Defective expression of prohormone convertase 4 and enhanced expression of insulin-like growth factor II by pleural solitary fibrous tumor causing hypoglycemia.

A 75-year-old man was admitted to our hospital because of unconsciousness. His plasma glucose was very low, but his serum levels of insulin and IGF-I were also low. He was found to have a giant solitary pleural tumor, which was completely resected, after which his hypoglycemia ameliorated postoperatively. Histologically, the tumor was consistent with the pathological diagnosis of a solitary fibrous tumor derived from the pleura. Immunohistochemical study revealed positive immunostaining for IGF-II in tumor cells. The presence of high molecular weight (HMW) form of IGF-II in the tumor tissue an

Cerebellar hemorrhage provoked by combined use of nattokinase and aspirin in a patient with cerebral microbleeds.

Nattokinase is used as a health-promoting medicine for preventing thrombosis due to its fibrinolytic activity. Cerebral microbleed is remnant of blood extravasations from the damaged vessels related to cerebral microangiopathies. We report a patient, having used aspirin for secondary stroke prevention, who had an acute cerebellar hemorrhage after taking nattokinase 400 mg daily for 7 consecutive days. In addition to the hemorrhagic lesion, multiple microbleeds were demonstrated on brain MR images. We suggest that nattokinase may increase risk of intracerebral hemorrhage in patients who have bl

Identification of flavonoids from natural products against hyperlipidaemia through computational approaches.

Over the centuries, hyperlipidaemia has been recognised as a major risk factor contributing to cardiovascular disease (CVD). Current research increasingly explores natural compounds such as flavonoids for their potential lipid-lowering effects. This study evaluated pharmacological potential of flavonoids to identify strong inhibitors for target receptor, Proprotein Convertase Subtilisin/Kexin Type (PCSK9). A total of 29 compounds, including one FDA-approved drugs were screened for drug-likeness and ADMET properties, followed by molecular docking and Molecular Dynamics (MD) simulations using PC

Effects of PCSK9 inhibitors on coronary atheroma morphology in patients with coronary artery disease treated with statins: a meta-analysis and systematic review.

BACKGROUND: Proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9i) reduce adverse cardiovascular events when added to statins, but their effects on coronary plaque morphology remain inadequately defined. METHODS: We searched PubMed via MEDLINE, Cochrane CENTRAL, Scopus, Web of Science for randomized controlled trials comparing the effects of PCSK9i plus statins versus statins alone on coronary atheroma morphology in patients with coronary artery disease, including total atheroma volume (TAV), percent atheroma volume (PAV), fibrous cap thickness (FCT), minimal lumen area (MLA), maxi

PCSK9 in the liver metastatic niche: From cholesterol homeostasis to tumor microenvironment reprogramming.

Liver metastasis is a major determinant of cancer mortality, driven not only by tumor-intrinsic traits but also by dynamic reprogramming of the hepatic tumor microenvironment. Proprotein convertase subtilisin/kexin type 9 (PCSK9), classically known for its role in cholesterol metabolism, has recently emerged as a key regulator of this process. Beyond hepatocyte lipid homeostasis, PCSK9 influences multiple liver-resident and infiltrating cell populations, including sinusoidal endothelial cells, hepatic stellate cells, macrophages, and lymphocytes, collectively promoting immune evasion, angiogen

Gene Silencing of ANGPTL3 Induces PCSK9: Exploring the Biological Significance in the Hepatoma Huh7 Cell Line.

BACKGROUND: Angiopoietin-like 3 (ANGPTL3) and proprotein convertase subtilisin/kexin type 9 (PCSK9) are key regulators of lipid homeostasis. We have previously shown that gene silencing of ANGPTL3 significantly induces PCSK9 expression in the human hepatoma cell line Huh7. Here, we investigated the biological significance of this regulation in the cultured human hepatoma cell line Huh7. METHODS: We performed an RNA-seq analysis in Huh7 cells transfected with siRNA-ANGPTL3, siRNA-PCSK9, and double siRNA-ANGPTL3/PCSK9. Selected findings were assessed by RT-qPCR, Western blotting, and ELISA. RE

PCSK9-Mediated Inflammation in Foam Cell Formation and Exploring the Biologically Active Compounds Derived From Natural Resources.

Atherosclerosis develops as a persistent inflammatory condition that gradually alters the vascular structure and condition, in which the formation of macrophage-derived foam cells plays a central role in disease initiation and progression. Although proprotein convertase subtilisin/kexin type 9 (PCSK9) is widely recognised for its involvement in low-density lipoprotein receptor regulation and systemic lipid homeostasis, growing evidence indicates that PCSK9 also exerts a direct influence on macrophage inflammatory responses and intracellular lipid metabolism. Within atherosclerotic lesions, PCS

Optimal LDL-C Levels for Ischemic Stroke Prevention: A Neurologist's Perspective.

Low-density lipoprotein cholesterol (LDL-C) lowering is a central therapeutic strategy in atherosclerotic cardiovascular disease (ASCVD) and is also important for ischemic stroke. As outcome-driven evidence has accumulated, lipid management guidelines have progressively adopted more stringent LDL-C targets. Landmark trials of statins, ezetimibe, and proprotein convertase subtilisin/kexin type 9 inhibitors have shown consistent cardiovascular benefit across progressively lower achieved LDL-C levels in ASCVD. In the stroke field, randomized controlled trials, including Stroke Prevention by Aggre

Injectable Lipid-Lowering Therapies in Advanced Chronic Kidney Disease: Efficacy, Safety and the Dialysis Paradox.

Cardiovascular (CV) disease represents the leading cause of morbidity and mortality in patients with chronic kidney disease (CKD), particularly in those with advanced stages and dialysis dependence. Dyslipidemia is highly prevalent in this population and displays distinct features, substantially affected by multiple factors such inflammation and dialysis modality. Although statins remain the cornerstone of lipid-lowering therapy in earlier CKD stages, their benefit becomes less certain in advanced disease and predominantly end-stage kidney disease. This phenomenon, termed the dialysis paradox,

Signaling Demand with Cost-Effectiveness Analysis: A Case Study of PCSK9 Inhibitors.

OBJECTIVES: Healthcare decision-makers increasingly account for heterogeneity in cost-effectiveness across patient populations. We aimed to characterize the demand curve implied by this heterogeneity. This framework was applied in an analysis of proprotein convertase subtilisin/kexin type 9 inhibitor (PCSK9-I) therapy for Scottish adults with familial hypercholesterolemia (FH). METHODS: We showed how individual and subgroup level treatment value can be estimated by combining incremental health benefits and non-drug costs with a cost-effectiveness threshold, then annualized to obtain value-bas

A composite hydrogel enables the spatiotemporal delivery of distinct cytokines to drive the native vascularized bone regeneration.

Bone defect represents one of the most prevalent clinical conditions in orthopedics diseases, and the key to successful therapeutic outcomes lies in achieving early vascularization coupled with late-phase osteogenic differentiation. Growth factor delivery is among the most widely adopted strategies for bone tissue regeneration. However, achieving the spatiotemporal coupled regulation of vascularized bone regeneration remains a major challenge. Proprotein convertase subtilisin/kexin type 9 (PCSK9) exerts a pivotal role in bone metabolism. Our preliminary findings demonstrated that PCSK9 express

Biologics for cardiovascular diseases: from bench to bedside.

The rise of biologics, including recombinant proteins, gene therapies, and cell therapies, is reshaping the landscape of modern therapeutics, offering new strategies to address previously "undruggable" targets. Cardiovascular diseases (CVDs), the leading cause of mortality worldwide, remain inadequately managed by traditional therapies, but biologics offer a paradigm shift from symptom control to disease modification. This review provides a comprehensive analysis of biologics in cardiovascular medicine, focusing on five key biological processes: cardiac regeneration, cardiac reverse remodeling

Gene Therapy for Lipid Management: Dawn of a New Era.

Dyslipidemia is a major driver of atherosclerotic cardiovascular disease, with elevated low-density lipoprotein cholesterol (LDL-C) and triglycerides playing key roles in disease progression. Although conventional lipid-lowering therapies such as statins, ezetimibe, bempedoic acid, and proprotein convertase subtilisin/kexin type 9 inhibitors are effective, their impact is often limited by lifelong treatment requirements, suboptimal adherence, and inadequate achievement of lipid targets, particularly in patients with inherited disorders such as familial hypercholesterolemia. Recent advances in

PCSK9 as a mediator of monocytes transendothelial activation in obstructive sleep apnoea: A translational study.

Obstructive sleep apnoea (OSA) causes intermittent hypoxia (IH) during sleep which is the main trigger for associated cardiovascular complications. IH increases endothelial permeability and vascular remodelling. The proprotein convertase subtilisine kexine type 9 (PCSK9) is capable of promoting vascular inflammation. This study aims at characterizing PCSK9 expression in OSA patients and cellular models exposed to IH for unravelling PCSK9 role in monocytes migration and endothelial permeability. PCSK9 levels were evaluated by ELISA in serum from 48 OSA patients without comorbidities and 27 cont

PCSK9 Inhibitors Reduce Oxidative Stress Biomarkers in Heterozygous Familial Hypercholesterolemia.

Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors, such as alirocumab and evolocumab, effectively reduce LDL-C levels, improve cardiovascular outcomes, and are well tolerated in the treatment of heterozygous familial hypercholesterolemia (HeFH). Oxidative stress, increased in HeFH, leads to DNA damage, LDL oxidation (ox-LDL), and reduced total plasma antioxidant capacity, which promotes the development of atherosclerosis. The aim of the study was to assess whether treatment with PCSK9 inhibitors reduces ox-LDL, anti-ox-LDL antibodies, oxidative stress, and DNA damage, including
