Dental effects of enzyme replacement therapy in case of childhood-type hypophosphatasia.

BACKGROUND: Hypophosphatasia (HPP), a skeletal disease characterized by hypomineralization of bone and teeth, is caused by an ALPL gene mutation that leads to low activity of the tissue non-specific alkaline phosphatase enzyme. Although enzyme replacement therapy (ERT) was recently introduced for affected patients, no known studies have been reported regarding its dental effects related to permanent teeth and jaw bones. In the present study, we examined the dental effects of ERT in a case of childhood-type hypophosphatasia, including panoramic radiography findings used to estimate the dental a

Survival rate of teeth with periodontally hopeless prognosis after therapies with intentional replantation and perioprosthetic procedures - a study of case series for 5-12 years.

The purpose of the present study was to evaluate the longitudinal survival rate of the treatment of teeth affected with periodontally hopeless prognosis and secondary occlusal traumatism (SOT) using intentional replantation (IR) and periodontal prosthesis. We collected data from 17 individuals who received IR and participated in the study during 1995 to 2014. Of the 17 teeth affected by periodontally extreme conditions with deep angular bone defects, severe alveolar bone loss extending to or beyond the apex, and SOT, was recognized as having hopeless prognosis. Those teeth were treated sequent

Comparative Evaluation of the Anesthetic Efficacy of Needle-Free Anesthesia and Conventional Anesthesia in Patients With Symptomatic Irreversible Pulpitis: A Randomized Clinical Trial.

Introduction Pain is the primary reason for which most of the patients seek endodontic treatment. Local anesthesia is considered to be the most important step in the procedure to reduce the pain. However, the majority of the patients do not cooperate due to the fear of syringe anesthesia. The aim of this clinical trial was to compare the anesthetic efficacy of needle-free anesthesia and conventional anesthesia in patients with symptomatic irreversible pulpitis undergoing root canal therapy. Materials and methods A total of 54 patients were enrolled in the study, and the treatment was administe

[Prognosis for the deciduous molars of juvenile dogs after root canal filling using apatite root sealer].

There are various restrictions on the root canal filling of deciduous roots after root canal treatment unlike in the case of permanent teeth, because deciduous molars undergo physiological root resorption with aging. Among the restrictions, it is most important that the root canal filling material be similarly absorbed and disappear along with the deciduous root so that it does not prevent the eruption of the permanent successors. Hardly any of the root canal filling materials currently on the market, however, are suitable for deciduous molars because some of them disappear earlier than the de

Accuracy of panoramic radiography, periapical radiography, and CBCT in diagnosing apical root resorption.

This study evaluated the accuracy of digital panoramic radiography, digital periapical radiography, and cone-beam computed tomography (CBCT) in diagnosing apical root resorption (ARR) using 85 extracted teeth from dry human skulls and mandibles. Four ARR levels were simulated: mild, moderate, severe, and extreme. CBCT scans, panoramic radiographs, and periapical radiographs were obtained before and after ARR simulation. Three examiners independently evaluated all images to determine ARR and, when present, to classify its severity level. Inter- and intra-observer agreement was calculated using

Hydration characteristics of calcium silicate cements with alternative radiopacifiers used as root-end filling materials.

INTRODUCTION: Mineral trioxide aggregate (MTA) is composed of calcium silicate cement and bismuth oxide added for radiopacity. The bismuth oxide in MTA has been reported to have a deleterious effect on the physical and chemical properties of the hydrated material. This study aimed to investigate the hydration mechanism of calcium silicate cement loaded with different radiopacifiers for use as a root-end filling material. METHODS: Calcium silicate cement loaded with barium sulfate, gold, or silver/tin alloy was hydrated, and paste microstructure was assessed after 30 days. In addition, atomic

Clinical and radiographic evaluation of vital pulp therapy in open apex teeth with MTA and ZOE.

The purpose of this study was to report the success rate of using mineral trioxide aggregate (MTA) and zinc oxide eugenol (ZOE) as vital pulp therapy agents in immature permanent teeth with carious or traumatic pulp exposure. Subjects were children with permanent teeth requiring pulpotomy (apexogenesis) and without systemic diseases. Both ZOE and MTA treatments as pulpotomy agents showed clinical and radiographic success in immature permanent teeth. Although MTA was more successful, it is more expensive, and dentin bridges may develop over canal orifices, complicating future root canal therapy

Molecular characterization of the microbial flora residing at the apical portion of infected root canals of human teeth.

INTRODUCTION: This study investigated the bacterial communities residing in the apical portion of human teeth with apical periodontitis in primary and secondary infections by using a culture-independent molecular biology approach. METHODS: Root canal samples from the apical root segments of extracted teeth were collected from 18 teeth with necrotic pulp and 8 teeth with previous endodontic treatment. Samples were processed for amplification via polymerase chain reaction and separated with denaturing gradient gel electrophoresis. Selected bands were excised from the gel and sequenced for ident

Sealer penetration and apical microleakage in smear-free dentin after a final rinse with either 70% isopropyl alcohol or Peridex.

Tensioactive agents may alter dentinal wettability allowing increased sealer penetration into dentinal tubules. This may improve the apical seal and better entomb remaining bacteria. The purpose of this study was to determine if a final rinse with either 70% isopropyl alcohol or Peridex in instrumented, smear-free canals would affect apical microleakage or Roth's 801 sealer penetration into dentinal tubules. Sixty root segments were instrumented followed by smear layer removal. The segments were randomly divided into three groups receiving a final rinse with 6% sodium hypochlorite (NaOCl), 70%

Complications of sinus perforations and their management in endodontics.

The intimate position of the maxillary sinus to the apices of teeth creates problems if periapical inflammation occurs. This can lead to a perforation into the sinus and cause sinusitis. Persistent and complicated situations that do not respond after nonsurgical root canal treatment demand a suitable surgical therapy. Knowledge of the specific anatomic conditions, an adequate diagnosis, and an appropriate surgical procedure facilitate success rates that are comparable with those obtained in other regions, even in unfavorable initial conditions.

Pulp revascularization after root canal decontamination with calcium hydroxide and 2% chlorhexidine gel.

INTRODUCTION: Pulp revascularization may be considered a promising alternative for necrotic immature teeth. Many studies have accomplished passive decontamination associated with an antibiotic paste. To date, there is no report evaluating calcium hydroxide associated with 2% chlorhexidine gel for revascularization therapy. The aim of this case report was to describe a new proposal for pulp revascularization with mechanical decontamination and intracanal medication composed of calcium hydroxide and 2% chlorhexidine gel. METHODS: The patient, a 9-year-old girl, suffered an intrusion associated

A hybrid approach to predicting and classifying dental impaction: integrating regularized regression and XG boost methods.

INTRODUCTION: Dental impaction is a significant clinical challenge that requires advanced predictive modeling and healthcare analytics approaches. Impaction, a tooth alignment issue, is diagnosed using radiographic measurements like panoramic radiographs and CBCT. Artificial Intelligence (AI) is improving the accuracy of predicting dental impaction. Advanced predictive models like logistic Regression and XGBoost analyze critical variables, identify patterns, and perform predictive analysis. These models can identify potential impactions, assess impaction type, and develop treatment plans. Inte

Apical barrier formation after incomplete orthograde MTA apical plug placement in teeth with open apex--report of two cases.

Two cases are reported in which incomplete placement of 4 mm mineral trioxide aggregate (MTA) plug was performed unintentionally at the apical third of permanent immature teeth with open apex and apical periodontitis. As confirmed radiographically, there were gaps between MTA and dentinal walls along the MTA-dentin interface. After setting of MTA was confirmed, endodontic treatment was completed and access was sealed with composite resin. At 6 to 16 months follow-up examinations, formation of dentin in contact with the MTA surface, as well as apical closure and periapical healing were ideied r

Micro-CT Assessment of Hydraulic Calcium Silicate Cements for Perforating Internal Resorption in 3D-printed Tooth Replicas at Different Root Thirds: An In Vitro Study.

INTRODUCTION: Effective repair of perforating internal resorption (PIR) requires adequate defect filling; however, the influence of defect location on the filling ability of hydraulic calcium silicate cements (HCSCs) remains unclear. This micro-computed tomography (micro-CT) study evaluated Bio-C Repair (BCR), Biodentine (BD), and white MTA-Angelus (WMTA) in three dimensional-printed tooth replicas with PIR defects in different root thirds. The null hypothesis was that material performance would not differ according to defect location. METHODS: A maxillary central incisor was instrumented to
