OBJECTIVE: To characterize the incidence and complications resulting from venous insufficiency after neurotologic-skull base surgery.
STUDY DESIGN: Retrospective case review of >3,500 cases.
SETTING: Tertiary referral center, inpatient surgery.
PATIENTS: Six patients: four with complications related to chronic venous insufficiency and two with complications related to acute venous insufficiency.
INTERVENTION(S): Medical (steroids, acetazolamide, hyperventilation, mannitol) and surgical (lumboperitoneal shunt, optic nerve decompression, embolectomy) interventions were undertaken.

A case of fibrous dysplasia of the frontal bone in a 51 year-old male is described. He was admitted to our hospital with a hard, painless growing mass in the left frontal region. A symmetrical protrusion of his forehead has been observed since several years before. Neurological examination and laboratory data revealed no abnormalities. Skull x-rays demonstrated two different lesions. One showed a ground glass appearance in the supraorbital region, and the other showed a radiolucent lesion with marginal sclerosis crossing the left coronal suture CT scan revealed an intradiploic multilocular mas…

Many tumours of the skull base, including schwannomas, paragangliomas, meningiomas and anterior pituitary tumours occur both as sporadic tumours and in clear cut familial syndromes. These cancers' highly penetrant dominantly inherited syndromes have proven to be a rich resource for locating tumour genes, most of which are of the tumour suppressor type. Recently, the gene for type 2 neurofibromatosis (NF2) was isolated by the technique of positional cloning. The NF2 gene has now been demonstrated to be involved in the pathogenesis of both familial and non-familial vestibular schwannomas as well…

The majority of case reports concerning intracerebral tumors are almost equally divided between teratomas and gliomas. We report a case of congenital intracranial teratoma with suggested management. An 18-year-old woman at 30 weeks' gestation in active labor because of severe fetal hydrocephalus and polyhydramnios was delivered by cesarean section. Spontaneous rupture of the neonatal skull led to delivery of a stillborn infant; a congenital teratoma was found. Although intracranial teratoma is a well-recognized entity in the differential diagnosis of pediatric brain tumors, massive congenital …

In infancy, prior to cranial suture and fontanel calcification, the craniofacial skeleton can be easily deformed by an externally exerted force. In this study, the relationship between the sleep position and skull morphology was investigated. A group of 81 cleft lip and/or palate infants without other systemic anomalies was first seen in the craniofacial center at approximately 1 month of age. The sleep position of each infant was recorded as supine, prone, or mixed type. The body and skull growth were longitudinally measured at 1, 3, and 6 months of age. Infants sleeping in the supine sleep p…

We studied two young males who had visual field defects consistent with optic chiasmal injury after blunt frontal head trauma. One patient also had a unilateral optic neuropathy. Long-term follow-up disclosed complete bi-temporal hemianopsias in these patients. Both patients had midline basilar skull fractures that traversed the midclivus through the sella turcica floor, dorsum sellae, and sphenoid sinus. Magnetic resonance images did not identify intrachiasmal hemorrhage as the cause of the visual field defect. We believe this fracture pattern, in conjunction with the magnetic resonance image…

Intraoperative monitoring of neurophysiologic function is rapidly evolving as an important adjunct during skull base surgery to reduce the incidence of neurologic deficit. Facial nerve monitoring is an excellent model, since electrical and mechanical evoked potentials can be directly presented to the surgeon in real-time through an acoustic loudspeaker display. The lower cranial nerves may also be monitored using similar electromyographic techniques. Auditory system monitoring is more difficult due to the low amplitude response that requires averaging and filtering to extract the evoked potent…

Cranial nerve palsies developed in 23% of 73 children with basilar skull fractures. The majority occurred in patients 10 years of age or older. Our overall incidence was higher than that in a review in which basilar skull fractures were indiscriminately included with all head injuries. Our experience with meningitis was similar to that present in the literature and would seem to support avoidance of prophylactic antibiotics. Because there was no improvement in neurological deficits in patients treated with steroids (the only case of meningitis in our series was associated with steroids), we be…

A case of a barbed spear injury to the left orbit and skull base is presented. The unusual nature and circumstances of the injury and the management problems related to the proximate neurovascular bundles are discussed.

A rapidly expanding huge cystic lesion of the skull was the cause of unilateral exophthalmus in a 33 year old man. The differential diagnosis included an aneurysmal bone cyst, keratinous cyst, leptomeningeal cyst and intraosseous hemangioma. Although an epithelial lining was verified, the exact nature of the cyst could not be clarified despite immunohistochemical and electron microscopical investigations.

A case of growing skull fracture with rapid growth is presented. A 4-month-old male fell and struck the right side of his head. Skull X-ray just after injury showed a right parietotemporal linear skull fracture with the maximum width of 4 mm. The width of the fracture line increased to 9 mm 7 hours after injury. Serial CT scan demonstrated growth of contusional hematoma just beneath of the fracture. Six days later, a pulsatile mass was palpated beneath the scalp, although neurological state was normal. The width of the fracture line was demonstrated to increase up to 11 mm on X-ray at that tim…

Two cases of Clostridium perfringens infection occurring less than 24 hours after compound depressed skull fracture are reported. The infection was principally intracranial in the first and extracranial in the second; both required surgical debridement and antibiotic treatment. Attention is drawn to the rapidity with which a potentially life-threatening infection can develop in civilian head injury and to the implications for acute management of patients with compound depressed fractures.

Just over 60 years ago, Dart's description and analysis of the Taung child's skull triggered an intellectual revolution about human origins. Recently, several authors have suggested that one of the most significant hominid-like traits of australopithecines, delayed maturation, may not after all be valid. This is a radical departure from Mann's classic study of australopithecine maturation and palaeodemography based on dental eruption patterns. The resolution of this debate has important implications for the history of the biological and social evolution of the human species. In view of the con…

Previous investigations on calvarial fractures in infants have shown that the flexibility and displacement of the infant calvarial are not sufficient to avoid fractures as a result of fall. From a table height onto hard ground - and in special cases, fractures cannot be avoided even after falls onto softly cushioned ground. The skull fractures are located in paper-thin, transparent, single-layer bone areas without diploe. The results of previous literature were compared with investigations of the skulls of 82 infants (from neonates up to infants 14 months of age). Congenital fissures, craniosc…

Mucoceles of the sphenoid sinus are rare. Only occasional reports have appeared in the medical literature since they were first described by Berg in 1889. The sphenoid sinus lies deep within the skull at the crossroads of 13 sensitive structures. The presence of a sphenoid sinus mucocele may be accompanied by a wide variety of signs and symptoms depending upon which of these structures are involved. The majority of these lesions are initially misdiagnosed as malignancies leading to increased patient morbidity and possible mismanagement. We present two cases of sphenoid sinus mucoceles which pr…

Although among Anglo-Saxons Paget's disease is relatively common, it is very rare in Japan. The incidence of Paget's disease increases as age advances, and in elderly people over 90 years of age it can be seen in as many as 10%. On the other hand, acute epidural hematoma seldom occurs in the older generation, because dural adhesion to the internal table of the skull becomes more intense. We have encountered an 86-year old woman with advanced Paget's disease who developed an acute epidural hematoma after trivial head trauma. Acute epidural hematoma complicating Paget's disease has been reported…
