Plastic surgery management of soft tissue loss in meningococcal septicemia: experience of the Melbourne Royal Children's Hospital.

Soft tissue necrosis associated with meningococcemia is a major challenge for any pediatric plastic surgery service. Records of patients treated by the Department of Plastic and Maxillofacial Surgery, Royal Children's Hospital, Melbourne, Australia, were reviewed. Two hundred fifty patients were treated for meningococcemia at our institution over a 40-year period. Of these, 31 patients suffered soft tissue necrosis. Three groups were identified: lesions that healed with nonoperative management (n = 12); those that required skin grafting, flaps, or minor amputations (n = 14); and those requirin

Closure of the abdominal cavity after severe peritonitis in bariatric surgery utilizing a mesh and plastic device.

The major cause of peritonitis in bariatric surgery is leakage of GI contents, which can have a catastrophic outcome for the bariatric patient. To resolve this serious problem, the surgeon must act quickly. This paper describes a 27-year-old female after gastric bypass with disruption of the gastroenterostomy and severe contamination and peritonitis. Closure of the anastomotic leak, drainage, and gastrostomy in the bypassed stomach were performed, but the abdomen could not be closed, due to dilated bowel and the intra-abdominal edema with the sepsis. Temporary laparostomy closure was performed

Comparison of diagnostic accuracy for cutaneous malignant melanoma between general dermatology, plastic surgery and pigmented lesion clinics.

BACKGROUND: Since the 1980s there have been dedicated pigmented lesion clinics (PLCs) in the U.K. Important considerations when comparing the efficacy of the PLC with other referral clinics include diagnostic accuracy. OBJECTIVES: To compare the false-negative rate of clinical diagnosis (FNR) in the PLC with that in the other clinics of primary referral of malignant melanoma (MM) in the same geographical area. We have previously shown that certain clinical features are risk factors for diagnostic failure of MM. A further aim of this study was to correct for any differences in frequency of the

Urinary tract infections and asymptomatic bacteriuria after vaginal plastic surgery. A comparison of suprapubic and transurethral catheters.

In a prospective randomized study comprising 90 women undergoing vaginal plastic surgery, suprapubic and transurethral catheter drainage of the bladder were compared regarding urinary tract infection and asymptomatic bacteriuria. No statistically significant differences were found. The mean duration of catheterization was 4.9 days for suprapubic vs 3.3 days for transurethral catheter patients. Postoperative urinary tract infection was diagnosed in 23.7% of patients with suprapubic and in 27.5% of patients with transurethral catheters. Asymptomatic bacteriuria at catheter removal was found in 2

[Early and late results of Stoeckel-Kraatz's modified loop plastic surgery in stress incontinence in the female].

Stress incontinence of women is a widespread and sophisticated problem. Its incidence is between five and 20 per cent in medium and advanced age. In the last two years, 34 patients received treatment modified after the sling operation which had been inaugurated by Stoeckel and Kraatz. This surgical approach had been reserved for severe forms and recurrences of urinary incontinence. The surgical technique is described in this paper. Surgical action action had been preceded by complex gynaecological, neurological, and urological examinations. Sixteen patients have been without complaint for more

The Role of Vitamin D in Plastic and Reconstructive Surgery: A Systematic Review of Human and Animal Studies.

BACKGROUND: Vitamin D, a fat-soluble hormone, is increasingly recognized for its critical roles in immunomodulation, inflammation control, and tissue regeneration. Its deficiency is a global public health concern, affecting postoperative outcomes across various surgical fields, including plastic and reconstructive surgery. OBJECTIVE: To systematically review the existing literature on the impact of preoperative and perioperative vitamin D levels on surgical outcomes in plastic and reconstructive surgery, combining findings from both animal and human studies. METHODS: A detailed literature se

Pathophysiology of nicotine, place of nicotine substitutes and electronic cigarettes in plastic surgery: A review of the literature.

OBJECTIVE: Smokers are more likely to undergo postoperative complications after plastic surgeries. Our main purpose was to update nicotine's effects after plastic surgeries and assess whether nicotine-replacement therapy and vaping are safe enough to be recommended in peri-operatory circumstances. METHODS: We set up a literature review including 40 documents from 1986 to 2020 available on Pubmed. RESULTS: Nicotine has undeniable detrimental effects on patients undergoing plastic surgeries like cutaneous necrosis, skin flap failure and surgical site infection. Nevertheless, this toxicity seem

[Plastic surgery of the thumb, using the epiploon. The problem of integrating mechanical hand prostheses into the body].

The techinque using epiloon tissue in hand surgery was introduced by the author in 1974. The efficiency of the method was checked in several cases of radionecrosis of the hand, deep burns with complete destruction of the tissues on the back of the hand, severe trauma with tearing of the teguments and uncovering of the bones. Epiploonoplasty of the hand achieves a massive transfer of thick epiplooic tissue which is well vascularized, rapidly integrated in the circulatory system of the hand. An extensive description of the surgical technique is presented. A case of severe trauma with amputation

The role of neuronal plasticity in cervical spondylotic myelopathy surgery: functional assessment and prognostic implication.

Cervical spondylotic myelopathy (CSM) is a degenerative disease representing the most common spinal cord disorder in the adult population. It is characterized by chronic compression leading to neurological dysfunction due to static and dynamic injury of the spinal cord in cervical spine. These insidious damage mechanisms can result in the reorganization of cortical and subcortical areas. The cerebral cortex can reorganize due to spinal cord injury and may play a role in preserving neurological function. To date, the gold standard treatment of cervical myelopathy is surgery, comprising anterior

Plasticity and function of human skeletal muscle in relation to disuse and rehabilitation: Influence of ageing and surgery.

In order to study the influence of disuse and aging on skeletal muscle homeostasis, different human models were employed. Effects of chronic disuse were investigated in elderly patients suffering from uni-lateral hip-osteoarthritis, whereas the effect of short-term disuse (4 and 14 days of unilateral lower limb immobilisation) was assessed in healthy young and old individuals. In summary, chronic muscle disuse in the elderly was associated with marked quantitative as well as qualitative neuromuscular impairments. More specifically, decreases were observed in muscle strength, quadriceps muscle

High-frequency cortical subdural stimulation enhanced plasticity in surgery of a tumor in Broca's area.

Functional areas located near or within brain gliomas prevent the complete resection of these tumors. It has recently been described that slow tumor invasion promotes neural reorganization, and even topographic plasticity, allowing a staged resection of those tumors. Thus, our aim was to promote plasticity by mimicking the tumor's capability to displace brain function. This proceeded through the production of a 'virtual lesion' in eloquent areas within a tumor using continuous high-frequency cortical electrical stimulation (cHFCS). An anaplastic astrocytoma located in Broca's area progressed i
