Surgical outcomes for velopharyngeal insufficiency in velocardiofacial syndrome and nonsyndromic patients.

OBJECTIVE: To compare speech outcomes after operative intervention for velopharyngeal insufficiency between velocardiofacial syndrome patien … n was conducted by a speech pathologist. Assessment parameters scored on a numerical scale included speech intelligibility, resonance, nasal air emissions, and overall severity of velopharyngeal insufficiency. Postoperative complications were recorded. RESULTS: Most velocardiofacial syndrome patients and nonsyndromic patients demonstrated significant improvements in al …

Comparison of two forms of intensive speech treatment for Parkinson disease.

This study investigated the effect of two forms of intensive speech treatment, (a) respiration (R) and (b) voice and respiration (Lee Silverman Voice Treatment [LSVT]), on the speech and voice deficits associated with idiopathic Parkinson disease. Forty-five subjects with Idiopathic Parkinson disease completed extensive pretreatment neurological, otolaryngological, neuropsychological, and speech assessments. All subjects completed 16 sessions of intensive speech treatment, 4 times a week for 1 month. Pre- and post-treatment measures included intensity and maximum duration during sustained vowe

Can AI make people happy? The effect of AI-based chatbot on smile and speech in Parkinson's disease.

INTRODUCTION: Approaches for objectively measuring facial expressions and speech may enhance clinical and research evaluation in telemedicine, which is widely employed for Parkinson's disease (PD). This study aimed to assess the feasibility and efficacy of using an artificial intelligence-based chatbot to improve smile and speech in PD. Further, we explored the potential predictive value of objective face and speech parameters for motor symptoms, cognition, and mood. METHODS: In this open-label randomized study, we collected a series of face and conversational speech samples from 20 participa

Effectiveness of the Superiorly Based Pharyngeal Flap in Treating Velopharyngeal Insufficiency.

UNLABELLED: Velopharyngeal insufficiency (VPI) is a condition characterized by failure of the posterior part of the soft palate to reach the … disorders were present in 91.4% of patients before surgery, and decreased to 71.4% postoperatively. Speech intelligibility improved postoperatively in comparison with preoperative findings. CONCLUSION: The present study concluded that the superiorly based pharyngeal flap was successful in treating VPI that persisted post cleft palate repair.

Optimising hearing aid output to paediatric prescriptive targets improves outcomes in children.

OBJECTIVE: This study examined the accuracy of hearing aid output to DSL v5.0 Child targets in a group of children who wear hearing aids, and the impact on their auditory outcomes. DESIGN: For each participant, the output of the initial hearing aid fitting was compared to DSL v5.0 Child prescriptive targets and again after the fitting was adjusted using coupler-based verification and RECD measures. Outcomes for initial and adjusted fittings were examined using the Speech Intelligibility Index (SII), Parent's Evaluation of Aural/Oral Performance of Children (PEACH) rating scale, and speech per…

Hearing.

The main hazard for hearing in the workplace is noise. Organic solvents and heavy metals may increase the danger of developing occupational hearing loss, particularly in the case of co-exposure with noise. While noise produces damage predominantly to the cochlea, chemicals may be responsible for pathologic changes in both peripheral and central parts of the auditory pathway. Noise-induced hearing loss develops slowly over the years, although its progression is most dynamic during the first 10-15 years of exposure. Pure-tone audiometry indicates a bilateral sensorineural hearing loss, affecting

Articulation and oromyofunctional behavior in children seeking orthodontic treatment.

OBJECTIVES: The purpose of this controlled study is to document articulation and oromyofunctional behavior in children seeking orthodontic treatment. In addition, relations between malocclusions, articulation, and oromyofunctional behavior are studied. MATERIALS AND METHODS: The study included 56 children seeking orthodontic treatment. The control group, consisting of 54 subjects matched for age and gender, did not undergo orthodontic intervention. To determine the impact of the occlusion on speech, the Oral Health Impact Profile was used. Speech characteristics, intelligibility and several l

Making non-fluent aphasics speak: sing along!

A classic observation in neurology is that aphasics can sing words they cannot pronounce otherwise. To further assess this claim, we investigated the production of sung and spoken utterances in eight brain-damaged patients suffering from a variety of speech disorders as a consequence of a left-hemisphere lesion. In Experiment 1, the patients were tested in the repetition and recall of words and notes of familiar material. Lyrics of familiar songs, as well as words of proverbs and prayers, were not better pronounced in singing than in speaking. Notes were better produced than words. In Experime

Acquired dysarthria in conversation: methods of resolving understandability problems.

BACKGROUND: People with acquired progressive dysarthria typically experience increased problems with intelligibility in everyday conversation as their disease progresses. Such problems are likely to impact on both the person with dysarthria and those with whom they interact. If this is the case then we may ask questions not just about the nature of these problems but how it is that such problems are dealt with by participants when they occur. AIMS: To investigate ways through which problems resulting from dysarthria in everyday conversation are resolved by participants. Further, to examine so

Comparisons of longitudinal speech outcomes in children born with cleft palate ± cleft lip with and without additional conditions at 5 and 10 years -a registry study.

BACKGROUND: Speech outcome differences in children with cleft palate ± cleft lip (CP ± L) with ( +) and without (-) additional conditions (i.e., diagnosed syndromes, additional malformations, and intellectual disabilities) are not fully explored. The Swedish cleft lip and palate (CLP) registry enables a longitudinal approach. Thus, the purpose was to compare longitudinal speech outcomes in children with different cleft types, with and without additional conditions, at ages 5 and 10 years, based on registry data. METHODS: Included were all children with oral clefts involving the palate born in

On the importance of interaural noise coherence and the medial olivocochlear reflex for binaural unmasking in free-field listening.

For speech in competition with a noise source in the free field, normal-hearing (NH) listeners recognize speech better when listening binaurally than when listening monaurally with the ear that has the better acoustic signal-to-noise ratio (SNR). This benefit from listening binaurally is known as binaural unmasking and indicates that the brain combines information from the two ears to improve intelligibility. Here, we address three questions pertaining to binaural unmasking for NH listeners. First, we investigate if binaural unmasking results from combining the speech and/or the noise from the

Adult Quality of Life Post Cleft Palate Repair: A Comparison of Two Techniques.

BACKGROUND AND PURPOSE: In 1989, the Cleft Palate-Craniofacial Journal published the first randomized prospective cleft surgery study, comparing the Kriens intravelar veloplasty (IVV) with a non-IVV 2-flap repair. Results in that and follow-up publications yielded no difference between the 2 groups for need for secondary velopharyngeal management. The subjects have now reached adulthood. This study was designed to ask: Is there any difference between the groups in the outcomes that multidisciplinary team care addresses: speech intelligibility, facial growth, breathing while awake and asleep, a…

Precision Pharyngeal Flap: An Individualized, Patient-Specific Surgery for the Treatment of Velopharyngeal Insufficiency.

BACKGROUND: The pharyngeal flap is one of the most common secondary surgeries for the correction of velopharyngeal insufficiency (VPI) for patients with cleft palate and/or cleft lip and palate. This study aimed to demonstrate the effectiveness of the precision pharyngeal flap surgery performed by the senior author. MATERIALS AND METHODS: Children with VPI, who underwent precision pharyngeal flap, were retrospectively examined. All surgical procedures were performed by the senior author. The flap size was individually configured based on the patients' preoperative nasopharyngoscopic analysis
