNS) and Staphylococcus aureus represent the most frequent cause, but Gram-negatives have also been reported. With a view to describing the aetiology of PJIs diagnosed from January 2005 to September 2007 at S. Corona Hospital in Pietra Ligure, Italy, we conducted retrospective analysis of pathogens isolated from PJIs by means of surgical specimens, needle aspirates or swabs of fistula (3 samples). During the study period 228 PJIs were described and 141 (62%) were microbiologic

The mid-term outcomes on reverse total shoulder arthroplasty with isolated latissimus dorsi transfer in patients with isolated external rotation lag without pseudoparalysis: an average follow-up of 5 years.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is a well-established replacement strategy to treat irreparable massive rotator cuff tears and cuff tear arthropathy. In patients with forward (FF) deficits combined with a loss of external rotation (ER), RSA with latissimus dorsi transfer (LDT), which was initially used for brachial plexopathies, has been addressed to restore both functional deficits. The aim of the study is to assess mid-term clinical and radiological outcomes of concomitant RSA with LDT in patients with significant preoperative external rotation lag greater than 30 degr

Effect of antiseptic irrigation with 0.05% chlorhexidine gluconate (Irrisept) on the incidence of Cutibacterium acnes in primary shoulder arthroplasty.

BACKGROUND: Previous literature has demonstrated high rates of Cutibacterium acnes (C. acnes) colonization in patients undergoing primary shoulder arthroplasty. Residing primarily within the dermal layer, C. acnes poses a major threat to intraoperative wound contamination with subsequent risk of developing periprosthetic joint infection. Discovering an effective perioperative prophylaxis strategy is imperative. To date, no such regimen has shown the ability to reliably suppress C. acnes from the deeper tissues. This study was designed to investigate whether intraoperative Irrisept irrigation w

Outcomes in the treatment of periprosthetic joint infection after shoulder arthroplasty: a systematic review.

BACKGROUND: Although many case series have been published on the treatment of shoulder periprosthetic joint infection (PJI), few provide direct comparison of outcomes. This systematic review synthesizes the available English literature on shoulder PJI to quantify treatment effectiveness. METHODS: The PubMed and Embase databases were searched in April 2014. Keywords included "shoulder replacement infection" and "shoulder arthroplasty infection." Of 663 titles, 55 manuscripts underwent full-text review after application of exclusion criteria. Thirty articles were finally included. RESULTS: Pro

[The magnetic resonance tomographic findings following the resection arthroplasty of the caput humeri].

PURPOSE: The resection of the humeral head is normally carried out only with respect to four-part and dislocation fractures in elderly patients. The purpose of this study was to examine whether there is a correlation between the clinical results and the MRI findings. MATERIAL AND METHODS: 8 patients underwent clinical and MR examination two to 6 years after resection. RESULTS: A cartilage-like tissue formation between the remaining humeral head and the glenoid fossa, with signal intensities comparable to hyalin cartilage was found in all patients. CONCLUSION: The good functional results fol

What Factors are Predictive of Patient-reported Outcomes? A Prospective Study of 337 Shoulder Arthroplasties.

BACKGROUND: Although shoulder arthroplasties generally are effective in improving patients' comfort and function, the results are variable for reasons that are not well understood. QUESTIONS/PURPOSES: We posed two questions: (1) What factors are associated with better 2-year outcomes after shoulder arthroplasty? (2) What are the sensitivities, specificities, and positive and negative predictive values of a multivariate predictive model for better outcome? METHODS: Three hundred thirty-nine patients having a shoulder arthroplasty (hemiarthroplasty, arthroplasty for cuff tear arthropathy, ream
