Hemifacial microsomia: extraoral appliance for the early treatment of an infant. Literature review and case report.

In this report a description of the different features of hemifacial microsomia is given and a new way of early treatment of concomitant res … n of mandibular growth was observed. It is claimed that the use of this appliance can help to avoid tracheotomy in many severe cases of uni- or bilateral facial microsomia with glossoptosis and apnea. This treatment should be considered as temporary until the general condition of the patient has improved and grafting is possible.

[Subglottic tracheal stenosis in Wegener's granulomatosis. Report of 3 cases].

We describe a 3 patients with Wegener's granulomatosis in remission while taking cyclophosphamide and prednisone who developed upper airway obstruction. The diagnosis was confirmed by parotid gland, pharynx, kidney biopsy. The stenosis developed in the area of previously noted tracheal ulceration and responded satisfactorily to bronchoscopic dilatation. Tracheotomy for progressive upper airway obstruction has remained the mainstay of treatment and was necessary in one of cases. We presented the others techniques of treatment subglottic stenosis-the surgical intervention and carbon dioxide lase…

Perioperative Management of a Patient With Tongue Cancer Who Developed Pneumomediastinum Following Tracheostomy Performed to Secure the Airway.

Prior to a scheduled operation for a 45-year-old male patient with tongue cancer, a tracheotomy performed under intravenous sedation to prevent asphyxia due to extensive bleeding resulted in pneumomediastinum and subcutaneous emphysema. The planned operations were postponed until reduction of the pneumomediastinum was confirmed. During operation, airway pressure was kept low to prevent tension pneumomediastinum along with a sufficient depth of anesthesia, controlled analgesia, and continuous administration of muscle relaxants. Postoperatively, sedation was used to avoid stress and complication…

[An unusual cause of high dysphagia: schwannoma of the preepiglottic space].

Schwannomas are well encapsulated mesenchymal tumors of peripheral nerves, with slow growth. The laryngeal schwanomma is exceptional, we pre … h a benign tumor of the preepiglottic space. External surgical excision was performed and a primary tracheotomy was required. The histological examination with an immunochemical study confirmed a benign schwannoma. There was no complication postoperatively. The outcome was excellent after 2 years of follow-up.

Outcomes research in pediatric otolaryngology.

There are several validated outcomes instruments available for use in the pediatric otolaryngology population. These include instruments for otitis media, rhinosinusitis, tonsil and adenoid disease; sleep-disordered breathing, voice disorders and tracheotomy. In addition, several prospective outcomes studies have been performed on common pediatric otolaryngology conditions. These studies have typically indicated that the quality of life impact of diseases such as otitis media and recurrent tonsillitis is quite high, and that surgical treatments (such as tympanostomy tube placement and adenoton…

Acute apnea caused by an epiglottic cyst.

A life threatening complication in the course of routine sedation in a 5 year old child is reported in a case study. A retention cyst of the … on the child presented with stridor and acute apnea. Emergency orotracheal intubation could prevent tracheotomy but was complicated by the unexpected presence of a tumor at the base of the tongue. Further evaluation revealed a large epiglottic cyst. After endoscopic removal of the cyst no further episodes of apnea or stridor were noted.

Laryngeal tuberculosis: presentation of 16 cases and review of the literature.

Tuberculosis of the larynx was quite common in the past and usually associated with advanced pulmonary cavitations with an overall mortality … ulosis with no apparent pulmonary involvement. Four patients (25%) presented with stridor and had a tracheotomy performed for the relief of their upper airway obstruction. In contrast to earlier studies, our series showed great variations as far as location and appearance of the laryngeal lesions.

[Esophageal atresia, brachio-cephalic arterial trunk and tracheomalacia (author's transl)].

The case of 6 children operated for type III esophageal atresia with re-establishment of continuity is reported owing to the association with anterior tracheal compression by the brachio-cephalic arterial trunk. In the 5 cases, control radiography after the intervention showed a tracheotomy opposite the superior esophageal pouch and sometimes a notch in the anterior trachea. In all of the cases, endoscopy confirmed the presence of compression of the anterior aspect of the trachea by the brachio-cephalic arterial trunk. In 2 cases, esophageal transit was normal, the signs of tracheal compressio…

Methotrexate in the Management of Laryngotracheal Stenosis: A Narrative Review.

BACKGROUND: Laryngotracheal stenosis (LTS) involves narrowing of the proximal upper airway that can cause obstructed airflow and respiratory distress. LTS can be caused by intubation, previous tracheotomy, trauma to the neck, chemical inhalation, burn, inflammatory and autoimmune diseases, and other conditions. LTS can be chronic and recurrent. LTS can be life-threatening in some cases; so, it is important to understand all available treatment options. Methotrexate is a known treatment for autoimmune diseases, and given the autoimmune and inflammatory etiology of some cases of LTS, it has been…

Spinal Muscular Atrophy Mortality Despite Novel Medications: Case Reports.

Despite new effective medications, patients with spinal muscular atrophy types 1-3 can continue to have inadequate cough flows to prevent episodes of acute respiratory failure. Ventilator unweanable intubated patients are thought to require tracheostomy tubes. As a result, potentially beneficial medications may be discontinued and patients die despite receiving these medications. Three cases are presented of medically treated, physically strengthening children, with spinal muscular atrophy type 1. All three subsequently died or underwent tracheotomy. However, there is no evidence of extubation…

[Epiglottitis].

BACKGROUND: Acute epiglottitis in adults is a rare, potentially life-threatening condition caused by a bacterial infection in the epiglottis. Typical symptoms are fever, sore throat, and respiratory distress caused by upper airway obstruction. Proper treatment is needed for a good outcome. CASE PRESENTATION: We here present a 54-year-old female patient with acute epiglottitis. Her airway was secured by endotracheal intubation and she received antimicrobial therapy. She developed an abscess around the epiglottis that needed surgical drainage and tracheotomy. However, she fully recovered after …

Successful emergency management of a bleeding tracheoinnominate fistula.

In this case, we describe a novel approach to achieving temporary haemostasis in acute massive haemorrhage from a bleeding tracheoinnominate … loon on a Foley catheter that was introduced by direct laryngoscopy into the upper larynx above the tracheotomy stoma. This provided temporary control of the bleeding until definitive management through ligation of the innominate artery via median sternotomy.

Inhaled treprostinil via the Tyvaso Inhalation System through a tracheostomy.

A 20-year-old man with pulmonary arterial hypertension secondary to systemic sclerosis was admitted to our hospital. Prior to admission, his PAH had been successfully managed with the use of tadalafil, ambrisentan and inhaled Tyvaso. Owing to respiratory failure from vocal cord paralysis, he underwent an emergent tracheotomy. The delivery of inhaled Tyvaso through a tracheostomy tube was explored. Post-tracheostomy, the patient continued his ability to self-administer the medication. His WHO functional classification, brain natriuretic peptide levels, and echocardiograms were not significantly…

[Fiberoptic tracheal intubation through a laryngeal mask airway in a pediatric patient with treacher collins syndrome].

A 6-month-old girl with Treacher Collins syndrome was scheduled for tracheotomy because of severe airway obstruction. During slow induction of anesthesia with inhalation of sevoflurane, assisted mask ventilation was successfully performed using oropharyngeal airway. Either direct laryngoscope or GlideScope Video Laryngoscope could not reveal any part of the epiglottis (Cormack and Lehane grade 4). Even fiberoptic bronchoscopic examination assisted by GlideScope Video Laryngoscope gave a poor view of the pharynx on the video monitor. Laryngeal mask airway (LMA) was inserted easily and allowed a…
