The Impact of Non-Invasive Ventilation on Sleep Quality in COPD Patients.

BACKGROUND: Non-invasive ventilation (NIV) has been shown to be the most appropriate therapy for COPD patients with chronic respiratory failure. While physiological parameters and long-term outcome frequently serve as primary outcomes, very few studies have primarily addressed the impact of NIV initiation on sleep quality in COPD. METHODS: This single-center prospective cohort study comprised NIV-naïve patients with COPD. All patients underwent polysomnographic evaluation both at baseline and at 3 months follow-up, accompanied by the assessment of health-related quality of life (HRQL) using t

Sleep profile during fasting in PPAR-alpha knockout mice.

Peroxisome proliferator-activated receptor alpha (PPARα) is a transcription factor that belongs to the nuclear receptor family and plays an important role in regulating gene expression associated with lipid metabolism. PPARα promotes hepatic fatty acid oxidation and ketogenesis in response to fasting. Because energy metabolism is known to affect sleep regulation, manipulations that change PPARα are likely to affect sleep and other physiological phenotypes. In this study, we examined the role of PPARα in sleep/wake regulation using PPARα knockout (KO) mice. Sleep, body temperature (BT), locomot

Zopiclone, the third generation hypnotic: a clinical overview.

Effective and safe hypnotics exist especially since the introduction of benzodiazepines (BZD) which appeared to bring major advantages over barbiturates. Ideally a new hypnotic should induce and maintain sleep without producing residual effect during the day and should be devoid of abuse and dependence potential. Zopiclone is a new hypnotic belonging to the cyclopyrrolone chemical class. Its elimination half-life is 5 to 6 h, no accumulation exists upon repeated administration, and its pharmacokinetic profile is not substantially modified in elderly and renal failure patients. Placebo-controll

Partial cortical deafferentation promotes development of paroxysmal activity.

This study tested the hypothesis that early functional alterations in neuronal synchrony in the partially deafferented cortex may lead to spontaneously occurring electrographic seizures. In vivo experiments with partial deafferentation of cat suprasylvian gyrus after extensive undercut of the white matter were conducted using multi-site EEG, extracellular unit and intracellular recordings. The amplitudes of EEG waves were much higher in the areas surrounding deafferented cortical fields as compared with control and with undercut cortex. In 40% of animals with undercut cortex, paroxysmal activi

Sleep and daytime sleepiness in upper airway resistance syndrome compared to obstructive sleep apnoea syndrome.

This study has investigated differences in the nocturnal sleep and daytime sleepiness among patients with obstructive sleep apnoea syndrome (OSAS), upper airway resistance (UARS), sleep hypopnoea syndrome, and normal control subjects, using sleep scoring and spectral activity analysis of the electroencephalogram (EEG). Twelve nonobese males with UARS aged 30-60 yrs were recruited. These subjects were strictly matched for age and body mass index with twelve OSAS patients, 12 sleep hypopnoea syndrome patients, and 12 normal controls, all male. Daytime sleepiness was evaluated using the Epworth S

Visual agnosia in a child with non-lesional occipito-temporal CSWS.

In this paper we describe a case of severe visual agnosia in a child with an electrophysiological pattern of continuous spike-wave discharges in slow sleep (CSWS) in the occipito-temporal regions. The neuropsychological spectrum related to this phenomenon is discussed. Published paediatric reports associate visual agnosia (i.e. an inability to recognize objects without impairment of visual acuity) mainly with symptomatic occipito-temporal aetiology (e.g. cortical dysplasia, vascular insults) and other neurological symptoms (e.g. autism). We describe a detailed 2 year electrophysiological and n

Sleep endocrine effects of antigluco- and antimineralocorticoids in healthy males.

In several mammalian species the responsiveness of brain neurons to corticosteroids is mediated by mineralo- (MR) and glucocorticoid (GR) receptors. These receptors play a key role not only in the endocrine adaptation to stress but also in corticosteroid-induced changes of behavior, including sleep. We further explored the specific physiological role of this binary receptor system in the human brain by studying electroencephalogram (EEG) sleep and changes in plasma concentrations of adrenocorticotropic hormone (ACTH), cortisol, and growth hormone from 1800 to 0700 h in a series of investigatio

Melatonin improves sleep in children with epilepsy: a randomized, double-blind, crossover study.

OBJECTIVE: Insomnia, especially maintenance insomnia, is widely prevalent in epilepsy. Although melatonin is commonly used, limited data address its efficacy. We performed a randomized, double-blind, placebo-controlled, crossover study to identify the effects of melatonin on sleep and seizure control in children with epilepsy. METHODS: Eleven prepubertal, developmentally normal children aged 6-11 years with epilepsy were randomized by a software algorithm to receive placebo or a 9-mg sustained release (SR) melatonin formulation for four weeks, followed by a one-week washout and a four-week cr

Electrocorticographic, astrocytic and transcriptomic signatures in the triple transgenic mouse model of Alzheimer's disease submitted to stearoyl-CoA desaturase inhibition.

Alzheimer's disease (AD) is associated with cognitive deficits and sleep disturbances. Research suggests the involvement of dysfunctions in lipid metabolism in the brain of AD patients and animal models. The inhibition of stearoyl-CoA desaturase (SCD), a lipid-converting enzyme, was shown to restore memory in triple transgenic (3xTg)-AD mice. In the brain, astrocytes regulate the synthesis of specific lipids. This project tested whether the inhibition of SCD restores sleep in 3xTg-AD mice, and whether this associates with modifications in lipids, astrocytic function and the transcriptome. Wild

The Effect of a Post-Learning Nap on Motor Memory Consolidation in People With Parkinson's Disease: A Randomised Controlled Trial.

Motor memory consolidation is a process by which newly acquired skills become stable over time in the absence of practice. Sleep facilitates consolidation, yet it remains unknown whether sleep-dependent consolidation is intact in people with Parkinson's disease. Here, we investigated whether a post-learning nap-as compared to wakefulness-improves motor memory consolidation in Parkinson. Thirty-two people with Parkinson's disease and 32 healthy older adults learnt a finger-tapping sequence task before a nap or wake intervention (pseudo-randomised assignment). Consolidation was measured as the c

Effects of nicotine on sleep architecture and ventilatory parameters: Results of the Tab-OSA cross-sectional observational study.

To investigate nicotine's effects on sleep, this cross-sectional observational study compared polysomnography (PSG) sleep architecture and ventilatory parameters in 160 participants divided into active smokers (AS), former smokers (FS), non-smokers (NS) and nicotine replacement therapy (NRT) users beyond post-cessation nicotine withdrawal, while controlling for psychiatric and other medical comorbidities, treatment and substance use. The duration of N3, or deep slow-wave-sleep (SWS), usually lasting approximately 90 min, was reduced in NRT users and AS, but not FS, suggesting that smoking is a

Late feeding in the active period decreases slow-wave activity.

AIMS: Sleep and feeding behaviors closely interact to maintain energy homeostasis. While it is known that sleep disorders can lead to various metabolic issues such as insulin resistance, the mechanism for this effect is poorly understood. We thus investigated whether different feeding rhythms during the active period affect sleep-wake regulation. MAIN METHODS: For 2weeks, mice were randomly assigned to 1 of 3 feeding schedules as follows: free access to lab chow during the active period (ZT12-24, Ad-lib group), free access to lab chow during the first half of the active period (ZT12-18; Morni

Cortical processing of respiratory occlusion stimuli in children with central hypoventilation syndrome.

RATIONALE: The ability of patients with central hypoventilation syndrome (CHS) to produce and process mechanoreceptor signals is unknown. OBJECTIVES: Children with CHS hypoventilate during sleep, although they generally breathe adequately during wakefulness. Previous studies suggest that they have compromised central integration of afferent stimuli, rather than abnormal sensors or receptors. Cortical integration of afferent mechanical stimuli caused by respiratory loading or upper airway occlusion can be tested by measuring respiratory-related evoked potentials (RREPs). We hypothesized that p

Effects of nasal airflow on breathing during sleep in normal humans.

The nasal airway is a common route for oxygen delivery to hypoxemic patients, and it has been advocated as a suitable route for intermittent positive pressure ventilation to patients in respiratory failure. There is, however, conflicting evidence on the question of whether nasal airflow has a stimulant or a depressant effect on ventilation. We therefore studied the effects on ventilation of increasing and decreasing nasal airflow during sleep, thereby avoiding the voluntary and behavioral influences on breathing seen during wakefulness. After an acclimatization night, each of nine normal male

Encapsulated Leptin-Producing Cells Facilitate Entrainment of Circadian Rhythms in Rodents and Nonhuman Primates.

Both the timing and composition of food intake have gained recognition for their effects on circadian (daily) rhythms in mammals, however the mechanisms by which these effects are exerted are incompletely understood. The present studies focus on synchronization of circadian rhythms to the environmental light-dark cycle, particularly re-entrainment following a phase shift such as occurs with travel across time zones or shift work. To examine the role and therapeutic potential of the peptide hormone leptin in these effects, we create and characterize injectable leptin factories consisting of enc

Widespread reduction in sleep spindle activity in socially anxious children and adolescents.

Social anxiety disorder (SAD) is one of the most prevalent psychiatric diseases typically emerging during childhood and adolescence. Biological vulnerabilities such as a protracted maturation of prefrontal cortex functioning together with heightened reactivity of the limbic system leading to increased emotional reactivity are discussed as factors contributing to the emergence and maintenance of SAD. Sleep slow wave activity (SWA, 0.75-4.5 Hz) and sleep spindle activity (9-16 Hz) reflect processes of brain maturation and emotion regulation. We used high-density electroencephalography to charact

Effect of subchronic inhalation exposure to low-level trichloroethylene on heart rate and wakefulness-sleep in freely moving rats.

Electroencephalographic (EEG) and heart rate (HR) responses to repeated 8-h exposure for 6 weeks to trichloroethylene (TRI) vapor of 50, 100 and 300 ppm were examined in freely moving rats with chronically implanted electrodes for polygraphic recordings. Exposure to all the TRI levels significantly decreased amount of time spent in wakefulness (W) during the exposure period. Exposure to 100 ppm and higher levels significantly decreased the time-averaged HR during the post-exposure period. The HRs during the stages of slow-wave sleep (SWS) and paradoxical sleep (PS) were significantly lowered a

Hypoxic arousal in intact and carotid chemodenervated sleeping cats.

To determine whether the carotid chemoreceptors or hyperpnea are required for arousal from sleep by hypoxia, 14 sleep-deprived cats were studied during slow-wave (SWS) and rapid-eye-movement (REM) sleep. Rapid hypoxia was produced by inhalation of 5% O2 in N2 or 6% CO in 40% O2 by intact cats and 5% O2 in N2 after carotid body denervation. Preliminary studies identified a period of SWS unassociated with spontaneous arousals. In 69 studies during SWS unassociated with spontaneous arousals, arterial O2 saturation (SaO2) values at arousal were: 47.1 +/- 1.5% (mean +/- SE) (5% O2, intact); 48.9 +/

Seizures influence sleep macrostructure and the sleep-wake circadian rhythm in Dravet syndrome.

OBJECTIVE: Dravet syndrome (DS) is a developmental and epileptic encephalopathy with a wide spectrum of comorbidities comprising sleep disorders, reported in up to 85% of cases. For this, a sleep study is recommended in patients with a sleep complaint. However, no data are available on sleep architecture in DS or on the impact of seizures on sleep quality and macrostructure. We aim to investigate the impact of epileptic phenomena on sleep in DS. METHODS: In this study, we report seizure type and frequency, seizures during sleep, and concomitant antiseizure medications (ASMs) of 30 patients wi
