Cerebrovascular events are the most common neurological complications seen in renal transplant recipients. Cerebral infarction and transient ischemic attacks are the most common events and may occur years after transplantation. Recipients older than 40 years at the time of transplantation and those with diabetes mellitus are at greater risk. No instances of aneurysmal subarachnoid hemorrhage occurred among 31 patients with polycystic kidney disease who had undergone transplantation.

Acquired hemiplegia in childhood is uncommon. All causes of stroke in adults should be considered in children, including atherosclerotic cerebrovascular disease even though it is rare. In addition, children suffer from many conditions that may result in stroke. Unlike adults, a cardiac illness resulting in stroke is extremely unusual in an otherwise healthy child. We report a case of a four-year-old child; he presented with the sudden onset of a right hemiplegia associated with supraventricular tachycardia. Cerebral angiography indicated a left internal carotid artery occlusion. To our knowled…

Acquired dysgraphia has been described as a disorder of graphemic selection and spatial temporal disorganization which can exist in isolation or as a component of a broader language or cognitive syndrome. There is little agreement on the locus of writing centers, although select areas within the left hemisphere have been suggested. We describe a patient who had dysgraphia after a right hemispheric stroke. He had no demonstrable signs of limb apraxia or visual field deficit, and only subtle signs of language impairment other than the writing disturbance. Treatment emphasized progressively more …

An innovative approach to the rehabilitation of the stroke patient was initiated at Riverside Methodist Hospital to accommodate an increasing number of patients. Patients spend six hours a day several days a week attending a day rehabilitation program at which all necessary therapies are provided. A year-end analysis of the program substantiates the original hypothesis that stroke patients would show significant improvement with treatment in a specialized day rehabilitation facility.

The possibility that acute stroke produces an increase in sympathetic tone with resultant cardiac abnormalities was examined in 100 stroke patients admitted to a stroke ICU and in 50 controls found to have diagnoses other than stroke or TIA after admission to the Unit. Continuous 24 hour Holter ECG tapings were performed and serum cardiac enzymes and plasma norepinephrine concentrations were measured within 48 hours after admission. Significantly, (p less than .001) more serious arrhythmias were observed during 24 hour Holter ECG monitoring in stroke patients compared with controls and the dif…

A vascular surgeon must recognize that strokes may be caused by embolism from or obstruction of the extracranial afferent arteries to the brain. Furthermore, embolic disease and the hypoperfusion syndrome may occur in the same patient. A transient ischemia attack (TIA) is diagnostic of localized cerebral ischemia. A bruit has little significance other than as an indication for further study. The most practical noninvasive method of studying patients for cerebrovascular disease in our laboratory has been oculopneumoplethysmography and B-mode scanning. Interobserver variations in interpretation …

In a three-year study, the clinical course and results of intracranial angiography were compared in patients having an acute stroke in the carotid artery territory combined with angiographic abnormalities indicating severe extracranial carotid stenosis or occlusion. Two major mechanisms of stroke were delineated. In one group, the angiographic intracranial abnormalities strongly suggested the presence of embolism in the cerebral vessels supplied by the stenotic or occluded carotid artery; many of these patients had no obvious transient ischemic attacks prior to their stroke and experienced a m…

Cardiac arrhythmias were more frequent (p less than 0.001) in 312 stroke patients admitted to an intensive care stroke unit, than in 92 patients admitted to the unit and subsequently found not to have strokes. This significant difference remained when a stroke subgroup and the non-stroke group were matched for age, sex and duration of stay in the unit (P less than 0.005). Hypertension and hypertensive cardiac disease were more common in the stroke than in the non-stroke patients (P less than 0.001). Ectopic beats and atrial fibrillation, as well as other arrhythmias, were most frequent in pati…
