Ultrasonography applications in diagnosis and management of early rheumatoid arthritis.

Ultrasonography is an elegant tool for the detection of tenosynovitis, synovitis, and erosions very early in rheumatoid arthritis, and the presence of a power Doppler signal is one of the best predictors of joint damage. Although clinical scores remain the mainstay of disease activity assessment, ultrasonography has proved to be a remarkably robust tool for reliable assessment of changes in rheumatoid arthritis. There is no evidence to suggest that problems with operator dependence would be greater than with other imaging modalities or physical examination, if performed by trained providers.

Tarsal tunnel syndrome: ultrasonographic and MRI features.

Tarsal tunnel syndrome is a well-known but rare entrapment neuropathy involving the posterior tibial nerve in the tarsal tunnel, a fibro-osseous channel extending from the medial aspect of the ankle to the midfoot. Posttraumatic fibrosis, ganglion cyst, tenosynovitis, tumor of the nerves or other structures, dilated or tortuous veins can cause significant nerve compression in this anatomic region. Herein, we present the typical ultrasonographic and magnetic resonance features of this disorder in patient with a ganglion cyst.

Imaging of acute musculoskeletal infections in children and their differential diagnoses.

Musculoskeletal complaints are a frequent cause of emergency department visits in pediatric patients. While many of these visits are due to acute trauma, infection is also common and can cause substantial morbidity. This article provides a review of acute pediatric musculoskeletal infections with an additional emphasis on important differential diagnoses to consider. The scope includes cellulitis, abscess, necrotizing soft tissue infections, pyomyositis, infectious tenosynovitis, septic bursitis, septic arthritis, and osteomyelitis. Important differential considerations for these musculoskelet…

How to perform Baxter's nerve blockage easily by ultrasound in chronic heel pain: Four simple steps.

Baxter's neuropathy is one of the overlooked causes of chronic heel pain. Diagnosing neuropathy in Baxter's can be challenging due to its potential occurrence as a secondary condition to other common syndromes that cause heel pain, such as plantar fasciitis, calcaneal spur, hypertrophic muscle, tenosynovitis, space-occupying lesions or trauma. Ultrasound is a reliable and easily accessible device that guides injections for the treatment of Baxter's neuropathy. We have written this letter as a guide, especially for beginner and professional pain specialists.

Clinical manifestations of Whipple's disease mimicking rheumatic disorders.

Whipple's disease is a rare, chronic, systemic disorder caused by Tropheryma whipplei infection. The most common symptoms are weight loss, arthralgia, diarrhea and abdominal pain. Other organ involvement can also occur in the patients. Joint manifestations may mimic rheumatoid arthritis or spondyloarthritis. Arthalgia, arthritis, spondylodiscitis, bursitis and/or tenosynovitis are seen in the majority of the patients. This explains why some of the symptoms are misdiagnosed as those of rheumatic diseases. Understanding of Whipple's disease is important for differential diagnostics of several rh…

[Intersection syndrome in a tetraplegic hand cyclist].

Wrist tendinopathies are common overuse injuries in athletes and often associated with activities such as rowing, riding or racket sports. Correctly diagnosing the various tendinopathies is essential for management of cases refractory to initial treatment. Easily mistaken for de Quervain's tenosynovitis, the intersection syndrome may be effectively managed by simple taping negating the need for corticosteroid injection and tendon release surgery. We present an unusual case report of intersection syndrome in a tetraplegic hand cyclist following a 24-hour endurance race.

Endoscopic Synovectomy of the Ulnar Bursa of the Palm of the Hand.

Synovitis of the ulnar bursa is associated with tenosynovitis of the flexor tendons. Reported causes include suppurative or tuberculous infection, rheumatoid arthritis, and pigmented villonodular synovitis. Open synovectomy requires extensive soft-tissue dissection and may result in extensive scarring. The purpose of this Technical Note is to describe the technical details of endoscopic synovectomy of the ulnar bursa of the palm of the hand. This minimally invasive approach with small incisions allows aggressive postoperative hand therapy and avoids the complications of tendon adhesions, joint…

Arthroscopic Management of Posteromedial Ankle Impingement.

Posteromedial ankle impingement is a rare clinical entity. It usually follows an inversion injury, with compression of the posterior tibiotalar ligament between the medial malleolus and talus. This can be treated by posterior ankle endoscopy through the posteromedial and posterolateral portals. The flexor hallucis longus tendon can be examined for any tenosynovitis or tendinopathy. The posteromedial corner of the ankle joint is reached with the instruments staying on the lateral side of the flexor hallucis longus tendon. The inflamed synovium, scar tissue, and fibrillated cartilage are debride…

A long-term follow-up of Japanese mother and her daughter with Blau syndrome: Effective treatment of anti-TNF inhibitors and useful diagnostic tool of joint ultrasound examination.

Blau syndrome (BS) is an autosomal dominant autoinflammatory disease associated with NOD2 gene mutations. It is characterized by arthritis, skin rash, and uveitis. Here, we report contrasting outcomes of a daughter and her mother with BS. Their long-term follow-up revealed the efficacy of anti-tumor necrosis factor inhibitor (TNF) with respect to BS. Joint findings of BS feature tenosynovitis over articular synovitis on ultrasonography. BS might be one of the differential diagnoses of juvenile idiopathic arthritis and rheumatoid arthritis.

Three new cases of palmar fasciitis with polyarthritis including a patient with atypical mutilating osteolysis.

We managed three patients who had both palmar fasciitis and polyarthritis coexisting with gynecological malignancies. The first patient was followed up for 3 years, during which she experienced mutilating osteolysis of the fingers and very severe inflammation as assessed by osteoarticular ultrasonography. The second patient had active synovitis by ultrasonography without structural joint damage. Finally, in the last patient, magnetic resonance imaging showed synovitis and extensor tenosynovitis of the fingers. The many differential diagnoses of this paraneoplastic syndrome are discussed.

The clinical entity of remitting seronegative synovitis with pitting edema (RS3PE syndrome).

Remitting seronegative symmetrical synovitis with pitting edema is a clinical entity distinct from polymyalgia rheumatica arthritis appearing in elderly patients. Distal pitting edema and tenosynovitis of the digitorum tendons are characteristically seen in this disorder. In contrast to polymyalgia rheumatica and rheumatoid arthritis, treatment with low-dose glucocorticoids usually produces a prompt and enduring remission after several months. Follow-up evaluations are necessary not only to assure this diagnosis is posterior, but also to observe for the possible occurrence of associated neopla…

Resection of the common digital extensor tendon in a gelding.

A four-year-old gelding was lame owing to a chronic septic common digital extensor tendon and sheath. The horse had been treated by open surgical lavage but the sepsis had recurred after three months. Physical, ultrasonographic, cytological and histological examinations confirmed chronic septic tenosynovitis and tendonitis. The entire intrathecal component of the common digital extensor tendon was resected under general anaesthesia and the synovial lining of the sheath was ablated. Postoperatively the horse regained good limb function and became sound.

Magnetic resonance imaging in the diagnosis of PMR.

The cause of musculoskeletal symptoms in polymyalgia rheumatica (PMR) is not clearly defined because joint synovitis may only partially explain the diffuse discomfort. MRI imaging of the shoulders, hip and extremities of patients with PMR has been analyzed. MRI showed that subacromial and subdeltoid bursitis of the shoulders and iliopectineal bursitis and hip synovitis are the predominant and most frequently observed lesions in active PMR. The inflammation of the bursae associated with glenohumeral synovitis, bicipital tenosynovitis and hip synovitis may explain the diffuse discomfort and morn…

Acute attenuation of the extensor pollicis longus tendon. A case report.

The current understanding of tendon biomechanics indicates that indirect injury to the tendon midsubstance requires the presence of preexisting disease during mechanical overload. This belief has been substantiated by the association of extensor pollicis longus rupture with chronic tenosynovitis caused by repetitive activity, inflammatory conditions such as rheumatoid arthritis, and minimally displaced distal radius fractures. This case of acute, traumatic, intratendinous attenuation of the extensor pollicis longus tendon offers a contradiction to the view that midsubstance tendon failure requ…

A severe infection following endoscopic carpal tunnel release.

A 59-year-old patient underwent endoscopic carpal tunnel release by Chow's two-portal technique. He developed a pyogenic tenosynovitis and an infection within the ulnar and radial bursae, an abscess in the middle palmar, thenar and Parona's space, and a pyogenic wrist arthritis. Surgical treatment included a wide exposure of the infected region, débridement, irrigation, application of a resorbable collagen sponge containing gentamicin, insertion of two drains and primary wound closure. The infection was brought under control and hand function restored.

[Measurement of carpal tunnel pressures in the idiopathic carpal tunnel syndrome without motor deficit].

The pressure in the carpal tunnel was measured in ten patients suffering from purely painful carpal tunnel syndrome. Five normal individuals were also investigated as controls. It was shown that the pressure was always high in involved patients. A rise of pressure on flexion and extension of the wrist was much greater in patients than in controls. It is concluded that true compression plays the main role in carpal tunnel syndrome. A localised tenosynovitis was constantly found at operation. It was responsible for the rise of pressure inducing a functional ischaemia of the nerve.
