Regulation of sodium, calcium and vitamin D metabolism in Dahl rats on a high-salt/low-potassium diet: genetic and neural influences.

1. A dietary combination of high salt and low potassium (HSLK) exacerbates hypertension in Dahl salt-sensitive (DS) rats and renders previously normotensive Dahl salt-resistant (DR) rats hypertensive. In both strains, the severity of hypertension correlates with urinary calcium loss. However, the magnitude of excretory calcium losses is significantly greater in DS rats and is potentiated by chemical sympathectomy in both strains. 2. We hypothesized that a defect in vitamin D metabolism may underlie the observed strain-dependent differences in calcium balance. 3. Arterial blood pressure (ABP), …

Part II: The treatment of primary and secondary Raynaud's phenomenon.

Raynaud phenomenon (RP) presents with either primary or secondary disease, and both have the potential to negatively impact patient quality … ical ischemia or those who fail pharmacologic therapy include botulinum toxin injection and digital sympathectomy. Herein, we describe in detail the treatment options for patients with RP as well as provide treatment algorithms for each RP subtype.

Pharmacologic and interventional paradigms of diuretic resistance in congestive heart failure: a narrative review.

Diuretic volume reduction continues to be the mainstay of congestive heart failure (CHF) management globally. However, diuretic resistance i … n blockade and Elabela-APJ axis targeting are reviewed, as well as percutaneous visceral splanchnic sympathectomy (converting a volume-focused to a distribution-focused paradigm).

Perioperative management of patients with arrhythmogenic syndromes refractory to medical treatment: A clinical case report.

Family heart diseases related to sudden death are a group of cardiovascular diseases (cardiomyopathies, channelopathies, aortic diseases …) that require familiarity of the anesthesiologist with the perioperative treatment of complex hemodynamic disorders, as well as their surgical treatment.1 We present the case of a 12-year-old man diagnosed with non-obstructive hypertrophic cardiomyopathy (MHNO), after cardiorespiratory arrest (PCR), who underwent video-guided thoracoscopy-guided left sympathectomy (VATS) for frequent syncope, despite pharmacological treatment and implantation of an implanta…

Autonomic control of intercellular communication in salivary glands.

Effects of autonomic agonists, antagonists and denervation on intercellular communication between cells in acini of rat submandibular glands were investigated to clarify the regulation and maintenance of intercellular communication by the autonomic nervous system. Addition of acetylcholine (ACh) and adrenaline rapidly and reversibly suppressed the dye-coupling between acinar cells. The suppressive effects of adrenaline were weaker than those of ACh. Antagonists inhibited the above effect of agonists. Electrical coupling between acinar cells decreased 2 weeks after parasympathectomy, but sympat…

Inhibition of gastric acid secretion by intracerebral injection of calcitonin gene related peptide in rats.

Calcitonin gene related peptide (CGRP), a 37 amino acid peptide recently characterized from the rat brain, injected into the cisterna magna or the lateral hypothalamus, inhibited gastric acid secretion and increased serum gastrin levels in conscious pylorus-ligated rats. CGRP suppressed pentagastrin- and histamine-induced stimulation of gastric secretion in urethane-anesthetized gastric fistula rats. Peptide action was dose-dependent, not modified by adrenalectomy and peripheral sympathectomy induced by guanethidine pretreatment and reversed by vagotomy. These results demonstrate that intracis…

Hyperhidrosis--causes and treatment of enhanced sweating.

BACKGROUND: Basically two types of sweating exist: thermoregulatory and emotional sweating. They are controlled by different centers: thermo … oresis, and the intracutaneous injection of botulinum toxin. Surgical approaches include endoscopic sympathectomy and axillary tumescent curettage and liposuction, removing the sweat glands. Systemic drugs (e.g. anticholinergic substances) can be used in the treatment of generalized hyperhidrosis. CONCLUSION: A step-by-step approach is recommended for the trea …

Side effects and complications of surgery for hyperhidrosis.

Most of the difficulties associated with hyperhidrosis surgery are due to unavoidable side effects and unforeseeable and unacceptable complications. Careful patient selection is important before surgery so surgeons can avoid some of these pitfalls. Patients should also be fully informed of all potential side effects and complications before surgical treatment.

Placement of a Continuous Stellate Ganglion Block for Treatment of Refractory Ventricular Fibrillation in the Setting of Known Prinzmetal Angina During Pregnancy: A Case Report.

A patient with a history of Prinzmetal angina, refractory ventricular fibrillation, cardiac arrest with an implantable cardioverter-defibrillator, and obesity presented to the emergency department at 17 weeks gestational age with a chief complaint of angina and multiple episodes of defibrillation. A T3/4 thoracic epidural was placed to assess the effectiveness of a partial chemical sympathectomy in alleviating symptoms of angina as well as decreasing the amount of defibrillation episodes. Once this proved to be beneficial in accomplishing both of these goals, a more specific approach was desig…

Microwave Treatment for Axillary Hyperhidrosis and Bromhidrosis.

Axillary hyperhidrosis (AH) and bromhidrosis are common causes of consultation in dermatology. Currently, the most widely prescribed treatment for AH is botulinum toxin, a very effective but temporary option; it is totally ineffective in bromhidrosis. Sympathectomy is an increasingly infrequent choice of treatment due to the high incidence of compensatory hyperhidrosis. We describe the treatment of AH and bromhidrosis with a novel microwave device that can fibrose eccrine and apocrine glands, achieving possibly permanent results. The procedure should preferably be performed under tumescent ane…

Critical upper limb ischemia caused by initially undiagnosed thoracic outlet syndrome--case report.

A case of a progressive ischemia of the right upper extremity in a 57-year-old male caused by primarily undiagnosed compression of a subclavian artery by an accessory cervical rib is presented. Critical limb ischemia persisting for 7 years despite conservative treatment and a thrombectomy of upper extremity arteries and thoracic sympathectomy, led to a loss of three fingers and development of a non-healing ulceration of right upper extremity. Eventually a cervical rib resection together with a subclavio-brachial venous bypass graft permanently reversed the critical limb ischemia and allowed th…

Use of iontophoresis or phonophoresis for delivering onabotulinumtoxinA in the treatment of palmar hyperidrosis: a report on four cases.

Idiopathic palmar hyperhidrosis is a common disease that exerts a considerable effect on patients' quality of life. The definitive treatment of this condition (sympathectomy) is associated with some adverse effects and surgical risks. Drugs such as onabotulinumtoxinA can be percutaneously delivered by phonophoresis or by iontophoresis and may be useful in the treatment of hyperhidrosis. In this paper, the authors describe four cases in which an objective and subjective improvement in sweating was seen following 10 daily sessions of phonophoresis or iontophoresis. No adverse effects were report…
