[Case of infectious spondylitis presenting as exudative pleural effusion].

A 59-year-old woman with lumbago, presented with cough and right chest pain. Her chest X-ray showed right pleural effusion, and laboratory s … ed an increased signal intensity at the level of T11-12, thus suggesting a disk space infection and spondylitis with an epidural abscess. Thereafter, she developed left pleural effusion, and the effusion was drained. Her infection was cured with long-term administration of antibiotics. However, the infectious spondylitis relapsed after four months, and she th …

Association of interleukin-23 receptor variants with ankylosing spondylitis.

OBJECTIVE: Recent studies have shown that a nonsynonymous single-nucleotide polymorphism (SNP) (Arg381Gln; rs11209026) in the interleukin-23 receptor (IL-23R) gene on chromosome 1p31 is associated with Crohn's disease and psoriasis. Given the clinical and immunologic overlap between ankylosing spondylitis (AS) and these diseases, and the potential function of this candidate SNP, this study was undertaken to examine the association of IL-23R variants with AS in multiple Canadian populations. METHODS: We examined 3 cohorts of AS patients from established rheumatic disease centers in Canada. The…

Treatment of ankylosing spondylitis with disease modifying antirheumatic drugs.

Ankylosing spondylitis (AS) is a common (prevalence 0.2-0.9%) chronic inflammatory disease that mainly affects young males and is characterised by inflammatory back pain with sacroiliitis and often arthritis of the peripheral joints. The disease can lead to deformities of the vertebral column, joints and extra-spinal structures, e.g. the eye (uveitis). Non-steroidal anti-inflammatory drugs (NSAIDs) and physical therapy seem to improve the long-term outcome of AS. However, the effect of disease modifying antirheumatic drugs (DMARDs) is less impressive compared with other rheumatic diseases, suc…

[Diagnosis of cervical spondylitis disease: An evidence based review].

INTRODUCTION: The diagnosis of cervical spondylitis (CS) is based on three factors: clinical, neuroradiological and neurophysiological data. However, in spite of being universally used, the validity of each of these diagnostic instruments has still to be clarified. The objective of our study was to make a critical analysis of the evidence currently available with regard to the diagnosis of CS. DEVELOPMENT: Review of the literature from 1967 to March 2001, quoted by MEDLINE, regarding the diagnosis of CS. The articles obtained were classified into four groups: Metanalysis or systematic reviews…

[Tuberculous spondylitis complicated by spinal disorders: diagnosis and treatment].

Complex diagnosis using additional radiation procedures (computed tomography and magnetic resonance imaging) for tuberculous spondylitis complicated by spinal disorders was made in 52 patients. It made the genesis of spinal cord disorders more clear. The genesis was due to epidural abscess, necrotic osseous tissue and bony compression of destroyed vertebrae. Spinal cord structural changes were detectable in the postoperative period. Hemosorption in combination with blood ultraviolet radiation was used in 25 (40%) patients for their indications, which allowed all the patients to be operated on.…

Cystic rheumatoid arthritis with Felty's syndrome and ankylosing spondylitis.

A 63-year-old man with strictly axial ankylosing spondylitis since the age of 28 years had a seven-year history of cystic seronegative rheumatoid arthritis with Felty's syndrome. Cysts were present in the hands, feet, wrists, shoulders, hips, one elbow, and one knee. There was no evidence of juxtaarticular demineralization, joint space loss, erosions, or joint destruction. Rheumatoid pannus was demonstrated within the cysts, particularly at the hip, ruling out cystic hip disease due to ankylosing spondylitis. HLA typing demonstrated the B27 and DR4 haplotypes. HLA B27 may be associated with a …

Lumboperitoneal shunt for cauda equina syndrome in ankylosing spondylitis.

Cauda equina syndrome is a rare complication in the late stage of ankylosing spondylitis, for which approximately 60 cases have been reported in the literature. The cause of the syndrome is unclear, and there is no effective treatment. Recently lumboperitoneal shunt was reported to have been effective in two patients. In our study, we performed lumboperitoneal shunt in a patient and evaluated the condition after the operation compared with that preoperatively. Some alleviation of neurologic symptoms was observed for 6 months after operation. Histopathologic examination of the dural diverticulu…

An atypical psoriatic spondylitis case, successfully treated with methotrexate.

We present a 45-year-old male patient who was hospitalized with lumbar disc herniation and whose control magnetic resonance imaging (MRI) findings initially suggested brucella spondylitis. Definitive diagnosis, however, indicated psoriatic spondylitis and the patient was successfully treated with methotrexate. A diagnosis of lumbar disc herniation was made in May 1991, during his psoriasis vulgaris treatment. He was hospitalized in August 1994 with a complaint of low-back pain persisting over the last six months despite treatment with analgesics. He was evaluated by clinical, radiological, lab…

Gallium-67 scintigraphy in tuberculous and nontuberculous infectious spondylitis.

Vertebral osteomyelitis is a difficult condition to diagnose clinically. A retrospective review of 21 cases of infectious spondylitis (6 tuberculous, 15 nontuberculous) confirmed the utility of 67Ga scintigraphy in imaging this process. Gallium-67 scans were positive at all sites of disease in this patient population. They anticipated the presence of spondylar infection in 23% of regions where radiographic abnormalities were originally lacking, while confirming a current and ongoing septic process at the other sites where vertebral destruction was already present on x-ray. Gallium-67 studies, …
