Laser midline glossectomy as a treatment for obstructive sleep apnea.

Multiple site specific procedures have been proposed to treat obstructive sleep apnea syndrome (OSAS). Midline glossectomy (MLG) is a procedure that directly enlarges the hypopharyngeal airspace using the carbon dioxide laser. The initial experience of 12 patients is presented. Midline glossectomy as the sole procedure was performed on 11 patients who had failed uvulopalatopharyngoplasty (UPPP) and who were felt to have significant hypopharyngeal collapse on physical examination and Müller's maneuver. One patient with primary hypopharyngeal narrowing underwent MLG. Five (42%) were considered r

Symptoms in heavy snorers with and without obstructive sleep apnea.

Five hundred and eighty persons who were heavy snorers filled in a questionnaire regarding symptoms on a 5-grade scale. Of these, 178 had a complete polysomnography investigation while 402 patients underwent oxymetric screening during the night only. On the basis of these investigations. 217 were classified as suffering from the obstructive sleep apnea syndrome (OSAS) and 363 as snorers without OSAS. The symptom scores differed between the two groups, but the range was wide and some persons with OSAS claimed only minor daytime sleepiness, somnolence, etc., while a high proportion of persons wi

Obstructive sleep apnea following topical oropharyngeal anesthesia in loud snorers.

Previous studies support the presence of an upper airway reflex mechanism that contributes to the maintenance of upper airway patency during sleep. We investigated the possibility that interference with this reflex mechanism contributes to the development of obstructive sleep apnea. Eight otherwise asymptomatic snorers (seven male and one female), age 39 +/- 5.3 yr (mean +/- SEM), underwent overnight sleep studies on three successive nights. An acclimatization night was followed by two study nights randomly assigned to control (C) and oropharyngeal anesthesia (OPA). On the OPA night topical an

[Treatment of obstructive sleep apnea with methods of orthognathic surgery].

The only surgical means available up to now for treating obstructive sleep apnea syndrome (OSAS) consisted in uvulopalatopharyngoplasty (UPPP), which failed to produce satisfactory results. The ultima ratio frequently resorted to was tracheotomy with all its medical and social drawbacks. Complete advancement of the maxillomandibular complex opens up new avenues for the treatment of upper airways obstruction. The present report is on 13 patients operated for polysomnographically established OSA. The maxillomandibular complex was brought as far forward as possible and esthetically justifiable. T

Naso- and oropharyngeal dimensions in children with obstructive sleep apnea.

Sixty children (3-11 years) were evaluated to determine variations in naso- and oropharyngeal dimensions associated with tonsil and adenoid hypertrophy. The subjects were grouped according to tonsil size and a clinical history of chronic upper airway obstruction. Intraoperative measurements included oropharyngeal diameter, length of the hard and soft palates, width and arch of the hard palate, nasopharyngeal volume, as well as tonsil and adenoid weights and volumes. A significantly larger oropharyngeal diameter was found in children with small, non-obstructing tonsils (P less than 0.01). Child

Paradoxical glottic narrowing in patients with severe obstructive sleep apnea.

Most patients with obstructive sleep apnea have increased pharyngeal collapsibility (defined in the present study as an increased lung volume dependence of pharyngeal area), which predisposes them to upper airway occlusion during sleep. However, there are patients with severe obstructive sleep apnea who have low-normal pharyngeal collapsibility. The factors leading to nocturnal upper airway obstruction in such patients have not been ascertained. We studied 10 overweight male patients with severe obstructive sleep apnea and low-normal pharyngeal collapsibility to determine the site of upper air

Cephalometric analysis for diagnosis and treatment of obstructive sleep apnea.

A detailed cephalometric analysis was conducted on lateral x-rays from 30 adult patients with obstructive sleep apnea (OSA) and 12 age- and sex-matched controls. Statistical findings show that OSA patients are different from controls in at least five ways: 1. Their tongue and soft palate are significantly enlarged. 2. The hyoid bone is displaced inferiorly. 3. The mandible is normal in size and position (no micrognathia or malocclusion), but the face is elongated by an inferior displacement of the mandibular body. 4. The maxilla is retropositioned and the hard palate elongated. 5. The nasophar

Ventricular dysfunction in children with obstructive sleep apnea: radionuclide assessment.

Ventricular function was evaluated using radionuclide ventriculography in 27 children with oropharyngeal obstruction and clinical features of obstructive sleep apnea. Their mean age was 3.5 years (9 months to 7.5 years). Conventional clinical assessment did not detect cardiac involvement in 25 of 27 children; however, reduced right ventricular ejection fraction (less than 35%) was found in 10 (37%) patients (mean: 19.5 +/- 2.3% SE, range: 8-28%). In 18 patients wall motion abnormality was detected. In 11 children in whom radionuclide ventriculography was performed before and after adenotonsill

Massive oropharyngeal papillomatosis causing obstructive sleep apnea in a child.

Obstructive sleep apnea in children is most often secondary to severe adenotonsillar hypertrophy. A 5 1/2-year-old boy presented with loud snoring, increasing dysphagia, nocturnal choking, and apnea. Extensive papillomatosis of the uvula, soft palate, and nasopharynx was found to be causing the obstruction. Although multiple papillomas of the larynx and tracheobronchial tree are well known, the occurrence of extensive papillomas of the oral cavity is rare. The unusual clinical presentation and pathologic significance are discussed.

Severe obstructive sleep apnea--I: Onset, clinical course, and characteristics.

The clinical course and characteristics of severe obstructive sleep apnea are described for 50 adults whose condition warranted recommendation for tracheostomy. All patients had a history of snoring, excessive daytime sleepiness and sleep attacks, nocturnal snorting and gasping sounds and observer-noted nocturnal breath cessations. Generally, these symptoms became manifest before age 40, their appearance tended to cluster together within only a few years and, invariably, they were chronic. Aside from snoring, excessive daytime sleepiness was on average often the first symptom and began at a me

Obstructive sleep apnea in association with posterior fossa neurologic disease.

Sleep apnea in association with neurologic disease is uncommon, and its documentation with nocturnal polysomnography (PSG) is rare. Two patients with posterior fossa neurologic disease had obstructive and central sleep apnea on PSG. The first was a 40-year-old woman who experienced a respiratory arrest ten days after neurosurgical decompression of a cervical syrinx associated with syringobulbia. Nocturnal PSG demonstrated obstructive sleep apnea. Tracheostomy was performed and a second nocturnal PSG showed no sleep apnea. The second patient was a 76-year-old woman with daytime hypersomnolence,

Reliability of Home Sleep Apnea Test as a Diagnostic Modality in Children With Obstructive Sleep Apnea- a Systematic Review.

BACKGROUND: The incidence of pediatric sleep disordered breathing has increased in the last decade because of many factors, such as childhood obesity, metabolic syndrome, and access to better diagnostic methods. OSA peaks between 2 and 8 years in children. Early diagnosis is recommended to prevent long-term complications. While PSG is the gold standard for diagnosis, it may not be feasible in certain circumstances; hence, HSAT is now being increasingly used to screen and diagnose OSA. METHODS: This review was conducted using the methodology in the "Cochrane handbook on systematic reviews". PR

Relationship between T90 and systemic hypertension in patients with obstructive sleep apnea: Data from the European sleep apnea database.

BACKGROUND: Hypertension (HTN) is one of the most prevalent cardiovascular comorbidities in patients with obstructive sleep apnoea (OSA). Although OSA severity measured by the apnoea-hypopnoea index (AHI) is a known risk factor, the role of hypoxic load in HTN remains less well established. METHODS: We analysed data from 12,141 patients with OSA enrolled in the European Sleep Apnea Database (ESADA). Clinical and sleep study parameters were assessed to identify factors associated with HTN. RESULTS: The cohort included 2650 patients with mild OSA, 3339 with moderate OSA, and 6152 with severe O

The Effect of Tonsillectomy on Sleep Architecture in Pediatric Patients With Obstructive Sleep Apnea.

OBJECTIVES: To evaluate the impact of pediatric adenotonsillectomy (T&A) on sleep architecture, related sleep factors, and respiratory parameters in children with obstructive sleep apnea (OSA), stratified by disease severity. METHODS: We analyzed 300 consecutive children who underwent full-night polysomnography (PSG) before and after T&A between January 2021 and March 2022. Patients were stratified into severe (apnea-hypopnea index [AHI] > 10 events/h or SpO2  < 80%) and very severe (AHI > 30 events/h) OSA groups. Correlations were assessed using Spearman analysis. RESULTS: The cohort includ

Tube Law of the Pharyngeal Airway in Sleeping Patients with Obstructive Sleep Apnea.

STUDY OBJECTIVES: Obstructive sleep apnea (OSA) is characterized by repetitive pharyngeal collapse during sleep. However, the dynamics of pharyngeal narrowing and re-expansion during flow-limited breathing are not well described. The static pharyngeal tube law (end-expiratory area versus luminal pressure) has demonstrated increasing pharyngeal compliance as luminal pressure decreases, indicating that the airway would be sucked closed with sufficient inspiratory effort. On the contrary, the airway is rarely sucked closed during inspiratory flow limitation, suggesting that the airway is getting

Nocturnal body position in sleeping children with and without obstructive sleep apnea.

To assess whether body position during sleep differs among children with obstructive sleep apnea (OSAS) and controls, and to assess the effects of body position, obesity, and tonsillar size on respiratory disturbance. Four hundred and thirty consecutive children with polysomnographically demonstrated OSAS. And 185 age-, gender-, and ethnically matched children (Controls) were compared. The effect of sleep body position on respiratory disturbance was examined in OSAS, and also in relation to obesity and tonsillar size. Children with OSAS spent more time in the supine position than Controls (P<0
